Aim This update of the interdisciplinary S3 guideline on the Diagnosis, Therapy and Follow-up of Cervical Cancer (AWMF Registry No. 032/033OL) was published in March 2021. This updated guideline was funded by German Cancer Aid (Deutsche Krebshilfe) as part of the German Guideline Program in Oncology. The guideline was coordinated by the German Society of Gynecology and Obstetrics (Deutsche Gesellschaft für Gynäkologie und Geburtshilfe, DGGG) and the Working Group on Gynecological Oncology (Arbeitsgemeinschaft Gynäkologische Onkologie, AGO) of the German Cancer Society (Deutsche Krebsgesellschaft, DKG). Method The process of updating the S3 guideline dating from 2014 was based on an appraisal of the available evidence using the criteria of evidence-based medicine, adaptations of existing evidence-based national and international guidelines or – if evidence was lacking – on a consensus of the specialists involved in compiling the update. After an initial review of the current literature was carried out according to a prescribed algorithm, several areas were identified which, in contrast to the predecessor version from September 2014, required new recommendations or statements which took account of more recently published literature and the appraisal of the new evidence. Recommendations The short version of this guideline consists of recommendations and statements on the epidemiology, screening, diagnostic workup and therapy of patients with cervical cancer. The most important new aspects included in this updated guideline include the newly published FIGO classification of 2018, the radical open surgery approach for cervical cancers up to FIGO stage IB1, and use of the sentinel lymph node technique for tumors ≤ 2 cm. Other changes include the use of PET-CT, new options in radiotherapy (e.g., intensity-modulated radiotherapy, image-guided adaptive brachytherapy), and drug therapies to treat recurrence or metastasis.

Zusammenfassung Hintergrund Empfehlungen aus S3-Leitlinien (S3-LL) stellen eine wichtige Grundlage fur die Entwicklung von Qualitatsindikatoren (QI) dar. Ein einheitliches methodisches Vorgehen zur Entwicklung von QI im Kontext von S3-LL existiert in Deutschland bisher nicht. Aus diesem Grund wurde ein methodischer Standard fur die leitlinienbasierte Entwicklung von QI (QI-Standard) anhand eines strukturierten Konsensprozesses unter Einbezug der relevanten Experten- und Interessengruppen erarbeitet. Methoden Die Inhalte des QI-Standards wurden evidenzgeleitet zusammengestellt, basierend auf den Ergebnissen von Ubersichtsarbeiten und qualitativen Studien sowie unter Berucksichtigung der bereits existierenden Manuale zur leitlinienbasierten QI-Entwicklung. Fur die Abstimmung des QI-Standards wurde ein multiperspektivisches Panel mit breiter Reprasentanz von Experten- und Interessengruppen aus dem deutschen Gesundheitssystem und Expertise in Leitlinienarbeit und/oder Qualitatsmanagement nominiert. Das iterative, strukturierte Konsensverfahren beinhaltete eine zweistufige Onlinebefragung angelehnt an das Delphi-Verfahren („vorlaufige Abstimmung“) sowie eine moderierte Abschlusskonferenz, bei der diejenigen Empfehlungen in den QI-Standard aufgenommen wurden, die eine Zustimmung von >75% (Konsens-Kriterium) des Expertenpanels erhielten. Ergebnisse Der erarbeitete QI-Standard sieht als QI-Entwicklungsschritte zuerst die kriteriengestutzte Auswahl „potenzieller QI“ vor, die nach ihrer Verabschiedung als „vorlaufige QI“ in der S3-LL veroffentlicht werden und schlieslich nach erfolgreicher Testung den Status „finale QI“ erlangen. Der QI-Standard setzt sich aus insgesamt 30 Empfehlungen zusammen, die sich auf sechs Bereiche verteilen: A) Vorbereitende Arbeitsschritte fur die leitlinienbasierte Empfehlung von QI, B) Zusammensetzung der QI-Entwicklergruppe und Zusammenarbeit mit der LL-Gruppe, C) Entwicklung der potenziellen QI, D) Bewertung der potenziellen QI, E) Verabschiedung und Publikation sowie F) Pilotierung/Testung der vorlaufigen und Uberfuhrung in finale QI. Diskussion Bevor der QI-Standard bei kunftigen S3-LL-Erstellungen oder Aktualisierungen zur Implementierung empfohlen werden kann, sollte dieser in ausgewahlten S3-Leitlinienprojekten erfolgreich erprobt worden sein. Neben methodischen Anforderungen an die QI-Entwicklung muss sichergestellt sein, dass den LL-Gruppen adaquate Ressourcen fur die Umsetzung des QI-Standards zur Verfugung stehen. Schlussfolgerung Vom Einsatz des QI-Standards durch Leitliniengruppen konnen wissenschaftlich fundierte und fur die Versorgung relevante QI erwartet werden.

