The interaction between dodecyltrimethylammonium bromide (DoTab) and methacrylic acid/ethyl acrylate copolymers was studied using isothermal titration calorimetry (ITC) and laser light scattering (LLS) techniques. The thermodynamic parameters extracted from ITC curves suggest that both the electrostatic binding and micellization are enthalpy-opposed driven by entropy.

- Abstract
Background: Glucagon-like peptide 1 receptor agonists differ in chemical structure, duration of action, and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. We aimed to determine the safety and efficacy of albiglutide in preventing cardiovascular death, myocardial infarction, or stroke. Methods: We did a double-blind, randomised, placebo-controlled trial in 610 sites across 28 countries. We randomly assigned patients aged 40 years and older with type 2 diabetes and cardiovascular disease (at a 1:1 ratio) to groups that either received a subcutaneous injection of albiglutide (30–50 mg, based on glycaemic response and tolerability) or of a matched volume of placebo once a week, in addition to their standard care. Investigators used an interactive voice or web response system to obtain treatment assignment, and patients and all study investigators were masked to their treatment allocation. We hypothesised that albiglutide would be non-inferior to placebo for the primary outcome of the first occurrence of cardiovascular death, myocardial infarction, or stroke, which was assessed in the intention-to-treat population. If non-inferiority was confirmed by an upper limit of the 95% CI for a hazard ratio of less than 1·30, closed testing for superiority was prespecified. This study is registered with ClinicalTrials.gov, number NCT02465515. Findings: Patients were screened between July 1, 2015, and Nov 24, 2016. 10 793 patients were screened and 9463 participants were enrolled and randomly assigned to groups: 4731 patients were assigned to receive albiglutide and 4732 patients to receive placebo. Introduction: Whether final height is associated with quality of life and mental health is a matter of epidemiological and medical concern. Both social and biological explanations have been previously proposed. This study aims to assess the associations in ethnic Chinese in Singapore. Materials and Methods: A cross-sectional study of 4414 respondents aged at least 21 years seen at a major polyclinic was performed. Socioeconomic and behavioural features of the sample and the Singapore population of similar ages were comparable. Height was measured by clinic nurses using an ultrasonic height senor. Participants were interviewed for socioeconomic, behavioural, health and quality of life information. Clinical morbidity data was collected from the participants’ treating physicians. The SF-6D utility index and its Mental Health domain were the main endpoints. This study compared the effectiveness of cure of bulk-fill composites using polywave light-emitting diode (LED; with various curing modes), monowave LED, and conventional halogen curing lights. The bulk-fill composites evaluated were Tetric N-Ceram bulk-fill (TNC), which contained a novel germanium photoinitiator (Ivocerin), and Smart Dentin Replacement (SDR). The composites were placed into black polyvinyl molds with cylindrical recesses of 4-mm height and 3-mm diameter and photopolymerized as follows: Bluephase N Polywave High (NH), 1200 mW/cm² (10 seconds); Bluephase N Polywave Low (NL), 650 mW/cm² (18.5 seconds); Bluephase N Polywave soft-start (NS), 0-650 mW/cm² (5 seconds) → 1200 mW/cm² (10 seconds); Bluephase N Monowave (NM), 800 mW/cm² (15 seconds); QHL75 (QH), 550 mW/cm² (21.8 seconds). Total energy output was fixed at 12,000 mJ/cm² for all lights/modes, with the exception of NS. The cured specimens were stored in a light-proof container at 37°C for 24 hours, and hardness (Knoop Hardness Number) of the top and bottom surfaces of the specimens was determined using a Knoop microhardness tester (n=6). Hardness data and bottom-to-top hardness ratios were subjected to statistical analysis using one-way analysis of variance/Scheffe's post hoc test at a significance level of 0.05. Hardness ratios ranged from 38.43% ± 5.19% to 49.25% ± 6.38% for TNC and 50.67% ± 1.54% to 67.62% ± 6.96% for SDR. For both bulk-fill composites, the highest hardness ratios were obtained with NM and lowest hardness ratios with NL. While no significant difference in hardness ratios was observed between curing lights/modes for TNC, the hardness ratio obtained with NM was significantly higher than the hardness ratio obtained for NL for SDR. The tunability of nonlinear optical (NLO) absorption in Bi
3.15 Nd0.85 Ti3 O12 (BNT) ferroelectric thin films was investigated through the open aperture Z-scan method with femtosecond laser pulses at the wavelength of 800 nm. NLO absorption responses of the BNT films were observed to be highly sensitive to the film thickness. As the film thickness increases from 106.8 to 139.7 nm, the NLO absorption changes from saturable absorption (SA) to reverse saturable absorption (RSA). When the film thickness further increases to 312.9 nm, the RSA effect is enhanced. A band-gap-related competition between the ground-state excitation and the two-photon absorption is responsible for the absorption switching behavior. Cryptococcosis is a systemic fungal disease and meningitis is the most serious complication. The purpose of this study is to define problems related to its diagnosis and treatment. This is a retrospective analysis of 25 patients admitted from January 1978 to December 1981. All patients had cryptococcal neoformans meningitis proven by culture of cerebrospinal fluid. One patient had a predisposing illness, being on immunosuppressant therapy after a renal transplant 2 years ago. A progressively severe headache of recent onset was the most striking presentation. Fever was frequently absent as a symptom. Cranial nerve palsies were commonly seen. Impairment of consciousness and areflexia signified a poor prognosis as all four patients who died early in the course of treatment were comatose and two of them were areflexic on admission. In newly suspected cases at least 3 separate lumbar punctures are recommended as initial smears or cultures may be negative. Cerebral CT scans were abnormal in 12 patients and those with cerebral oedema or hydrocephalus had a poorer prognosis. Combined amphotericin B and 5-fluorocytosine therapy was the treatment of choice. If there is no relapse 3 years after completion of treatment, patients are considered as cured. Positive smears may remain for years after completion of treatment and retreatment is only indicated if the cultures are positive. Twenty patients are alive today and none of them have relapsed. One patient had vasculitis of both anterior cerebral arteries as a result of cryptococcal meningitis. [ABSTRACT FROM AUTHOR]

