Background Cardiac interventions may be challenging in patients with congenital cardiac abnormalities. This case reports cardiac resynchronization therapy pacemaker (CRT-P) implantation and single catheter ablation of atrioventricular node (AVN) with remote magnetic navigation (RMN) via peripheral vascular access in a patient with Kartagener's syndrome and permanent atrial fibrillation (AF). Case summary A 74-year-old male with situs inversus presented for treatment of permanent AF and severe heart failure. In echocardiography, left ventricular ejection fraction was 30%, and there was severe dyskinesia due to a left bundle branch block. After successful CRT-P implantation, we performed AVN ablation because biventricular (BiV) pacing was <75% despite maximal rate control medication. The ablation catheter was inserted from the right basilic vein, and no other catheters were used. Despite peripheral vascular access, manipulation of the ablation catheter with RMN was easy, and the ablation was successful. After the ablation, BiV pacing instantly increased to 100%, and left ventricular function and symptomatic status improved gradually. Conclusions Cardiac resynchronization therapy pacemaker implantation and RMN-guided single catheter ablation of the AVN in a patient with dextrocardia via peripheral vascular access was effective and safe. Remote magnetic navigation (RMN) presents a promising approach for catheter ablation in complex arrhythmias. This study aims to offer additional insights into the feasibility, safety, and efficacy of RMN for catheter ablation of epicardial atrial and ventricular arrhythmias in patients without structural heart disease. We present a case series of three patients (39-year-old female, 28-year-old female, and 17-year-old male) referred to our institute for epicardial ablation following unsuccessful endocardial procedures. The first patient exhibited approximately 40,000 monomorphic premature ventricular complexes (PVCs) in a 24-hour Holter recording, while the second patient experienced incessant left ventricular tachycardia (VT). In both cases, the QRS morphology indicated an epicardial origin of the arrhythmia. The third patient suffered from incessant atrial tachycardia (AT) originating from the distal portion of the vein of Marshall (VOM). Epicardial 3D electroanatomic mapping and ablation with RMN were performed with ease and minimal fluoroscopy usage. Coronary angiogram visualization on the RMN screen provided real-time information on the catheter tip's position relative to the coronary arteries. In all cases, the earliest signals were recorded epicardially, and unipolar recordings displayed sharp negative deflections at the sites of successful ablation. In the first patient, pace mapping was superior in the epicardium compared to the endocardium, and PVCs were eradicated by epicardial ablation. In the second patient, epicardial ablation immediately terminated the VT. In the third patient, epicardial ablation at the site exhibiting a distinct VOM potential promptly terminated the AT. No perioperative complications were observed, and all patients remained asymptomatic and free of arrhythmias during long-term follow-up. Our findings suggest that RMN is a feasible, effective, and safe approach for epicardial ablation of left ventricular outflow tract PVCs and VT, as well as atrial tachycardia originating from the VOM. We investigated whether posterior left atrial (LA) box isolation (BOXI) improved the rhythm outcome of catheter ablation in persistent atrial fibrillation (AF) when compared with complex fractionated atrial electrograms (CFAE) ablation. A total of 183 patients with persistent AF who underwent catheter ablation were recruited from the AF ablation registry database at Mie University Hospital; 68 patients underwent CFAE ablation, and 115 patients underwent BOXI. All patients underwent clinical review and 24-hour ambulatory ECG monitoring to detect asymptomatic arrhythmias at 1, 3, 6, and 12 months post-initial ablation. Patients in the BOXI group had a significantly higher freedom from AF rate than those in the CFAE group (81.5% vs 63.9%, p=0.014), with a similar trend in propensity score-matched patients (p=0.003). The BOXI procedure emerged as the only independent prognostic factor for freedom from AF recurrence. BOXI led to greater reduction in left atrial diameter from baseline to 3-6 months post-catheter ablation compared to CFAE ablation. Thus, BOXI may offer superiority over CFAE ablation in maintaining sinus rhythm in patients with persistent AF, in addition to pulmonary vein isolation. [ABSTRACT FROM AUTHOR]

Background: Data comparing remote magnetic catheter navigation (RMN) with manual catheter navigation in combination with contact force sensing (MCN-CF) ablation of atrial fibrillation (AF) is lacking. The primary aim of the present retrospective comparative study was to compare the outcome of RMN versus (vs.) MCN-CF ablation of AF with regards to AF recurrence. Secondary aim was to analyze periprocedural risk, ablation characteristics and repeat procedures. Methods: We retrospectively analyzed 452 patients undergoing a total of 605 ablations of AF: 180 patients were ablated using RMN, 272 using MCN-CF. Results: Except body mass index there was no significant difference between groups at baseline. After a mean 1.6 ± 1.6 years of follow-up and 1.3 ± 0.4 procedures, 81% of the patients in the MCN-CF group remained free of AF recurrence compared to 53% in the RMN group (P < 0.001). After analysis of 153 repeat ablations (83 MCN-RF vs. 70 RMN; P = 0.18), there was a significantly higher reconnection rate of pulmonary veins after RMN ablation (P < 0.001). In multivariable Cox-regression analysis, RMN ablation (P < 0.001) and left atrial diameter (P = 0.013) was an independent risk factor for AF recurrence. Procedure time, radiofrequency application time and total fluoroscopy time and fluoroscopy dose were higher in the RMN group without difference in total number of ablation points. Complication rates did not differ significantly between groups (P = 0.722). Conclusions: In our retrospective comparative study, the AF recurrence rate and pulmonary vein reconnection rate is significantly lower with more favorable procedural characteristics and similar complication rate utilizing MCN-CF compared to RMN. Various studies reported that RMN allows for high catheter stability, improved lesion formation and a superior safety profile. So far, no guidelines or recommendations on RMN-guided CA have been published.PurposeThe aim of this consensus paper was to summarize knowledge and provide recommendations on management of arrhythmias using RMN-guided CA as treatment of atrial fibrillation (AF) and ventricular arrhythmias (VA).MethodologyAn expert writing group, performed a detailed review of available literature, and drawing on their own experience, drafted and voted on recommendations and summarized current knowledge and practice in the field. Recommendations on RMN-guided CA are presented in a guideline format with three levels of recommendations to serve as a reference for best practices in RMN procedures. Each recommendation is accompanied by supportive text and references. The various sections cover the practical spectrum from system and patient set-up, EP laboratory staffing, combination of RMN with fluoroscopy and mapping systems, use of automation features and ablation settings and targets, for different cardiac arrhythmias.ConclusionThis manuscript, presenting the combined experience of expert robotic users and knowledge from the available literature, offers a unique resource for providers interested in the use of RMN in the treatment of cardiac arrhythmias.