Zusammenfassung Hintergrund Die Handsuche in Fachzeitschriften ist eine Moglichkeit, die Literaturrecherche im Rahmen systematischer Ubersichtsarbeiten und anderer Evidenzsynthesen zu erganzen. Sie ist angezeigt, wenn Fachzeitschriften mit potenziell relevanten Publikationen zur Beantwortung der Forschungsfrage nicht in einer Fachdatenbank indexiert sind. Haufig ist jedoch unklar, ob bzw. in welchen Fachdatenbanken diese Zeitschriften indexiert sind. Ebenso ist oft nicht bekannt, welche Fachzeitschriften nach Festlegung der zu durchsuchenden Fachdatenbanken mittels Handsuche erganzend durchsucht werden sollten. Ziel des Projekts war daher, gesundheitswissenschaftliche Fachzeitschriften hinsichtlich ihrer Fachdatenbankindexierung zu untersuchen und eine Indexierungsubersicht bereitzustellen, um den Planungsprozess der Handsuche zu erleichtern. Methode Es wurden Fachzeitschriften aus dem deutschsprachigen Raum aus acht gesundheitswissenschaftlichen Bereichen (biomedizinische Analytik/medizinisch-technische Laboratoriumsassistenz, Ergotherapie, Hebammenwesen, Logopadie, Pflege, Physiotherapie, Public Health und Rehabilitation) berucksichtigt, in denen wissenschaftliche Originalarbeiten oder systematische Ubersichtsarbeiten und andere Review-Arten in deutscher und/oder englischer Sprache publiziert werden. Je zwei Forschende pro Bereich identifizierten relevante Fachzeitschriften und analysierten die Indexierungsquellen unabhangig voneinander anhand der Zeitschriften-Websites. Bei fehlenden Informationen erfolgte eine direkte Anfrage an die Redaktionen. Ergebnisse Insgesamt 70 Fachzeitschriften wurden eingeschlossen – je Bereich 1-17 Fachzeitschriften. Die Fachzeitschriften sind in 1-29 Fachdatenbanken indexiert. Zwolf Zeitschriften sind nicht indexiert bzw. stellen keine Angaben hierzu zur Verfugung. Die Indexierung verteilt sich auf n = 74 verschiedene Fachdatenbanken. Die meisten Fachzeitschriften sind in LIVIVO (n = 55) und bibnet.org (n = 33) indexiert. Weitere haufige Indexierungsquellen sind Scopus (n = 18), Web of Science Core Collection (n = 16), PSYNDEX (n = 13) und Embase (n = 10). Schlussfolgerungen Die Ergebnisse deuten auf eine heterogene Indexierung der eingeschlossenen Fachzeitschriften hin. Lediglich eine geringe Anzahl ist in gangigen internationalen Fachdatenbanken wie MEDLINE oder CINAHL indexiert. Andererseits gibt es nur wenige Fachzeitschriften, die in keiner Datenbank indexiert sind. Die Ergebnisse konnen als Entscheidungsgrundlage dienen, um Fachdatenbanken fur die Literaturrecherche im Rahmen systematischer Ubersichtsarbeiten festzulegen. Daruber hinaus konnen sie bei der Auswahl von Fachzeitschriften fur die Handsuche nach entsprechender Festlegung der Fachdatenbanken leitend sein.