34. Osmotic Compressibility of Soft Colloidal Systems

- Abstract
A turbidimetric analysis of particle interaction of model pH-responsive microgel systems consisting of methacrylic acid−ethyl acrylate cross-linked with diallyl phthalate in colloidal suspensions is described. The structure factor at zero scattering angle, S(0), can be determined with good precision for wavelengths greater than 500 nm, and it measures the dispersion's resistance to particle compression. The structure factor of microgels at various cross-linked densities and ionic strengths falls onto a master curve when plotted against the effective volume fraction, φ
eff = kc, which clearly suggests that particle interaction potential and osmotic compressibility is a function of effective volume fraction. In addition, the deviation of the structure factor, S(0), of our microgel systems with the structure factor of hard spheres, SPY (0), exhibits a maximum at φeff ~ 0.2. Beyond this point the osmotic de-swelling force exceeds the osmotic pressure inside the soft particles resulting in particle shrinkage. Good agreement was obtained when the structural properties of our microgel systems obtained from turbidimetric analysis and rheology measurements were compared. Therefore, a simple turbidimetric analysis of these model pH-responsive microgel systems permits a quantitative evaluation of factors governing particle osmotic compressibility.- Published

35. Microstructure and Rheology of Stimuli-Responsive Nanocolloidal Systems&sbd;Effect of Ionic Strength

- Abstract
The effect of ionic strength on the rheological behavior of model pH-responsive nanocolloidal systems consisting of methacrylic acid−ethyl acrylate (MAA−EA) cross-linked with diallyl phthalate (DAP) was examined. Neutralization of acid groups increases the osmotic pressure exerted by counterions trapped in the polymeric network against ions in bulk solution, which is responsible for the swelling and increase in viscosity. Swelling decreases with increasing salt concentration as a result of reduced osmotic pressure inside the microgels, which is attributed to the charge shielding effect of counterions (salt) on the negatively charged carboxylate groups. Electromotive measurements using ion-selective electrodes confirmed that not all the counterions, that is, K+, remain mobile, but a fraction of these ions can penetrate the porous microgel particles to shield the negatively charged carboxylate groups. A consequence of this is that some of the Na+ counterions inside the particles are expelled, thus regaining their translational entropy, and become mobile sodium ions in the bulk solution. We successfully developed a new scaling law that relates the swelling ratio, Q, of microgels as a function of neutralization degree, α, cross-linked density, N
x , molar fraction of acidic units, y, and concentration of mobile counterions, C K + and CNa +, represented as (Nx /c 0 )(CK + + CNa +)Q + Q2/3 ∝ yNx α. The new scaling law no longer assumes that all the counterions are trapped inside the microgels. The proportionality reduces to the form Q ∝ (yαN x )3/2 in the absence of salt, that is, C K + + CNa + ≈ 0. By combining the results from light scattering and rheological measurements, we are able to correlate the microstructural evolution of the colloidal systems with their bulk rheological behavior.- Published