Supraventricular arrhythmias have become an increasingly significant contributor to the risk of mortality and morbidity in adults with complex congenital heart disease (CHD), especially in light of recent advances in palliative corrective surgeries. Because of their unique characteristics, they demand specific treatment approaches. While pharmaco-logical interventions are an option, they have limited effectiveness and may lead to side effects. Although performing radiofrequency ablation (RFA) can be exceptionally challenging in patients with complex CHD, due to particular vascular access and also modified anatomy, it has paved the way to enhance comprehension of the underlying mechanisms of supraventricular arrhythmias. This, in turn, enables the provision of improved therapies and, ultimately, an enhancement in the quality of life and symptom management for these patients. The purpose of this case report is to highlight the benefits of utilizing advanced technologies such as three-dimensional electro-anatomical mapping systems, remote magnetic navigation, and highly flexible mapping and ablation catheters during RFA in a young adult with complex congenital heart disease. Although he lacked venous connections to the right atrium (RA) due to multiple corrective surgeries we, remarkably, were capable to advance a decapolar deflectable diagnostic catheter inside the Fontan tunnel and from there to record and stimulate the RA. Successful ablation of two accessory pathways was achieved with no arrhythmia recurrence during follow-up.

Introduction: Pulmonary vein isolation (PVI) is a pivotal part of ablative therapy for atrial fibrillation (AF). Currently, there are multiple techniques available to realize PVI, including: manual‐guided cryoballoon (MAN‐CB), manual‐guided radiofrequency (MAN‐RF), and robotic magnetic navigation‐guided radiofrequency ablation (RMN‐RF). There is a lack of large prospective trials comparing contemporary RMN‐RF with the more conventional ablation techniques. This study prospectively compared three catheter ablation techniques as treatment of paroxysmal AF. Methods: This multicenter, prospective study included patients with paroxysmal AF who underwent their first ablation procedure. Procedural parameters (including procedural efficiency), complication rates, and freedom of AF during 12‐month follow‐up, were compared between three study groups which were defined by the utilized ablation technique. Results: A total of 221 patients were included in this study. Total procedure time was significantly shorter in MAN‐CB (78 ± 21 min) compared to MAN‐RF (115 ± 41 min; p <.001) and compared to RMN‐RF (129 ± 32 min; p <.001), whereas it was comparable between the two radiofrequency (RF) groups (p =.062). A 3% complication rate was observed, which was comparable between all groups. At 12‐month follow‐up, AF recurrence was observed in 40 patients (19%) and was significantly lower in the robotic group (MAN‐CB 19 [24%], MAN‐RF 16 [23%], RMN‐RF 5 [8%] AF recurrences, p =.045) (multivariate hazard ratio of RMN‐RF on AF recurrence 0.32, 95% confidence interval: 0.12–0.87, p =.026). Conclusion: RMN‐guided PVI results in high freedom of AF in patients with paroxysmal AF, when compared to cryoablation and manual RF ablation. Catheter ablation (CA) is an important treatment option for ventricular arrhythmias (VA) in pediatric cardiology. Currently, various CA techniques are available, including remote magnetic navigation (RMN)-guided radiofrequency (RF) ablation. However, no studies evaluate RMN-guided ablative therapy outcomes in children with VA yet. This study aimed to compare procedural and long-term outcomes between RMN-guided and manual (MAN)-guided VA ablation in children. This single-center, retrospective study included all CA procedures for VA performed in children with or without structural heart disease from 2008 until 2020. Two study groups were defined by CA technique: RMN or MAN. Primary outcome was recurrence of VA. Baseline clinical, procedural and safety data were also evaluated. This study included 22 patients, who underwent 30 procedures, with a median age of 15 (IQR 14–17; range 1–17) years and a mean weight of 57 ± 20 kg. In total, 14 procedures were performed using RMN and 16 using MAN (22 first and 8 redo procedures). Regarding first procedures, recurrence rates were significantly lower in RMN compared to MAN (20% versus 67%, P = 0.029), at a mean follow-up of 5.2 ± 3.0 years. Moreover, fluoroscopy dosages were significantly lower in RMN compared to MAN [20 (IQR 14–54) versus 48 (IQR 38–62) mGy, P = 0.043]. In total, 20 patients (91%) were free of VA following their final ablation procedure. This is the first study to investigate the use of RMN in pediatric VA ablation. Background: Stereotaxis™ remote magnetic navigation (RMN) and high density mapping with the Rhythmia™ system have been individually shown to be safe and effective for ablation of ventricular arrhythmias (VAs). No studies have described the conjoint use of Stereotaxis™ with Rhythmia™ for the ablation of VAs. Methods: We describe our single center experience of conjoint Rhythmia™ mapping of VAs and catheter ablation with RMN in a retrospective cohort of ten patients, five with extensive structural heart disease (SHD) due to ischemic and nonischemic cardiomyopathies requiring recurrent ICD shocks despite antiarrhythmic medications, and five with structurally normal hearts. Results: All patients underwent successful procedures targeting their VAs with conjoint Rhythmia™ mapping and Stereotaxis™ RMN. No complications occurred. Over a follow up period ranging between 2 - 25 months (average of 9 ± 6 months), one SHD patient received a single shock for VF. No sustained VAs were recorded in the other SHD patients. No patients with normal hearts had VA recurrence. Conclusions: Conjoint use of Stereotaxis™ RMN and Rhythmia™ to treat VAs in patients with and without SHD is technically feasible and in our ten patient series, was safe and effective. Background: Robotic systems have been used in a diverse spectrum of cardiac interventions, mainly in the field of invasive cardiology, namely interventional cardiology and electrophysiology (EP). The state-of-the-art robotic systems allow mapping and ablation of various arrhythmias without the need for manual conventional catheter steering. The aim of robotic procedures is to achieve the same effectiveness and safety as conventional procedures, whilst reducing the radiation exposure for patients and operators, and to be able to reach targets within the heart that are otherwise unreachable conventionally with greater precision and catheter stability. Purpose: We aimed to demonstrate the feasibility of 3D electroanatomical mapping (EAM) using the remote magnetic navigation (RMN) system (Niobe ES, Stereotaxis Inc) by comparing the mapping performance of 2 operators (one located in the electrophysiology catheter lab control room and the second ~1200 km away via remote online connection). Methods: Two operators were tasked to perform 3D fast anatomical maps (FAM, CARTO 3 RMT, Biosense Webster) of the right and left atrial and ventricular chambers, as well as the aorta of a 3D phantom representing normal cardiac anatomy. All procedures were recorded on the Odyssey Cinema system for further analysis. Parameters compared were duration of FAM, total volume acquired and average distance of surface match to a contrast enhanced computed tomography (CT) of the same phantom. A composite endpoint of map completeness, mapping time and surface match accuracy was calculated to demonstrate if quality of the maps between operators were comparable. Results: A total of 60 maps were created (6 maps for each right atrium (RA), left atrium (LA), right ventricle (RV), left ventricle (LV) and aorta (Ao) per operator) in an alternating fashion. Average mapping time was 16:08±3:36 min for all chambers with shorter mapping times for right atrial chamber 14:28±3:47 min. Total volumes did not significantly differ between operators. Match statistics also revealed no difference between map completeness. Comparing the composite endpoint, both operators achieved the same accuracy. Conclusion: A distant operator located more than 1200 km away from the RMN laboratory connected online to perform 3D electroanatomical maps of all cardiac chambers of a phantom with equal accuracy and procedure parameters as compared to an operator located in the control room. Background: Robotic magnetic navigation (RMN) has shown utility in the ablation of supraventricular tachycardia (SVT). We report our efficacy and safety results from a single center performing RMN catheter ablation of atrioventricular nodal reentry tachycardia (AVNRT), atrioventricular reentrant tachycardia (AVRT), WPW (Wolff-Parkinson-White) syndrome, and focal atrial tachycardia (AT). Methods: A single center retrospective study evaluated the use of RMN in the catheter ablation of SVT including AVNRT, accessory pathway (AP) and focal AT. The primary endpoint was documented recurrence of tachyarrhythmia or recurrence of preexcitation on twelve-lead ECG post ablation. Additional endpoints included total procedural time, procedural fluoroscopic time, operator fluoroscopic time, acute procedural success, and complication rates. Results: 144 SVTs were ablated in 131 pts: AVNRT 79/144 (55%), AP 40/144 (28%), and AT 25/144 (17%). The acute ablation success rate with RMN catheters was 142/144 (99%). Twenty-three (16%) SVTs were septal in location (17 AP and 6 AT). All 23 SVTs were successfully ablated without complication including 7 mid septal & anterior septal APs and 3 parahisian ATs. Five patients with 6 SVTs (3 AVNRT, 1 RA anterior tricuspid valvular AT, 1 right anterior lateral AP, and 1 parahisian AT) were successfully ablated with a RMN catheter after failing ablation with a manual catheter. Total fluoroscopy times were 12 ± 9 minutes with operator fluoroscopic exposure of 1.5±1 min. 111/131 (85%) patients with 123 SVTs were seen at 5 ± 3 months (range 3-14M). The success rates at follow-up were AVNRT 69/69 (100%), AP 29/31 (94%), and AT 22/23 (96%). Conclusions: RMN guided ablation of SVT demonstrated excellent acute and chronic success rates with minimal operator fluoroscopic exposure. Background and aim: Studies evaluating the results of remote magnetic navigation (RMN) using catheter-tissue contact feedback technology are scarce. The aim of this study was to compare the results of ablation of ventricular outflow tract arrhythmias with RMN using the catheter-tissue contact feedback technology with manual ablation with and without contact-force (CF) technology. Methods: Retrospective study of patients that underwent ablation of ventricular outflow tract arrhythmias between May 2017 and December 2021 by the same operator in two hospitals. Patients were excluded in the presence of structural heart disease or previous ablation. Procedural data, success and complication rates and recurrence were compared. Results: Total of 81 patients, 45 underwent ablation with RMN (RMN group), 18 with manual catheters without CF technology (Manual group) and 18 with CF catheters (CF group). The three groups did not differ in relation to baseline characteristics. Patients in the CF group had a higher frequency of arrhythmias originating from the LVOT. The procedure and radiofrequency times were not significantly different, the fluoroscopy time was significantly lower in the RMN group when comparing with Manual and CF groups, 3 (2-5.5) min vs 12 (5.7-17) vs 9.5 (4.9-14.4) min, p<0.0001. There was a direct correlation between fluoroscopy time and procedure time for manual ablation (R=0.480, p=0.003), but not for RMN (R=0.200, p=0.188). p<0.0001). Global success rate was 88% and complication rate was 1% which were not significantly different between groups. Median follow-up was 910 (485-1440) days, recurrence rate was not significantly different (Log-Rank=0.455) Conclusions: RMN ablation of ventricular outflow tract arrhythmias using the catheter-tissue contact feedback technology demonstrated high success and low recurrence rates, with a significantly lower fluoroscopy time than manual or CF guided ablation. Remote Magnetic Navigation (RMN) is a well-described technology with which a flexible catheter is guided through vascular and cardiac structures by a directional magnetic field. When utilized with EAM, the technology allows for atraumatic and precise creation of real-time anatomical representation as well as stable positioning of the ablation catheter tip at the time of energy delivery. The present report describes an alternative method utilizing RMN during 3D Electroanatomic mapping to safely provide real-time visualization of SOV anatomy, including the coronary ostia, in patients undergoing mapping and ablation for idiopathic VAs. Aims: This study aims to compare procedural parameters and clinical efficacy of remote magnetic navigation (RMN) vs. manual navigation (MAN) approach for radiofrequency ablation (RFA) in patients with atrial fibrillation (AF). Methods: 146 patients with AF were enrolled in the study. In the RMN group (n=57), patients were treated with the CARTO® 3 in combination with the Niobe ES system. In the MAN group (n=89), ablation was performed with the EnSite Velocity and TactiCath™ Quartz catheter with direct contact force measurement. Procedural time, ablation time, fluoroscopy time, radiation dose and ablation counts were measured and compared between the groups. Recurrence of AF was evaluated after 6 months of follow-up. Background: The quality of catheter–tissue contact is one of the most important determinants of catheter ablation (CA) success. The absence of catheter–tissue contact feedback has been regarded a major limitation of remote magnetic navigation (RMN)–guided CA. The e-Contact module (ECM) is a novel feature designed for RMN that measures the quality of catheter–tissue contact. Objective: The purpose of this study was to describe the first clinical experience with this feature and to test its effect on procedural parameters and interference with other ablation equipment during CA procedures as well as its safety. Methods: This was a prospective, single-center, 2-phase study investigating ECM use during complex RMN procedures in 30 patients. Impact of ECM on procedural parameters was evaluated in the feasibility phase (FP), and its interference with other equipment was tested in the interference phase (IP) using pacing maneuvers at 3 randomly selected right atrial sites. Intracardiac electrograms were evaluated for disturbances by 2 independent electrophysiologists. Results: For FP, mean procedural time was 162 ± 66 minutes, fluoroscopy time 21 ± 9 minutes, and ablation time 34 ± 21 minutes. For IP, no significant differences in pacing capture or thresholds were found (ECM– vs ECM+: site 1: 2.05 vs 2.21 mA; P = .320; site 2: 2.15 vs 2.12 mA; P = .873; site 3: 2.51 vs 2.50 mA; P = .976). Electrogram disturbances did not significantly differ between ECM– and ECM+. No adverse events were reported. Conclusion: The ECM is a novel catheter–tissue contact technology designed for RMN-guided CA. Our study suggests that this feature is feasible and does not interfere with other electrophysiology equipment while maintaining an excellent safety profile.