In contrast to postmenopausal women, evidence for a favorable effect of exercise on Bone Mineral Density (BMD) is still limited for men. This might be due to the paucity of studies, but also to the great variety of participants and study characteristics that may dilute study results. The aim of the present systematic review and meta-analysis was to evaluate the effect of exercise on BMD changes with rational eligibility criteria. A comprehensive search of six electronic databases up to 15 March 2021 was conducted. Briefly, controlled trials ≥6 months that determined changes in areal BMD in men >18 years old, with no apparent diseases or pharmacological therapy that relevantly affect bone metabolism, were included. BMD changes (standardized mean differences: SMD) of the lumbar spine (LS) and femoral neck (FN) were considered as outcomes. Twelve studies with 16 exercise and 12 control groups were identified. The pooled estimate of random-effect analysis was SMD = 0.38, 95%-CI: 0.14–0.61 and SMD = 0.25, 95%-CI: 0.00–0.49, for LS and FN, respectively. Heterogeneity between the trials was low–moderate. Funnel plots and rank and regression correlation tests indicate evidence for small study publication bias for LS but not FN-BMD. Subgroup analyses that focus on study length, type of exercise and methodologic quality revealed no significant difference between each of the three categories. In summary, we provided further evidence for a low but significant effect of exercise on BMD in men. However, we are currently unable to give even rough exercise recommendations for male cohorts.

Purpose This is an official guideline, published and coordinated by the Gynecological Oncology Working Group (AGO) of the German Cancer Society (DKG) and the German Society for Gynecology and Obstetrics (DGGG). Vaginal cancers are rare tumors, which is why there is very little evidence on these tumors. Knowledge about the optimal clinical management is limited. This first German S2k guideline on vaginal cancer has aimed to compile the most current expert knowledge and offer new recommendations on the appropriate treatment as well as providing pointers about individually adapted therapies with lower morbidity rates than were previously generally available. The purpose of this guideline is also to set up a register to record data on treatment data and the course of disease as a means of obtaining evidence in future. Methods The present S2k guideline was developed by members of the Vulvar und Vaginal Tumors Commission of the AGO in an independently moderated, structured, formal consensus process and the contents were agreed with the mandate holders of the participating scientific societies and organizations. Recommendations To optimize the daily care of patients with vaginal cancer: 1. Monitor the spread pattern; 2. Follow the step-by-step diagnostic workup based on initial stage at detection; 3. As part of individualized clinical therapeutic management of vaginal cancer, follow the sentinel lymph node protocol described here, where possible; 4. Participate in the register study on vaginal cancer.

Recommendations of evidence- and formally consensus-based clinical practice guidelines (CPGs) represent a valuable source of quality indicators (QIs). Nevertheless, a standardized methodological procedure for developing QIs in the context of CPGs does not yet exist in Germany for all CPGs. For this reason, a methodological standard for the guideline-based development of QIs (QI Standard) was developed based on a structured consensus process involving multiple key stakeholders.The proposed content of the QI Standard was derived from evidence, drawing upon results of reviews and qualitative studies, and considered German manuals for guideline-based QI development of two guideline programs. A multi-perspective consensus panel, broadly representing key stakeholders from the German healthcare system with expertise in CPGs and/or quality management, was nominated to vote on recommendations for guideline-based development of QIs. The iterative, structured consensus process included a two-stage online survey based on the Delphi method ("preliminary voting") and a moderated final stakeholder conference where all those recommendations were definitely included in the QI Standard that received approval of more than 75 % (consensus criterion) of the consensus panel.Based on the agreed QI Standard, the QI development process starts with a criteria-based selection of "potential" QIs which - in case of adoption - are published in CPGs as "preliminary" QIs and can achieve the status "final" after successful testing. The QI Standard is composed of a total of 30 recommendations, which are allocated to six areas: A) preparatory work steps for the guideline-based recommendation of QIs, B) QI development group and cooperation with the CPG group, C) development of potential QIs, D) critical appraisal of potential QIs, E) formal adoption and publication as well as F) piloting/testing of preliminary QIs and conversion into final QIs.Before the QI Standard can be recommended for implementation in future CPGs, it should have been successfully tested in selected German CPG projects. In addition to methodological requirements for the QI development, it must be ensured that guideline groups have adequate resources for the implementation of the QI Standard.By using the QI Standard, scientifically sound and healthcare-relevant QIs can be expected.

