The 'breast-sharing' procedure uses the disposable tissue as a flap to reconstruct the post-mastectomy defect. This enables simultaneous breast reconstruction and contralateral symmetrisation without additional donor site morbidity. However, controversies related to the oncological safety of this procedure have prevented its widespread uptake. We aim to present this technique based on a pedicled internal mammary artery perforator (IMAP) flap and discuss its technical feasibility and oncological safety. Between April 2013 and May 2022, a series of 10 consecutive patients underwent a breast-sharing procedure using the pedicled IMAP flap for breast reconstruction. Clinical and surgical aspects of the breast-sharing procedure were analysed. In all the cases, the breast-sharing technique allowed for simultaneous breast reconstruction and contralateral breast symmetrisation. Immediate complications included two total venous congestions and one hematoma. Three flaps had distal flap congestion. One flap required debridement and local flap reconstruction. All flaps required fat grafting during the secondary procedures to improve breast symmetry. With an average follow-up of 5.6 years, there was no evidence of recurrent disease. All patients were satisfied to very satisfied with the aesthetic outcome of this reconstructive option. The breast-sharing technique based on the IMAP flap combines breast reconstruction and contralateral symmetrisation with good aesthetic outcomes in selected patients. The flap has a high complication rate related to venous drainage. Flap design modification and using indocyanine green imaging may reduce venous congestion. A secondary venous micro-anastomosis at the axilla is highly recommended for persistent flap congestion. The reported incidence of contralateral breast cancer is low, and thus, the residual tissue obtained from the contralateral breast mammaplasty can be safely used for breast reconstruction. Objective. To analyze the coexistence of chronic non-communicable diseases (NCDs) and common neoplasms among endocrine adult inpatients. Materials and method. The retrospective analysis was performed using clinical data of 2,462 adult patients (2,003 women and 459 men), hospitalized in the reference endocrine department. Diagnoses of 18 types of benign tumours and 16 types of malignant tumours, together with the most common 25 NCDs and demographic parameters, were all collected from the medical records. The most frequently found 6 types of benign tumours (of thyroid, pituitary, uterus, breast, adrenal and prostate) and 4 types of malignant tumours (of thyroid, breast, prostate and uterus) were taken for further statistical analyses. Results. Age predicted the existence of accumulated as well as individual types of benign and malignant tumours, whereas BMI predicted the occurrence of accumulated and some individual types of benign tumours. Accumulated as well as individual types of benign and malignant tumours coexisted more frequently with several NCDs, such as diabetes, hypertension, metabolic syndrome, osteoporosis, Graves' disease, coronary artery disease, state after cholecystectomy, thus being disorders usually resulting from excessive exposure to harmful environmental factors. The most distinct coexistence was found between breast cancer and metabolic syndrome, between breast cancer and Graves' disease, between cancer of the uterus and type 2 diabetes, between cancer of the uterus and metabolic syndrome, and between cancer of the uterus and dyslipidemia. Conclusion. The results obtained indicate a significant relationship between the most common NCDs and several cancers in endocrine adult patients, which suggests that the prevention of the former may reduce the frequency of the latter. Abstract: Objective: To analyze clinical implications of the thoracodorsal nerve division in the latissimus dorsi musculocutaneous flap breast reconstruction. Patients and methods: Prospective cohort study was conducted on 29 patients. Breast reconstruction with latissimus dorsi musculocutaneous flap was performed unilaterally in 20 patients or bilaterally in 9 women (38 breasts). Thoracodorsal nerve was divided during reconstruction of 20 breasts (group 1) and was preserved for 18 breasts (group 2). Height, width, projection, area of the covering skin and volume of the reconstructed and healthy breasts were measured on the 3D images of the anterior chest wall, taken 6 weeks and 6 months postoperatively with the Di3D 3D camera. Data regarding tissue consistency, painfulness and animation of the reconstructed breast, symmetry of both breasts and overall satisfaction after the surgery were collected at 6 months. Results: The reconstructed and healthy breasts decreased in volume in group 1 (−45.85 cm3 ± 48.41 cm3, p = 0.0004; −29.13 cm3 ± 14.98 cm3, p = 0.0009) and in group 2 (−31.5 cm3 ± 25.35 cm3, p = 0.0001; −15.4 cm3 ± 21.96 cm3, p = 0.0537). There were no differences in decrease in volume between groups 1 and 2 (p > 0.05). Respondents in group 1 in comparison to group 2 showed similar satisfaction of the tissue consistency of the reconstructed breast (p > 0.05) and