Magnetic navigation systems are used to precisely manipulate magnetically responsive materials enabling the realization of new minimally invasive procedures using magnetic medical devices. Their widespread applicability has been constrained by high infrastructure demands and costs. The study reports on a portable electromagnetic navigation system, the Navion, which is capable of generating a large magnetic field over a large workspace. The system is easy to install in hospital operating rooms and transportable through health care facilities, aiding in the widespread adoption of magnetically responsive medical devices. First, the design and implementation approach for the system are introduced and its performance is characterized. Next, in vitro navigation of different microrobot structures is demonstrated using magnetic field gradients and rotating magnetic fields. Spherical permanent magnets, electroplated cylindrical microrobots, microparticle swarms, and magnetic composite bacteria-inspired helical structures are investigated. The navigation of magnetic catheters is also demonstrated in two challenging endovascular tasks: 1) an angiography procedure and 2) deep navigation within the circle of Willis. Background: Data comparing remote magnetic catheter navigation (RMN) with manual catheter navigation (MCN) ablation of atrial fibrillation (AF) is lacking. The aim of the present prospective observational study was to compare the outcome of RMN versus (vs.) MCN ablation of AF with regards to AF recurrence. Methods: The study comprised 667 consecutive patients with a total of 939 procedures: 287 patients were ablated using RMN, 380 using MCN. Results: There was no significant difference between the groups at baseline. After 2.3 ± 2.3 years of follow‐up, 23% of the patients in the MCN group remained free of AF recurrence compared to 13% in the RMN group (p <.001). After analysis of 299 repeat ablations (133 MCN, 166 RMN) there was a significantly higher reconnection rate of pulmonary veins after RMN ablation p <.001). In multivariable Cox‐regression analysis, RMN ablation was an independent risk factor for AF recurrence besides age, persistent AF, number of isolated pulmonary veins, and left atrial diameter. Procedure time, radiofrequency application time and total number of ablation points were higher in the RMN group. Total fluoroscopy time and total fluoroscopy dose were significantly lower for RMN. Complication rates did not differ between groups (p =.842), although the incidence of significant pericardial effusion was higher in the MCN group (seven cases vs. three in RMN group). Conclusions: In our study the AF recurrence rate and pulmonary vein reconnection rate is higher after RMN ablation with a similar complication rate but reduced probability of pericardial effusion when compared to MCN. Background: Catheter ablation (CA) is the first-choice treatment for tachyarrhythmia in children. Currently available CA techniques differ in mechanism of catheter navigation and energy sources. There are no large studies comparing long-term outcomes between available CA techniques in a pediatric population with atrioventricular reentry tachycardia (AVRT) or atrioventricular nodal reentry tachycardia (AVNRT) mechanisms. Objective: This study aimed to compare procedural and long-term outcomes of remote magnetic navigation-guided radiofrequency (RF) ablation (RMN), manual-guided RF ablation (MAN) and manual-guided cryoablation (CRYO). Methods: This single-center, retrospective study included all first consecutive CA procedures for AVRT or AVNRT performed in children without structural heart disease from 2008 to 2019. Three study groups were defined by the ablation technique used: RMN, MAN or CRYO. Primary outcome was long-term recurrence of tachyarrhythmia. Background: There are limited data comparing remote magnetic navigation (RMN) to contemporary techniques of manual-guided ventricular arrhythmia (VA) catheter ablation. Objectives: We compared acute and long-term outcomes of VA ablation guided by either RMN or contemporary manual techniques in patients with structural heart disease. Methods: From 2010–2019, 192 consecutive patients, with ischemic cardiomyopathy (ICM) or non-ischemic cardiomyopathy (NICM) underwent catheter ablation for sustained ventricular tachycardia (VT) or premature ventricular complexes (PVCs), using either RMN (n = 60) or manual (n = 132) guided techniques. Acute success and VA-free survival were compared. Results: In ICM, acute procedural success was comparable between the 2 techniques (manual 43.5% vs. RMN 29%, P = 0.11), as was VA-free survival (manual 83% vs. RMN 74%, P = 0.88), and survival free from cardiac transplantation and all-cause mortality (manual 88% vs. RMN 87%, P = 0.47), both at 12-months after final ablation. In NICM, manual compared to RMN guided, had superior acute procedural success (manual 46% vs. RMN 19%, P = 0.003) and VA-free survival 12-months after final ablation (manual 79% vs. RMN 41%, P = 0.004), but comparable survival free from cardiac transplantation and all-cause mortality 12-months after final ablation (manual 95% vs. RMN 90%, P = 0.52). Procedural duration was shorter in both subgroups undergoing manual guided ablation, whereas fluoroscopy dose and complication rates were comparable. Conclusion: RMN provides similar outcomes to manual ablation in patients with ICM. In NICM however, acute success, and long-term VA-free survival was better with manual ablation. Prospective, multi-centre randomised trials comparing contemporary manual and RMN systems for VA catheter ablation are needed.

Aims: Supraventricular tachycardia (SVT) is one of the most frequent complications in adult patients with congenital heart disease (CHD). Limited experience exists for successful ablation in complex CHD patients with prior Damus-Kaye-Stansel (DKS) surgery. Methods: All patient files of a single-center were screened retrospectively to identify CHD patients with prior DKS surgery throughout a time-interval from 1998 to 2020. These were analysed with focus on SVT, treatment and outcome of catheter ablation (CA). Results: Overall, 13 patients with prior DKS surgery were identified (median age 22.0 years (IQR 20.5–25.5 years), 76.9% male, 92.3% NYHA Class I, median systemic ventricular ejection fraction 58.5% (IQR 53.0–65.5), 69.2% extra cardiac tunnel). Seven patients (53.8%) presented with symptomatic SVT (median cycle length 440 ms (IQR 360–517 ms)). Of these, five patients underwent CA using remote magnetic navigation. The majority of procedures were performed through a retrograde access using three-dimensional image integration. Median procedure time was 180.5 min (IQR 160.8–274.5 min), median fluoroscopy time 1.5 min (IQR 0.5–4.7 min) and radiofrequency time 851 s (265–2005 s). No peri- or postinterventional complications were observed. After a mean of 1.5 procedures, all patients were complaint-free without documentation of tachycardia recurrence throughout a median follow-up of 13 months (IQR 5.8–17.5 months). Conclusion: In complex univentricular situations after Damus-Kaye-Stansel surgery, ablation for supraventricular tachycardia can be achieved safely and successfully. Techniques such as remote magnetic navigation and three-dimensional image integration may aid for successful procedure results.