Journal hand searching offers the possibility to complement a literature search as part of systematic reviews and other evidence syntheses. Hand searching is indicated in cases where scientific journals with potentially relevant publications addressing the research question are not indexed in a literature database. However, it is often unclear whether these journals are actually indexed, and when they are, in which literature databases. In many cases, it is also unknown which journals should be searched by hand in addition to systematic literature search after databases to be searched have been specified. Therefore, the project aimed to investigate the indexation of selected scientific health science journals and to provide an overview of indexation in order to facilitate the hand search planning process.Journals from German-speaking countries covering eight professional fields (medical laboratory assistance, occupational therapy, midwifery, logopedics, nursing, physiotherapy, public health and rehabilitation) were considered that publish original research papers or systematic reviews or other review types in German and/or English. Two researchers per field identified relevant journals and independently analyzed the indexing locations using the journal websites. In case of missing information, we contacted the editors.A total of 70 journals were included: from 1 to 17 journals per field. These journals are indexed in 1 to 29 databases. Twelve journals are not indexed or do not offer information concerning indexation. Indexation is distributed across n=74 different literature databases. Most journals are indexed in LIVIVO (n=55) and bibnet.org (n=33). Other common indexing databases are Scopus (n=18), Web of Science Core Collection (n=16), PSYNDEX (n=13), and Embase (n=10).The results indicate a heterogeneous indexation of the included journals. Only a small number is indexed in common international literature databases such as MEDLINE or CINAHL. On the other hand, only a few journals are not indexed in any database. The results can be used as a basis to define databases for literature searches as part of systematic reviews. In addition, the findings might guide the selection of journals for hand searching after literature databases have been defined.

Funded by German Cancer Aid as part of an Oncology Guidelines Program, the lead coordinators of the guideline were the German Society of Gynecology and Obstetrics (DGGG) and the Gynecological Oncology Working Group (AGO) of the German Cancer Society (DKG). Purpose Using evidence-based, risk-adapted therapy to treat low-risk women with endometrial cancer avoids unnecessarily radical surgery and non-useful adjuvant radiotherapy and/or chemotherapy. This can significantly reduce therapy-induced morbidity and improve the patientʼs quality of life as well as avoiding unnecessary costs. For women with endometrial cancer and a high risk of recurrence, the guideline defines the optimal extent of surgical radicality together with the appropriate chemotherapy and/or adjuvant radiotherapy if required. An evidence-based optimal use of different therapeutic modalities should improve the survival rates and quality of life of these patients. This S3-guideline on endometrial cancer is intended as a basis for certified gynecological cancer centers. The aim is that the quality indicators established in this guideline will be incorporated in the certification processes of these centers. Methods The guideline was compiled in accordance with the requirements for S3-level guidelines. This includes, in the first instance, the adaptation of source guidelines selected using the DELBI instrument for appraising guidelines. Other consulted sources included reviews of evidence, which were compiled from literature selected during systematic searches of literature databases using the PICO scheme. In addition, an external biostatistics institute was commissioned to carry out a systematic search and assessment of the literature for one part of the guideline. Identified materials were used by the interdisciplinary working groups to develop suggestions for Recommendations and Statements, which were then subsequently modified during structured consensus conferences and/or additionally amended online using the DELPHI method, with consent between members achieved online. The guideline report is freely available online. Recommendations Part 2 of this short version of the guideline presents recommendations for the therapy of endometrial cancer including precancers and early endometrial cancer as well as recommendations on palliative medicine, psycho-oncology, rehabilitation, patient information and healthcare facilities to treat endometrial cancer. The management of precancers of early endometrial precancerous conditions including fertility-preserving strategies is presented. The concept used for surgical primary therapy of endometrial cancer is described. Radiotherapy and adjuvant medical therapy to treat endometrial cancer and uterine carcinosarcomas are described. Recommendations are given for the follow-up care of endometrial cancer, recurrence and metastasis. Palliative medicine, psycho-oncology including psychosocial care, and patient information and rehabilitation are presented. Finally, the care algorithm and quality assurance steps for the diagnosis, therapy and follow-up of patients with endometrial cancer are outlined.