Thin films of Nd
3+ /Zr4+ cosubstituted Bi4 Ti3 O12 (BIT), i.e., Bi3.15 Nd0.85 Ti2.8 Zr0.2 O12 (BNTZ), were fabricated on Pt/Ti/SiO2 /Si(100) substrates by chemical solution deposition and annealed at different temperatures of 600, 650, 700, and 800 °C. The effects of annealing temperature on the microstructure, leakage current, ferroelectric, and dielectric properties of the BNTZ films were investigated in detail. Significantly, compared with the Bi3.15 Nd0.85 Ti3 O12 film, the BNTZ thin film has a lower coercive field (2Ec ) and leakage current density and a slightly larger remnant polarization (2Pr ). It shows that Nd3+ /Zr4+ cosubstitution in BIT film might be an effective way to improve ferroelectric properties of BIT. [ABSTRACT FROM AUTHOR]- Published

Background: \ud Glucagon-like peptide 1 receptor agonists differ in chemical structure, duration of action, and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. We aimed to determine the safety and efficacy of albiglutide in preventing cardiovascular death, myocardial infarction, or stroke.\ud \ud Methods: \ud We did a double-blind, randomised, placebo-controlled trial in 610 sites across 28 countries. We randomly assigned patients aged 40 years and older with type 2 diabetes and cardiovascular disease (at a 1:1 ratio) to groups that either received a subcutaneous injection of albiglutide (30–50 mg, based on glycaemic response and tolerability) or of a matched volume of placebo once a week, in addition to their standard care. Investigators used an interactive voice or web response system to obtain treatment assignment, and patients and all study investigators were masked to their treatment allocation. We hypothesised that albiglutide would be non-inferior to placebo for the primary outcome of the first occurrence of cardiovascular death, myocardial infarction, or stroke, which was assessed in the intention-to-treat population. If non-inferiority was confirmed by an upper limit of the 95% CI for a hazard ratio of less than 1·30, closed testing for superiority was prespecified. This study is registered with ClinicalTrials.gov, number NCT02465515.\ud \ud Findings: \ud Patients were screened between July 1, 2015, and Nov 24, 2016. 10 793 patients were screened and 9463 participants were enrolled and randomly assigned to groups: 4731 patients were assigned to receive albiglutide and 4732 patients to receive placebo. On Nov 8, 2017, it was determined that 611 primary endpoints and a median follow-up of at least 1·5 years had accrued, and participants returned for a final visit and discontinuation from study treatment; the last patient visit was on March 12, 2018. These 9463 patients, the intention-to-treat population, were evaluated for a median duration of 1·6 years and were assessed for the primary outcome. The primary composite outcome occurred in 338 (7%) of 4731 patients at an incidence rate of 4·6 events per 100 person-years in the albiglutide group and in 428 (9%) of 4732 patients at an incidence rate of 5·9 events per 100 person-years in the placebo group (hazard ratio 0·78, 95% CI 0·68–0·90), which indicated that albiglutide was superior to placebo (p

- Abstract
As a means of preventing secondary ischaemic stroke, angioplasty and stenting are considered potentially beneficial for patients with severe intracranial atherosclerotic stenosis. However, the role of stenting has been challenged since the publication of the first randomised controlled trial on Stenting versus Aggressive Medical Management for Preventing Recurrent stroke in Intracranial arterial Stenosis (SAMMPRIS). This indicated that aggressive medical management was superior to stenting using Wingspan to prevent recurrent stroke, because stenting has a high peri-procedural stroke and death rate. In this paper, we review the management of intracranial atherosclerosis, revisit the skepticism on stenting, and state our position on the topic in the form of recommendations. These are based on the prevalence of the disease in Hong Kong, the high risk of recurrent stroke despite medical therapy in the presence of haemodynamic intracranial stenosis without sufficient collaterals, an analysis of the weak points of SAMMPRIS, and results of clinical studies in Hong Kong.