Background: Studies comparing manual catheter navigation (MCN) to remote magnetic navigation (RMN) for atrial fibrillation (AF) ablation showed variable results. Objective: The aim of this meta-analysis is to compare the safety and clinical outcomes of AF radiofrequency (RF) ablation using MCN versus RMN with irrigated tip catheters. Methods: Medline and the Cochrane Central Register of Controlled Trials (CENTRAL) were queried from inception through January 2019. Studies comparing safety and clinical outcomes of AF ablation with RF using MCN versus RMN with irrigated tip catheters were included. Random effects meta-analysis was used to pool outcomes across studies. Study endpoints included freedom of AF at the end of the study, procedure total time, fluoroscopy time, and complications. Results: A total of 14 studies (3 controlled non-randomized trials, 1 prospective observational, and 10 retrospective observational studies) involving 3375 patients (1871 in MCN and 1504 in RMN) were included in this meta-analysis. There was no significant difference between the two groups in terms of freedom of AF (OR 1.08, 95% CI 0.82–1.42, p = 0.52). The MCN group was associated with shorter procedure time (mean difference in minutes − 50.39, 95% CI − 67.99 to − 32.79, p < 0.01), longer fluoroscopy time (mean difference in minutes 18.01, 95% CI 10.73–25.29, p < 0.01), and higher complication rate (OR 2.18, 95% CI 1.24–3.82, p < 0.01). Conclusions: AF ablation utilizing MCN was associated with similar efficacy to RMN but with higher complication rates. Abstract Background Remote magnetic navigation (RMN) is often used in combination with a 3‐dimensional mapping system to perform catheter ablations. This study aim to investigate the feasibility and effectiveness of a novel 3D‐mapping system, EnSite Precision, combined with RMN for catheter ablation of premature ventricular contractions (PVCs), and compared it to the procedures performed by CARTO3 with RMN. Methods Forty‐three consecutive PVC patients were either ablated with the guidance of EnSite Precision (n = 22) or CARTO (n = 21) navigated by RMN. Procedure‐related details, acute and long‐term success were assessed. Results Patient characteristics between both the groups were similar (age: 47.1 ± 19.8 vs 47.1 ± 12.7, female: 63.6% vs 57.1%). No significant difference was found in the procedure time (99.5 ± 30.4 vs 92.9 ± 24.8 min, P = 0.436), mapping time (18.6 ± 12.8 vs 15.5 ± 10.2 min, P = 0.390), radiofrequency ablation time (333.4 ± 267.0 vs 469.3 ± 343.1 s, P = 0.154), fluoroscopy time (4.0 ± 1.9 vs 3.8 ± 2.0 min, P = 0.635), and X‐ray dose (1.8 ± 1.4 vs 2.0 ± 1.2 Gycm2, P = 0.649) between the two groups. No significant procedural complication occurred in either group. In addition, there was no significant differences regarding the acute success rate (90.9% vs 90.5%, P = 0.961) and long‐term success rate (86.4% vs 81.0%, P = 0.631) after 16.2 ± 6.2 months of follow‐up between the two groups. Conclusions RMN combined with EnSite Precision mapping system is effective and safe for catheter ablation of PVCs.

Purpose: Ventricular tachycardia (VT) ablation is a complex procedure that requires remarkable catheter manipulation skill, great mapping accuracy and catheter stability, and can expose patients to serious complications. Magnetic navigation system (RMN)‐guided ablation and contact force‐sensing (CFS) catheters have the potential to overcome these obstacles. We performed a systematic review and updated meta‐analysis of all available studies evaluating the outcomes of VT ablation by using RMN‐guided compared to manual navigation (MAN)‐guided, with and without CFS catheters. Methods: MEDLINE/PubMed, Cochrane, and Google Scholar were searched for randomized controlled trials (RCT) or observational studies with multivariate adjustment comparing RMN‐guided versus MAN‐guided VT ablation. Results: Thirteen studies enrolling 1348 patients (656 RMN‐guided vs. 692 MAN‐guided) were included. CFS catheter were used in 14% of MAN‐guided patients. In comparison to MAN‐guided and CFS‐guided, RMN‐guided VT ablation was associated with a significant higher acute ablation success (OR 2.32, 1.66–3.23 and OR 2.91, 1.29–6.53, respectively) but similar results in term of long‐term VT recurrence (OR 0.75, 0.56–1.01 and OR 0.79, 0.27–2.36, respectively). RMN‐guided showed a better safety profile (for all complications, OR 0.52, 0.34–0.81) and allowed a significant x‐ray reduction compared to MAN‐guided (OR 0.21, 0.14–0.32) and CFS‐guided VT ablation (OR 0.23, 0.11–0.52, all 95% CI). Conclusions: RMN‐guided was superior to MAN‐guided and CFS‐guided VT ablation in term of acute ablation success, all complications endpoint, and reduction of fluoroscopy exposure, but did not reduce long‐term VT recurrence. Remote magnetic navigation (RMN) can be safely and effectively utilized in patients with difficult cardiac arrhythmias and complex anatomy. Interruption of the inferior vena cava (IVC) is a rare congenital abnormality that results in the inability to use conventional femoral access in patients that require interventional procedures. Introduction: There are numerous challenges to catheter ablation in patients with congenital heart disease (CHD), including access to cardiac chambers, distorted anatomies, displaced conduction systems, multiple and/or complex arrhythmia substrates, and excessively thickened walls, or interposed material. Areas covered: Herein, we review recent developments in catheter ablation strategies for patients with CHD that are helpful in addressing these challenges. Expert opinion: Remote magnetic navigation overcomes many challenges associated with vascular obstructions, chamber access, and catheter contact. Patients with CHD may benefit from a range of ablation catheter technologies, including irrigated-tip and contact-force radiofrequency ablation and focal and balloon cryoablation. High-density mapping, along with advances in multipolar catheters and interpolation algorithms, is contributing to new mechanistic insights into complex arrhythmias. Ripple mapping allows the activation wave front to be tracked visually without prior assignment of local activation times or window of interest, and without interpolations of unmapped regions. There is growing interest in measuring conduction velocities to identify arrhythmogenic substrates. Noninvasive mapping with a multielectrode-embedded vest allows prolonged bedside monitoring, which is of particular interest in those with non-sustained or multiple arrhythmias. Aims. This study aims to compare procedural parameters and clinical efficacy of remote magnetic navigation (RMN) vs. manual navigation (MAN) approach for radiofrequency ablation (RFA) in patients with atrial fibrillation (AF). Methods. 