Zusammenfassung Hintergrund Die zervikale Radikulopathie (ZR) ist ein relativ häufiges Krankheitsbild, welches in sehr vielen Bereichen der Medizin vorkommt und einen interdisziplinären Behandlungsansatz erfordert. Obwohl das Leitsymptom der Schmerz ist, weist die ZR einige Besonderheiten auf, denen die Deutsche Gesellschaft für Neurologie in einer eigenen Leitlinie Rechnung trägt. Das wesentliche Ziel dieser Leitlinie war es, den aktuellen Wissensstand zur Therapie der ZR zu vermitteln. Die vorliegende Leitlinie bezieht sich ausschließlich auf die degenerativ bedingten ZR. Methodik Der Leitlinie liegt eine Recherche von Leitliniendatenbanken und Medline zugrunde. Unter der Moderation durch die Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften (AWMF) erfolgte die Konsensfindung mittels eines nominalen Gruppenprozesses und des Delphi-Verfahrens. Aufgrund des interdisziplinären Behandlungsansatzes wurde das Leitliniengremium aus Experten verschiedener ärztlicher (Neurologie, Orthopädie, Unfallchirurgie, Neurochirurgie, Schmerztherapie, Physikalische und Rehabilitative Medizin und Manuelle Medizin) und therapeutischer Fachdisziplinen (Ergotherapie, Physiotherapie) und unter Beteiligung der Schmerz-Selbsthilfe (Deutsche Schmerzliga) gebildet. Ergebnisse Hinsichtlich der Evidenzbewertung der Literatur ist festzustellen, dass die Datenlage zur Frage des optimalen Vorgehens bei ZR sehr heterogen ist. Die Anwendung von Physiotherapie in Kombination von manueller Therapie und Bewegungsübungen sowie bestimmte operative Verfahren bei ZR sind dagegen evidenzbasiert. Starker Konsens besteht bei den beteiligten Fachgesellschaften, dass das primäre Vorgehen in aller Regel konservativ ist. Hier werden Physiotherapie in Kombination von manueller Therapie, Ergotherapie zur Stärkung der Alltagskompetenz, frühzeitige Schmerztherapie, kurzzeitige intermittierende Ruhigstellung durch das Tragen einer Halskrause und eine periradikuläre Infiltrationstherapie empfohlen. Eine Operation sollte bei therapierefraktärem Schmerzsyndrom nach Ausschöpfung der konservativen Maßnahmen spätestens 6 Monate nach Symptombeginn dem Patienten empfohlen werden. Bei lateralen und intraforaminalen Bandscheibenvorfällen können als Operationsverfahren eine ACDF („anterior cervical discectomy with fusion“) oder eine PCF („posterior cervical foraminotomy“) durchgeführt werden. Das bevorzugte Verfahren ist hier im Einzelfall zu wählen. Bei medianen und paramedianen Bandscheibenvorfällen soll der ventrale Zugang gewählt werden. Schlussfolgerung Die Diagnose einer ZR beruht auf der Anamnese und der neurologischen Untersuchung und soll durch eine bildgebende Methode bestätigt werden. Bei Fehlen behindernder Ausfälle soll das Vorgehen primär konservativ und bei rasch progredienten Paresen primär operativ sein. Bei Einhalten der in der Leitlinie aufgeführten Empfehlungen haben die Schmerzen bei ZR eine günstige Prognose.