- Abstract
Objectives: To define the role of bypass surgery and percutaneous transluminal angioplasty to manage critical limb ischaemia due to infrainguinal arterial occlusive disease., Design: Retrospective review., Setting: Regional hospital, Hong Kong., Patients: Consecutive patients treated for critical limb ischaemia, for whom data were prospectively collected in those who underwent: (i) infrainguinal percutaneous transluminal angioplasty first, or (ii) infrainguinal bypass surgery., Results: Among patients with critical lower limb ischaemia, 364 consecutive individuals having infrainguinal bypass operations and 100 having percutaneous transluminal angioplasty first were compared. The latter patients were older (77 vs 74 years, P=0.014) and had more co-morbidities but higher ankle pressure than those having bypass surgery. In the angioplasty-first group, 74% had favourable lesions (classified as TransAtlantic Inter-Society Consensus A/B). In the bypass group, operative mortality was higher (4% vs 1%, P=0.03) than that in the angioplasty-first group. Hospital mortality was comparable (8% vs 3%, P=0.15). In the bypass group, median hospital stay was longer than that in the angioplasty-first group (24 vs 4 days, P<0.001), and postoperatively they also had a higher median ankle-brachial index (0.92 vs 0.70, P<0.001) and superior long-term patency. In the bypass group, American Society of Anesthesiologists class 4 patients suffered very high operative and hospital mortality (15% and 31%, respectively). Long-term patency of percutaneous transluminal angioplasty depended on the TransAtlantic Inter-Society Consensus class of the treated lesion. Limb salvage rates at 3 years were 89% and 78% for percutaneous transluminal angioplasty first and surgical bypass, respectively (P=0.046). Long-term survival was poorer in the percutaneous transluminal angioplasty-first group (21% vs 51% at 5 years, P=0.04)., Conclusion: Infrainguinal bypass and percutaneous transluminal angioplasty are complementary. For TransAtlantic Inter-Society Consensus A and B lesions, percutaneous transluminal angioplasty should be offered first. For American Society of Anesthesiologists class 4 patients, percutaneous transluminal angioplasty should be considered first, regardless of the TransAtlantic Inter-Society Consensus class.

Introduction: Microalbuminuria is a marker of increased cardiovascular morbidity and mortality. It represents the earliest clinical evidence of diabetic nephropathy. Its early detection allows for implementation of individually-tailored cardiovascular risk reduction management programmes. Despite this, information on the prevalence of microalbuminuria in hypertensive patients with type 2 diabetes mellitus in Singapore is limited., Methods: The Microalbuminuria Prevalence Study (MAPS) assessed the prevalence of macroalbuminuria and microalbuminuria in consecutively-screened hypertensive adult patients with type 2 diabetes mellitus in ten Asian countries. This paper presents the results of a sub-analysis of data from patients in Singapore., Results: Singapore contributed seven percent of the overall enrolment into MAPS; a total of 499 patients were enrolled and 388 constituted the per-protocol population (patients with bacteriuria and haematuria were excluded). Overall, the prevalence of diabetic kidney disease was high. In our study population, 23.5 percent of patients had macroalbuminuria (95 percent confidence interval [CI] 21.3-25.6), and 48.5 percent of patients had microalbuminuria (95 percent CI 45.9-51.0). Only 28.1 percent (95 percent CI 25.8-30.4) of patients were normoalbuminuric. Associated factors were poor glycaemic control and poor blood pressure control., Conclusion: The high prevalence (72 percent) of microalbuminuria and macroalbuminuria found in hypertensive patients with type 2 diabetes mellitus in Singapore is a cause for concern. These findings highlight the need to screen for microalbuminuria and better manage hypertensive patients with type 2 diabetes mellitus, if we are to avoid a major increase in end-stage renal disease.