146 patients with AF were enrolled in the study. In the RMN group (n=57), patients were treated with the CARTO® 3 in combination with the Niobe ES system. In the MAN group (n=89), ablation was performed with the EnSite Velocity and TactiCath? Quartz catheter with direct contact force measurement. Procedural time, ablation time, fluoroscopy time, radiation dose and ablation counts were measured and compared between the groups. Recurrence of AF was evaluated after 6 months of follow-up. Results. Mean procedure times (236.87±64.31 vs. 147.22±45.19 min, P<0.05), counts of RF applications (74.30±24.77 vs. 49.15±20.33, P<0.05) and total RFA times (4323.39±1426.69 vs. 2780.53±1157.85 s, P<0.05) were all significantly higher in the RMN than in the MAN group, respectively. In the same order, mean X-ray dose (9722.6±7507.4 vs. 8087.9±6051.5 mGy/cm2, P=0.12) and mean total X-ray exposure time (8.07±4.20 vs. 9.54±5.47 min, P=0.08) were not statistically different. At 6-month follow-up, freedom from AF was similar in RMN and MAN group for paroxysmal (60.8% and 73%, respectively, P=0.42) and persistent AF (69.6% and 75.0%, respectively, P=0.77). Conclusions. Due to the fact that mid-term clinical outcomes showed no significant differences in AF recurrences between groups and manual ablation strategy provided more favorable results regarding acute procedural parameters, we can conclude that the remote magnetic navigation is not superior to the manual approach. Remote magnetic navigation (RMN)-guided catheter ablation (CA) is a feasible treatment option for patients presenting with ischemic ventricular tachycardia (VT). Catheter-tissue contact feedback, enhances lesion formation and may consequently improve CA outcomes. Until recently, contact feedback was unavailable for RMN-guided CA. The novel e-Contact Module (ECM) was developed to continuously monitor and ensure catheter-tissue contact during RMN-guided CA. The present study aims to evaluate the effect of ECM implementation on acute and long-term outcomes in RMN-guided ischemic VT ablation. This retrospective, two-center study included consecutive ischemic VT patients undergoing RMN-guided CA from 2010 to 2017. Baseline clinical data, procedural data, including radiation times, and acute success rates were compared between CA procedures performed with ECM (ECM+) and without ECM (ECM−). One-year VT-free survival was analyzed using Cox-proportional hazards models, adjusting for potential confounders: age, left ventricular function, VT inducibility at baseline and substrate based ablation strategy. The current study included 145 patients (ECM+ N = 25, ECM− N = 120). Significantly lower fluoroscopy times were observed in the ECM+ group (9.5 (IQR 5.3–13.5) versus 12.5 min (IQR 8.0–18.0), P = 0.025). Non-inducibility of the clinical VT at the end of procedure was observed in 92% ECM+ versus 72% ECM− patients (P = 0.19). ECM guidance was associated with significantly lower VT-recurrence rates during 1-year follow-up (16% ECM+ versus 40% ECM−; multivariable HR 0.29, 95%–CI 0.10–0.69, P = 0.021, reference group: ECM−). Background: Pulmonary artery denervation (PADN) procedure has not been applied to patients with residual chronic thromboembolic pulmonary hypertension (CTEPH) after pulmonary endarterectomy (PEA).Objectives: This study sought to assess the safety and efficacy of PADN using remote magnetic navigation in patients with residual CTEPH after PEA.Methods: Fifty patients with residual CTEPH despite medical therapy at least 6 months after PEA, who had mean pulmonary artery pressure ≥25 mm Hg or pulmonary vascular resistance (PVR) > 400 dyn‧s‧cm-5 based on right heart catheterization were randomized to treatment with PADN (PADN group; n = 25) using remote magnetic navigation for ablation or medical therapy with riociguat (MED group; n = 25). In the MED group, a sham procedure with mapping but no ablation was performed. The primary endpoint was PVR at 12 months after randomization. Key secondary endpoint included 6-min walk test.Results: After PADN procedure, 2 patients (1 in each group) developed groin hematoma that resolved without any consequences. At 12 months, mean PVR reduction was 258 ± 135 dyn‧s‧cm-5 in the PADN group versus 149 ± 73 dyn‧s‧cm-5 in the MED group, mean between-group difference was 109 dyn‧s‧cm-5 (95% confidence interval: 45 to 171; p = 0.001). The 6-min walk test distance was significantly increased in the PADN group as compared to distance in the MED group (470 ± 84 m vs. 399 ± 116 m, respectively; p = 0.03).Conclusions: PADN in patients with residual CTEPH resulted in substantial reduction of PVR at 12 months of follow-up, accompanied by improved 6-min walk test. Robotic cardiac electrophysiology technology includes remote magnetic navigation and manual robotic navigation. Robotics can confer advantages with respect to ease of catheter manipulation in anatomically challenging spaces, minimization of fluoroscopic exposure to both patients and operators, and reduction in operator fatigue. Remote magnetic navigation (RMN) is as safe and effective as manual navigation for catheter ablation of ventricular arrhythmias. This case is the first description of a soft‐tip ablation catheter entrapment in the mitral valve apparatus during an RMN ablation procedure. The tight knot created by the catheter around a mitral valve chordae required surgical removal. This complication, which has never been reported before, highlights the need for closer fluoroscopic monitoring when performing catheter loops inside the ventricles when using the RMN system. Background: Percutaneous transcatheter radiofrequency ablation of atrial fibrillation with remote controlled magnetic navigation (RMN) has been shown to reduce radiation exposure to patients and physicians compared with conventional manual (MAN) ablation techniques. Methods: Catheter ablation for atrial fibrillation was performed utilizing RMN in 214 consecutive patients and MAN ablation techniques in 229 patients. We compared the fluoroscopy and procedural times between RMN and MAN catheter ablation of atrial fibrillation. Secondary objectives included comparing acute procedural success and short-term complication rates between both ablation strategies. Results: Fluoroscopy time was significantly shorter in the RMN group than the MAN group (53.5±30.1 vs 68.1±27.6 min, respectively; p0.001). Further subgroup analysis of the most recent 50 ablations each from the RMN and MAN groups, to attenuate the RMN learning curve effect, showed an even greater difference in fluoroscopy time (RMN vs MAN: 53.5±30.1 vs 68.1±27.6 min), though a consistently longer procedure time with RMN (249.5±65.5 vs 186.3±65.6 min, respectively). The acute procedural success rate was comparable between the groups (98.6% vs 95.6%, respectively; p=0.07). The rates of acute complications were similar in both groups (2.3% vs 4.8%, respectively; p=0.16). Conclusions: In radiofrequency ablation of atrial fibrillation, RMN appears to significantly reduce fluoroscopy time compared with conventional MAN ablation, though at a cost of increased total procedural time, with comparable acute success rates and safety profile. A reduction in procedure and fluoroscopy times is possible with gaining experience.