the level of symmetry between both breasts (p > 0.05), gave lower scores for painfulness (p < 0.0001), animation (p < 0.0001) and higher scores for the overall satisfaction about the reconstructed breast (p = 0.0001). Conclusion: We suggest that division of the thoracodorsal nerve during latissimus dorsi musculocutaneous flap breast reconstruction is a useful undertaking to minimize unnatural animation of the reconstructed breast. Since 2009, the companies listed on regulated markets in the European Union have been required to present total comprehensive income (CI) and its components in the consolidated financial statements prepared under International Financial Reporting Standards (IFRS). CI is a measure of company income from investor's perspective-it reflects the enhancement of the entity owners' wealth. CI includes the net income (NI) and other comprehensive income (OCI). The change of perspective from which the performance is presented may have an impact on its evaluation and decisions taken by financial information users. This raises the questions of how much CI differs from net income (NI) and what problems are connected with presenting OCI in financial statements of public companies in Poland, for which CI reporting is still a new requirement. The paper has been based on literature review, analysis and interpretation of International Accounting Standard 1 (IAS 1), and a review of consolidated financial statements for the years 2010-2011 of 140 companies, listed on the Warsaw Stock Exchange (WSE). The conducted analysis has been the basis for the main conclusion that the usefulness of the statements of CI of the companies, listed on the WSE, for explaining changes in entities' shareholder wealth is very limited because the majority of the companies do not present material OCI and its components. Autologiczna rekonstrukcja piersi polega na zastosowaniu różnych płatów tkankowych przełożonych z odległych okolic ciała. Modyfikacje dotychczasowych technik operacyjnych mają na celu utrzymanie estetycznych i onkologicznych wyników odtworzenia piersi, przy zminimalizowaniu powikłań dodatkowych okolic dawczych. Procedura tzw. dzielenia piersi polega na jednoczasowej autologicznej rekonstrukcji piersi z symetryzacją przeciwległej okolicy. Pozwala to na wyeliminowanie dodatkowego miejsca dawczego. Odtworzona pierś z tkanki gruczołu piersiowego ma dobrą objętość, projekcję i konsystencję. Skóra płata ma natomiast doskonałą jakość i kolorystykę, bardziej dopasowaną niż w płatach z odległych okolic ciała. Celem pracy jest przedstawienie koncepcji tzw. dzielenia piersi jako dobrej i onkologicznie bezpiecznej jednoetapowej techniki odroczonej rekonstrukcji piersi z przeciwległą mammaplastyką. W operacji dzielenia piersi tkanka do rekonstrukcji jest przełożona jako płat skórno-gruczołowy oparty na perforatorze tętnicy piersiowej wewnętrznej (IMAP). W 2016 roku została wykonana pierwsza w Polsce operacja tzw. dzielenia piersi. Ukrwienie płata było zapewnione przez IMAP z czwartej przestrzeni międzyżebrowej. W okresie pooperacyjnym obserwowano objawy brzeżnego przekrwienia żylnego. W 2017 roku została przeprowadzona operacja modelowania piersi. Uzyskano prawidłowy efekt estetyczny. Onkologicznie, po 15-miesięcznej obserwacji, nie stwierdza się objawów nawrotu miejscowego. Wnioski Procedura dzielenia piersi uszypułowanym skórno-gruczołowym płatem perforatorowym IMAP jest użyteczną metodą rekonstrukcji piersi u pacjentek wymagających przeciwległej symetryzacji. Częstość występowania raka przeciwległej piersi jest akceptowalnie niska i procedura jest bezpieczna onkologicznie. Ta technika może dawać dobre wyniki estetyczne u odpowiednio dobranych pacjentek bez chorobowości odległych okolic ciała. [ABSTRACT FROM AUTHOR]

Wstęp Pacjenci po amputacji kończyny dolnej bez wykonania odpowiedniego pokrycia kikuta kostnego tkankami miękkimi mają ograniczone możliwości protezowania. Wynikający z tego duży stopień niepełnosprawności poważnie ogranicza ich prawidłowe funkcjonowanie w życiu społecznym i zawodowym. Jednocześnie, znaczne skrócenie tkanek kostnych w celu zastosowania plastyk miejscowych wiąże się często z koniecznością zastosowania mniej wszechstronnej protezy. Cel pracy Celem pracy jest przedstawienie sposobu rekonstrukcji tkanek miękkich kikuta podudzia za pomocą wolnego płata przednio-bocznego uda jako metody pomocnej w wyborze optymalnej metody protezowania. Materiał i metody Do Poradni Chirurgii Plastycznej Kliniki Chirurgii Plastycznej, Rekonstrukcyjnej i Estetycznej w Bydgoszczy zgłosił się 20-let-ni pacjent z kikutem Vi bliższej podudzia prawego, pokrytym wolnym przeszczepem skóry, po pourazowej amputacji z powodu wypadku podczas pracy. U pacjenta podjęto wcześniej nieskuteczne metody protezowania. Chory był konsultowany celem stwierdzenia możliwości odtworzenia funkcjonalności kończyny dolnej. U mężczyzny wykonano rekonstrukcję tkanek miękkich kikuta przy użyciu wolnego płata przednio-bocznego uda. Wyniki W wyniku operacji uzyskano dobre pokrycie końca dalszego pozostałej części kości piszczelowej grubą warstwą tkanek miękkich, co umożliwiło późniejsze odpowiednie uformowanie kikuta. Dlatego możliwe stało się zastosowanie protezy stałej podudzia ze stopą dynamiczną. Znaczne zwiększenie zakresu aktywności fizycznej przyczyniło się do pełnego powrotu pacjenta do życia zawodowego i prywatnego. Wnioski Wolny płat przednio-boczny uda zapewnia odpowiednią ilość i jakość tkanek dla uzyskania funkcji podporowej kikuta kończyny dolnej wymaganej przy protezowaniu. [ABSTRACT FROM AUTHOR]