Tears and lesions of the rotator cuff are a frequent cause of shoulder pain and disability. Surgical repair of the rotator cuff is a valuable procedure to improve shoulder function and decrease pain. However, there is no consensus concerning the rehabilitation protocol following surgery. To review and evaluate current rehabilitation contents and protocols after rotator cuff repair by reviewing the existing scientific literature and providing an overview of the clinical practice of selected German Society of Shoulder and Elbow Surgery e. V. (DVSE) shoulder experts. A literature search for the years 2004–2014 was conducted in relevant databases and bibliographies including the Guidelines International Network, National Guidelines, PubMed, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, and the Physiotherapy Evidence Database. In addition, 63 DVSE experts were contacted via online questionnaire. A total of 17 studies, four reviews and one guideline fulfilled the inclusion criteria. Based on these results and the obtained expert opinions, a four-phase rehabilitation protocol could be developed.

Lasionen und Rupturen der Rotatorenmanschette sind ein haufiger Grund fur Schulterbeschwerden. Die Sehnenrekonstruktion hat sich heute als erfolgreiches operatives Behandlungsverfahren etabliert. Leider besteht hinsichtlich der sich anschliesenden Rehabilitation und deren Inhalte kein Konsens. Das Ziel ist es, die wichtigsten Therapieinhalte nach Rekonstruktion der Rotatorenmanschette anhand einer umfassenden Literaturrecherche evidenzbasiert zu bewerten und mit Hilfe einer Umfrage unter ausgewiesenen DVSE-Schulterexperten (Deutsche Vereinigung fur Schulter- und Ellenbogenchirurgie e. V.) einen evtl. bestehenden Konsens fur oder gegen spezifische Therapieinhalte zu eruieren. Die Literatursuche erfolgte nach Leitlinien, „health technology assessments“, systematischen Ubersichtsarbeiten und klinischen Studien, die die postoperative Rehabilitation nach Rekonstruktion der Rotatorenmanschette untersuchten. Abgefragt wurden die Datenbanken des „Guidelines International Network“, diverse nationale Leitlinien, PubMed, das „Cochrane Central Register of Controlled Trials“, die „Cochrane Database of Systematic Reviews“ und die „Physiotherapy Evidence Database“, jeweils fur den Zeitraum 01/2004 bis 10/2014. Im Rahmen einer Online-Umfrage wurden 63 ausgewahlte DVSE-Experten zu spezifischen Therapieinhalten der Rehabilitation befragt. Im Zeitraum seit 2004 liesen sich eine Leitlinie, 4 Reviews und 17 Originalarbeiten identifizieren, die als Grundlage zum Aufbau einer strukturierten Nachbehandlung dienten. In Zusammenschau aller Ergebnisse und unter Einschluss der DVSE-Expertenmeinungen konnte ein Grundkonzept der Nachbehandlung erstellt werden, welches in einem Vier-Phasen-Modell zusammengefasst wurde.

Recommendations Part 1 of this short version of the guideline presents recommendations on epidemiology, screening, diagnosis and hereditary factors, The epidemiology of endometrial cancer and the risk factors for developing endomentrial cancer are presented. The options for screening and the methods used to diagnose endometrial cancer including the pathology of the cancer are outlined. Recommendations are given for the prevention, diagnosis, and therapy of hereditary forms of endometrial cancer.