Introduction: Catheter ablation of frequent para‐Hisian premature ventricular contractions (PH‐PVCs) is considered to be challenging. The purpose of this study was to evaluate the strategy, potential technical advantages, and clinical outcomes of remote magnetic navigation (RMN) in the ablation of PH‐PVCs. Methods: Fifteen consecutive patients with PH‐PVCs were included in this study. Electrical mapping was initially performed in the right ventricular septum by manipulating the RMN catheter with a "U‐curve." In the case of no optimal ablation site or ablation failure, the ablation catheter was directed to the left ventricular (LV) septum through a transseptal approach for further mapping and ablation by manipulating the RMN catheter with a "reverse S‐curve." Results: Nine of 15 patients were submitted to ablation on the right side. However, ablation success was only achieved in only three (33%) cases. Of the other 12 patients, 11 underwent LV mapping and ablation. In this subset, 9 of 11 (82%) PH‐PVCs were totally eliminated on the left side. Overall, RMN‐guided mapping and ablation successfully eliminated 12 (80%) of 15 idiopathic PH‐PVCs. During follow‐up, the reoccurrence of PVCs was reported in 1 (8%) of 12 patients. No atrioventricular block was observed during or after the procedure. Conclusion: RMN‐guided catheter ablation for PH‐PVCs is effective and safe in unselected patients. Due to the excellent reachability and contact with special morphologies of the RMN catheter on both sides of the ventricular septum, RMN can be considered an effective approach for frequent PH‐PVCs. Background: The aim of this review was to evaluate the efficacy and safety between remote magnetic navigation (RMN) and manual control navigation (MCN) for atrial fibrillation (AF) ablation.Methods: We searched the PubMed, EMBASE and Cochrane library databases using the key words AF, ablation and magnetic navigation.Results: Eighteen studies were identified in this analysis including 4046 patients comparing RMN and MCN in AF ablation, which were all non-randomized controlled studies. No significant difference of AF recurrence rate (40% vs. 38%, OR 1.00, 95% CI 0.82-1.22, p = 0. 97) and acute success rate in achieving pulmonary vein isolation (91% vs. 93%, OR 0.44, 95% CI 0.16-1.17, p = 0.10) was found between RMN and MCN. However, compared with MCN, RMN was associated with significantly lower complication rate (2% vs. 5%, OR 0.44, 95% CI 0.28-0.69, p = 0. 0003) and shorter fluoroscopy time (MD -9.71 min, 95% CI -15.80 to -3.63, p = 0.002). Methods: Forty‐three consecutive PVC patients were either ablated with the guidance of EnSite Precision (n = 22) or CARTO (n = 21) navigated by RMN. Procedure‐related details, acute and long‐term success were assessed. Results: Patient characteristics between both the groups were similar (age: 47.1 ± 19.8 vs 47.1 ± 12.7, female: 63.6% vs 57.1%). No significant difference was found in the procedure time (99.5 ± 30.4 vs 92.9 ± 24.8 min, P = 0.436), mapping time (18.6 ± 12.8 vs 15.5 ± 10.2 min, P = 0.390), radiofrequency ablation time (333.4 ± 267.0 vs 469.3 ± 343.1 s, P = 0.154), fluoroscopy time (4.0 ± 1.9 vs 3.8 ± 2.0 min, P = 0.635), and X‐ray dose (1.8 ± 1.4 vs 2.0 ± 1.2 Gycm2, P = 0.649) between the two groups. No significant procedural complication occurred in either group. In addition, there was no significant differences regarding the acute success rate (90.9% vs 90.5%, P = 0.961) and long‐term success rate (86.4% vs 81.0%, P = 0.631) after 16.2 ± 6.2 months of follow‐up between the two groups. Objectives: Catheter ablation is regarded as first-line therapy for symptomatic atrioventricular nodal reentry tachycardia (AVNRT). Ablation induces intended myocardial damage and the extent of myocardial damage may differ between ablation methods. The objective of this MAGMA AVNRT(NCT00875914) substudy was to compare high-sensitive cardiac troponin T (hs-cTnT) levels as a surrogate marker for myocardial damage after manually guided (MAN) AVNRT ablation versus AVNRT ablation using remote magnetic navigation (RMN).Design: In total, 70 patients (mean age 44 ± 14 years, 26% male) undergoing catheter ablation for AVNRT in the MagMa-AVNRT-Trial were randomized to remote magnetic navigation (n = 34, 49%) or manually guided catheter ablation (n = 36, 51%). hs-cTnT was measured the day after the procedure.Results: The median follow-up time was 6.2 ± 1.1 years. Acute success was 100% in both groups. hs-cTnT release was significantly lower in the remote magnetic navigation group (52 ng/L versus 95 ng/L, p < .01), even though the ablation time was longer and number of applications was higher with remote magnetic navigation (4.2 min vs 2.8 min, p = .017; 4.9 vs 3.3 applications, p = .01). hs-cTnT released per minute ablation time was also lower with remote magnetic navigation (12 ng/L versus 34 ng/L, p < .01). Both groups exhibited similar clinical long-term follow up regarding recurrence and complications.Conclusion: Remote magnetic navigation controlled catheter ablation of AVNRT has similar clinical outcome, but leads to less hs-cTnT release than manually guided catheter ablation. Rationale: Pulmonary vein isolation (PVI) is the method of choice for the treatment of drugresistant atrial fibrillation (AF). However, arrhythmia recurrences are frequent due to suboptimal lesions formation and pulmonary vein reconnection. Contact force (CF)- and remote magnetic navigation (RMN)-guided catheters are both capable of improving contact and energy transfer to the tissue, possibly improving PVI outcome. Objective: The objective of our study was to compare CF- and RMN-guided PVI in patients with paroxysmal AF in terms of surrogate parameters of the dimension and quality of the lesions. Methods and results: Between March 2014 and March 2016, 44 patients affected by paroxysmal AF underwent a circumferential PVI procedure, 22 with a CF catheter and 22 with a RMN catheter. Signals were recorded before and after 30 seconds of radiofrequency point-by-point delivery. For each location, signal energy attenuation and impedance drop were evaluated as lesion dimension surrogates and signal fragmentation and shrinkage were estimated as lesion quality surrogates. Statistical analysis shows that CF catheter achieves higher attenuation and impedance drop than RMN-guided catheter, which instead performs better in terms of shrinkage and fragmentation. There were no differences in terms of PVI rate and sinus rhythm maintenance 1 year after the procedure (77.2% in both groups). Conclusions: CF-guided catheters produce lesions that are larger but less homogeneous than those produced by RMN-guided catheters in terms of surrogate parameters. Aims: The objective of this study is to evaluate the outcomes of magnetically guided ablation of atrial fibrillation (AF) rotors in conjunction with magnetically guided pulmonary vein isolation (PVI) in a large consecutive series of patients.Methods and results: A total of 110 consecutive patients with drug-refractory AF underwent rotor ablation followed by conventional PVI and ablation of other spontaneous arrhythmias, all of which were performed with remote magnetic navigation (RMN). The patients were followed to assess the recurrence of atrial arrhythmia. Patients had a mean age of 62.5 ± 9.9 years, 64.5% had persistent AF, and 36.4% had a prior failed PVI. All patients had mapped rotors (3.9 ± 1.5 per patient), with right atrial (RA) rotors in 77.3% (85/110) of patients. After a mean follow-up of 17.6 ± 9.5 months, 90.9% (100/110) were in stable sinus rhythm including patients on previously ineffective antiarrhythmic drugs (AADs). 