Background: Nanotextured implants, an innovation in implant technology, are designed to enhance tissue interaction, reduce inflammation, and mitigate complications of traditional implants., Objectives: The aim of the study was to evaluate the aesthetic potential and safety profile of nanotextured breast implants in breast augmentation from a single-surgeon experience., Methods: This prospective cohort study included 1000 patients who underwent breast augmentation with nanotextured implants. Aesthetic outcomes were assessed with direct measurements and BCCT.core software, focusing on breast projection, symmetry, and upper pole fullness. Patient satisfaction was measured with BREAST-Q and overall satisfaction surveys. Safety profiles were evaluated based on the incidence of complications such as infection, hematoma, capsular contracture, and implant rupture. Statistical analyses were conducted to assess the outcomes., Results: The study found significant improvements in aesthetic outcomes, with 92.8% of cases achieving substantial aesthetic enhancements. Patient satisfaction was high, with 96.4% of patients reporting enhanced self-esteem and contentment. The safety profile was favorable, with a low incidence of complications (0.7% requiring revisional surgery) and no major complications reported. Long-term follow-up data indicated sustained aesthetic outcomes and a stable safety profile., Conclusions: The synergistic quality of nanotextured implants in relation to the natural breast gland tissue is defined by a significantly wider range of aesthetic options. Background: As a burgeoning technique in reconstructive and aesthetic surgery, lipofilling's success is hindered by the unpredictability of graft integrity and quality. This study addresses the critical need to enhance consistency and reproducibility by exploring the clinical utility of adipose tissue from specific body areas, considering the influence of patient-specific factors and mechanical processing on fat graft integrity and morphology., Methods: In a prospective, randomized, single-blind study, 52 patients undergoing surgical reconstruction due to significant deformities were enrolled. Lipoaspiration from four areas was performed. Adipose tissue was compared using five parameters of tissue damage and 10 parameters of graft integrity, assessed immediately postcollection and after centrifugation. The study aimed to evaluate the structural integrity and clinical applicability of adipocytes., Results: Morphological assessment revealed no significant differences in adipose tissue quality across donor sites, suggesting consistent graft quality regardless of the harvesting location. Centrifugation induced more morphological damage than noncentrifuged samples, but the overall graft integrity was maintained due to increased cell density. Higher graft acceptance parameters were noted in noncentrifuged samples compared with centrifuged ones., Conclusions: Despite centrifugation-induced morphological changes, adipose tissue integrity remains relatively unaffected, supporting a flexible approach to donor site selection. The consistent quality of adipose tissue underscores the potential for autologous fat transplantation across various clinical scenarios. Patients without proper covering of the bone stump with soft tissues after below-knee amputation have limited opportunities for prosthesis. The resulting high degree of disability severely restricts their proper functioning in social and professional life. The commonly used significant reduction of the bone length for local coverage limits rehabilitative options to the less comprehensive prosthesis. We aimed to describe a delayed reconstruction with soft tissues of the lower leg stump using free anterolateral thigh flap as an alternative surgical method allowing for optimal prosthesis. A 20-year-old patient was consulted because of right lower leg stump, covered only with a skin graft following posttraumatic amputation. Previously, the patient had ineffective attempts of using a prosthesis. He asked to determine the possibility of recovering the functionality of the lower limb. We performed reconstruction of soft tissues of the stump with a free anterolateral thigh flap. Postoperatively, the patient achieved good coverage of the remaining part of the tibia with a thick layer of soft tissues, allowing the subsequent adequate forming of the stump. Therefore, a fixed prosthesis with the dynamic foot could be implemented. A significant increase in physical activity contributed to a full return to the patient's professional and private life. In conclusion, the free anterolateral thigh flap provides a robust amount of good-quality tissues for supportive function of the lower limb stump.