- Abstract
The neutralization behaviors of random and cross-linked methacrylic acid-ethyl acrylate (MAA-EA) copolymers were examined as a function of degree of neutralization (alpha) using potentiometric titration and laser light scattering techniques. The random MAA-EA copolymers exhibit a conformational transition from a compact latex particle to a swollen randomly coiled aggregate upon neutralization over a certain range of alpha. With further addition of NaOH, the swollen aggregates dissociate into several smaller clusters. This conformational change is controlled by the balance between electrostatic repulsion within ionized MAA groups and hydrophobic attraction of EA. The cross-linked MAA-EA copolymers do not undergo a drastic conformational change during neutralization. The polymer latex particles swell slightly upon neutralization, and the extent of chain expansion is proportional to MAA molar composition and inversely proportional to cross-linked density. The electrostatic Gibbs energy (DeltaG(el)) obtained from the potentiometric titration data indicates that a higher MAA portion is favorable for the deprotonization of both the random and cross-linked MAA-EA copolymers, suggesting that the dissociation is mainly dominated by polymer structure instead of the electrostatic attraction between H(+) and -COO(-). Moreover, static and dynamic light scattering results confirmed that the cross-linked latex particle exists as monodispersed hard sphere in the collapsed state, whereas in its swollen state the latex particle possesses a core-shell structure.

- Abstract
The binding of dodecyltrimethylammonium bromide (DoTab) to cross-linked methacrylic acid-ethyl acrylate (MAA-EA) copolymers with various MAA/EA molar ratios at different degrees of neutralization (alpha) was quantitatively studied using isothermal titration calorimetry, dynamic light scattering, surfactant selective electrode, and electrophoresis techniques. The surfactant binds to the polymers at all degrees of neutralization, but via different mechanisms. When alpha is sufficiently high, the binding is primarily electrostatic interaction between the surfactant and ionized polymer chains, which is reinforced by the micellization of electrostatically bound surfactant molecules. The saturation takes place at charge ratio ([DoTa(+)]/[ approximately COO(-)]) close to 1, indicating that the binding is a one-to-one charge neutralization between the cationic surfactant headgroups and anionic carboxylate sites of the polymers. When alpha is low, the binding of DoTab to the unneutralized polymers is driven by the hydrophobic interaction. The onset of hydrophobic binding takes place at DoTab concentration as low as 0.01 mM in 0.05 wt % polymer solution, where the saturation occurs at C(DoTab) approximately 0.19 mM and the amount of bound surfactant is approximately 0.09 mmol of DoTab/(g of polymer) at saturation concentration. The binding results in the formation of the polymer-surfactant complex. For the polymer with low MAA/EA molar ratio, the complex coagulates at a higher DoTab concentration that leads to phase separation; however, for polymers with high MAA/EA molar ratio, the complex remains dispersed and the mixture is stable even at high DoTab concentration.

49. Growth and spontaneous regression of a middle cerebral artery aneurysm after surgical clipping.

- Abstract
Summary: We present a case of growth and spontaneous regression of a middle cerebral artery aneurysm after surgical clipping. A 36-year-old woman who presented with grade II subarachnoid haemorrhage was found to have a right middle cerebral artery aneurysm. The aneurysm was surgically clipped in view of the morphology of the aneurysm. Second clipping was performed four months later as there was re-growth of the neck of the aneurysm. Again re-growth of the aneurysm was noted five months after the second clipping. As further clipping of the aneurysm would be difficult, external and internal carotid arteries bypass was the treatment option but unfortunately, the by-pass operation was unsuccessful. Interestingly, angiogram performed after the bypass surgery revealed partial thrombosis and decreasing size of the aneurysm. Complete obliteration of the aneurysm was noted without further intervention one month after the failed by-pass surgery.

- Abstract
We describe three patients with aberrant regeneration of the third nerve secondary to traumatic brain injury. The full blown features of the syndrome include horizontal gaze-eyelid synkinesis, pseudo-Graefe sign, limitation of elevation and depression of the eye with retraction of the globe on attempted vertical movements, adduction of the involved eye on attempted elevation or depression, pseudo-Argyll Robertson pupil and absent vertical optokinetic response. The 'misdirection' incidence in our study is 15%.