69.1% (76/110) were in stable sinus rhythm without any AADs. Outcome did not differ between patients with persistent or paroxysmal AF (69.2% vs. 69.0%; P = 0.75), failed prior ablation or those undergoing an initial ablation (77.5% vs. 64.3%; P = 0.193), or patients with and without intra-procedural AF termination (67.3% vs. 70.5%; P = 0.723).Conclusion: Ablation of rotors in combination with PVI using RMN was associated with a high success rate in this large cohort of consecutive patients. Aims: Remote magnetic navigation (RMN) is an alternative to manual catheter control (MCC) radiofrequency ablation of right ventricular outflow tract (RVOT) arrhythmias. The data to support RMN approach is limited. We aimed to investigate the clinical and procedural outcomes in a cohort of patients undergoing RVOT premature ventricular complex/ventricular tachycardia (PVCs/VT) ablation procedures using RMN vs. MCC.Methods and results: Data was collected from two centres. Eighty-nine consecutive RVOT PVCs/VT ablation procedures were performed in 75 patients; RMN: 42 procedures and MCC: 47 procedures. CARTOXPTM or CARTO3 (Biosense Webster) was used for endocardial mapping in 19/42 (45%) in RMN group and 28/47 (60%) in MCC group; EnSiteTM NavXTM (St. Jude Medical) was used in the rest of the cohort. Stereotaxis platform (Stereotaxis Inc., St. Louis, MO, USA) was used for RMN approach. Procedural time was 113 ± 53 min in the RMN group and 115 ± 69 min in MCC (P = 0.90). Total fluoroscopic time was 10.9 ± 5.8 vs. 20.5 ± 13.8 (P < 0.05) and total ablation energy application time 7.0 ± 4.7 vs 11.9 ± 16 (P = 0.67) accordingly. There were two complications in RMN group and five in MCC (P = 0.43). Acute procedural success rate was 80% in RMN vs. 74% in MCC group (P = 0.46). After a median follow-up of 25 months (interquartile range 13-34), the success rate remained 55% in the RMN group and 53% in MCC (P = 0.96).Conclusion: Right ventricular outflow tract arrhythmia ablations were performed using half of fluoroscopic times with Stereotaxis platform RMN compared to manual approach. Aims: Remote magnetic navigation (RMN) is a safe and effective means of performing ventricular tachycardia (VT) ablation. It may have advantages over manual catheter ablation due to ease of manoeuvrability and catheter stability. We sought to compare the safety and efficacy of RMN vs. manual VT ablation.Methods and results: Retrospective study of procedural outcomes of 139 consecutive VT ablation procedures (69 RMN, 70 manual ablation) in 113 patients between 2009 and 2015 was performed. Remote magnetic navigation was associated with overall higher acute procedural success (80% vs. 60%, P = 0.01), with a trend to fewer major complications (3% vs. 9% P = 0.09). Seventy-nine patients were followed up for a median of 17.0 [interquartile range (IQR) 3.0-41.0] months for the RMN group and 15.5 (IQR 6.5-30.0) months for manual ablation group. In the ischaemic cardiomyopathy subgroup, RMN was associated with longer survival from the composite endpoint of VT recurrence leading to defibrillator shock, re-hospitalization or repeat catheter ablation and all-cause mortality; single-procedure adjusted hazard ratio (HR) 0.240 (95% CI 0.070-0.821) P = 0.023, multi-procedure HR 0.170 (95% CI 0.046-0.632) P = 0.002. In patients with implanted defibrillators, multi-procedure VT-free survival was superior with RMN, HR 0.199 (95% CI 0.060-0.657) P = 0.003.Conclusion: Remote magnetic navigation may improve clinical outcomes after catheter ablation of VT in patients with ischaemic cardiomyopathy. Aims: Oesophagogastroduodenoscopy (OGD) after catheter ablation (CA) of atrial fibrillation (AF) revealed a high rate of procedure related oesophageal lesions. We hypothesized that magnetically guided CA with careful radiofrequency energy titration at the posterior left atrial (LA) wall limits the incidence of oesophageal tissue damage.Methods and results: As a part of the prospective "Remote MAGNetic catheter Ablation for Atrial Fibrillation" (MAGNA-AF) registry, 251 out of 266 consecutive patients underwent OGD after magnetically guided single-CA for AF. All detected pathologies were analysed. Simultaneous pacing and ablation from the tip of the magnetically guided catheter was found to be a safe and feasible method for energy titration. Post-interventional OGD documented midoesophageal tissue damage in four (1.6%) patients. Although a thermal origin of these injuries must be discussed, none of them was located at the anterior oesophageal wall. Risk factors for midoesophageal lesions were female gender and concomitant acetylsalicylic acid (ASA) treatment. Mechanical lesions in 16 patients were attributed to periprocedural transoesophageal echocardiography (TOE). There was no atrio-oesophageal fistula (AOF). Five hundred and one incidental pathologies were found endoscopically, most frequent axial oesophageal herniation (71%), oesophagitis (22%), and gastritis (57%).Conclusion: Magnetically guided CA for AF with careful energy titration at the posterior LA wall and no oesophageal temperature monitoring is not associated with an increased incidence of oesophageal thermal injury. The routine use of periprocedural TOE may cause a low rate of mechanical oesophageal lesions but reliably prevents major complications like transient ischaemic attack, stroke, or cardiac tamponade. An observed high prevalence of upper digestive system inflammation (63%) may further support the recommendation for a routine post-interventional treatment with a proton-pump-inhibitor. Aims: The objectives of this study were to assess the acute and long-term outcomes of catheter ablation in incisional atrial tachycardia (IAT) using remote magnetic navigation (RMN) in patients after heart surgery.Methods and results: A total of 46 patients with IAT after heart surgery who underwent catheter ablation using RMN were included. Of these patients, 22 patients had acquired heart disease (AHD) and the remaining 24 patients had various types of congenital heart disease (CHD). In these 46 patients, 57 re-entry circuits were found in 56 procedures. The re-entry circuits were mainly distributed in right atrium (RA). Acute success of first ablation reached in 42 of 46 (91%) patients. Mean procedure duration was 115 ± 39 min, ablation duration was 678 (920.5) s, X-ray time was 4 (4.8) min, and X-ray dose was 3 (6.0) gy cm2. After a mean follow-up of 28 ± 19 months, 39 of 46 (85%) patients were free from IAT. No major complications were observed. There were no significant differences in procedure durations (AHD 113 ± 40 min vs. CHD 119 ± 38 min), ablation durations [AHD 643 (1027) s vs. CHD 712 (929) s], X-ray time [AHD 4 (4.5) min vs. CHD 4 (5.0) min], circuits in RA (AHD 85% vs. CHD 86%), acute success rates (AHD 91% vs. CHD 92%), and long-term success rates (AHD 86% vs. CHD 83%) between the two groups (P > 0.05).Conclusion: Catheter ablation of IAT in patients after heart surgery using RMN is safe and effective.
