4. Association Between Coronary Calcium Detection on Chest Computed Tomography and Ischemic Cardiovascular Events and Mortality in Asymptomatic Chronic Obstructive Pulmonary Disease Patients. Systematic Review of the Literature

Introduction: Literature suggests that there is a close relationship between chronic obstructive pulmonary disease (COPD) and cardiovascular (CV) disease. The aim of this study was to assess whether the presence of coronary calcium (CC) on chest computed tomography (CT) in asymptomatic COPD patients is associated with an increased risk of CV events and mortality. Material and methods: A systematic review of the literature was performed following PRISMA recommendations. Studies published in the last 20 years in four databases (PubMed, Web of Science, Embase and MEDLINE) were included. Results: Three hundred fifty articles were identified, eight of them met the selection criteria. The included studies, conducted between 2013 and 2024, were predominantly multicentre cohort studies. The meta-analysis showed that the presence of CC on chest CT of COPD patients is an independent predictor of CV events (hazard ratio 1.44, 95% CI 1.22–1.70) and associated with an increased mortality during the follow-up period (hazard ratio 1.57, 95% CI 1.35–1.83). Conclusions: Our analysis suggests that the identification of CC on chest CT scans of COPD patients may be useful in the early detection and treatment of CV disease in asymptomatic patients. Prospective, multicentre studies confirming our findings are needed to explore the potential impact of early detection and treatment of CV risk in COPD patients. Resumen: Introducción: La literatura sugiere que existe una estrecha relación entre la enfermedad pulmonar obstructiva crónica (EPOC) y la enfermedad cardiovascular (CV). El objetivo de este estudio fue evaluar si la presencia de calcio coronario (CC) en la tomografía computarizada (TC) torácica en pacientes asintomáticos con EPOC se asocia con un mayor riesgo de eventos CV y mortalidad CV. Material y métodos: Se realizó una revisión sistemática de la literatura siguiendo las recomendaciones PRISMA. Se incluyeron estudios publicados en los últimos 20 años en cuatro bases de datos (PubMed, Web of Science, Embase y MEDLINE). Resultados: Se identificaron 350 artículos, ocho de los cuales cumplieron los criterios de inclusión. Los estudios fueron realizados entre 2013 y 2024, con un diseño de cohortes y de carácter multicéntricos. El metaanálisis concluyó que la presencia de CC en la TC torácica de pacientes con EPOC es un predictor independiente de eventos CV (hazard ratio: 1,44; IC 95%: 1,22-1,70), y se relacionó con un aumento de la mortalidad CV en el período de seguimiento (hazard ratio: 1,57; IC 95%: 1,35-1,83). Conclusiones: El análisis sugiere que la identificación de CC en las TC torácicas de pacientes con EPOC puede ser útil en la detección precoz y en el tratamiento de la enfermedad CV en pacientes asintomáticos. Se necesitan estudios prospectivos multicéntricos que confirmen nuestros hallazgos para explorar el impacto potencial de la detección precoz y el tratamiento del riesgo CV en pacientes con EPOC.

Introducción. En los países de medianos y bajos ingresos, los datos sobre la mortalidad y los factores de riesgo en pacientes con enfermedad pulmonar obstructiva crónica son limitados. Objetivo. Identificar la incidencia de muerte y sus variables relacionadas en una población colombiana durante 12 meses de seguimiento. Materiales y métodos. Se llevó a cabo un estudio retrospectivo de sujetos con diagnóstico de enfermedad pulmonar obstructiva crónica en una clínica de tercer nivel en Colombia. Los cocientes de probabilidades se calcularon mediante un análisis de regresión logística multivariable con la variable de resultado “mortalidad a los 12 meses”. Resultados. Ingresaron 524 pacientes, de los cuales el 18,1 % (95 / 524) murió. La edad promedio fue de 69,7 (DE = 8,92) y el 59,2 % (310 / 524) eran mujeres. Las variables asociadas con la mortalidad fueron la edad (OR = 6,54; IC95%: 3,65-11,36; p < 0,001), años de exposición al humo de leña (OR = 4,59; IC95%: 1,64-12,82; p = 0,002), insuficiencia cardiaca crónica (OR = 1,81; IC95%: 1,13-2,91; p = 0,014), enfermedad cerebrovascular (OR = 3,35; IC95%: 1,04-10,75; p = 0,032) y enfermedad renal crónica (OR=6,96; IC 95%:1,15-41,67; p = 0,015). Al ajustar las variables en el análisis multivariado únicamente se mostró asociación entre el sexo (OR = 1,55; IC95%: 0,95-2,54; p = 0,008) y la edad (OR = 5,94; IC95%: 3,3-10,69; p < 0,001). Conclusión. La edad, los años de exposición al humo de leña, la insuficiencia cardiaca crónica, la enfermedad cerebrovascular y la renal crónica fueron variables clínicas asociadas a un desenlace fatal. Sin embargo, la edad y el sexo fueron las únicas relacionadas con la mortalidad al ajustarlas por factores de confusión.

Introduction: Learning encompasses both traditional and active methodologies, with active methods involving students taking on an engaged role and actively participating in class discussions. This study aims to compare the impact of 2 learning methods, team-based learning (TBL) and seminar-based learning (SBL), on nursing students' knowledge of caring for patients with chronic obstructive pulmonary disease (COPD) at the Islamic Azad University, Mahabad Branch. Methods: This semi-experimental study involved 60 nursing students divided into 2 groups. The TBL group received educational pamphlets 7 days before the workshop and participated in an Individual Readiness Assurance Test followed by a Team Readiness Assurance Test. The SBL group engaged in seminar presentations and discussions. Data were collected using a questionnaire with 2 sections: demographic information and a 24-question section on COPD care, with 20 questions approved after review by professors. Reliability was confirmed with a Cronbach's alpha of 0.86. Statistical analysis was conducted using SPSS 16 software. Results: Pre-intervention, both groups had comparable knowledge levels. Post-intervention, the TBL group showed significantly higher knowledge levels than the SBL group. However, 1 month later, both methods had similar effects on long-term learning. Conclusion: The TBL method was more effective than SBL in improving short-term knowledge of COPD care among nursing students, although both methods were effective for long-term learning. This study underscores the importance of active learning methodologies in nursing education. Resumen: Introducción: El aprendizaje abarca tanto metodologías tradicionales como activas, siendo los métodos activos aquellos en los que los estudiantes asumen un papel comprometido y participan activamente en las discusiones en clase. Este estudio tiene como objetivo comparar el impacto de dos métodos de aprendizaje, Aprendizaje Basado en Equipos (TBL) y Aprendizaje Basado en Seminarios (SBL), en el conocimiento de los estudiantes de enfermería sobre el cuidado de pacientes con Enfermedad Pulmonar Obstructiva Crónica (EPOC) en la Universidad Islámica Azad, Filial de Mahabad. Métodos: Este estudio semexperimental involucró a 60 estudiantes de enfermería divididos en dos grupos. El grupo TBL recibió folletos educativos 7 días antes del taller y participó en una Prueba de Garantía de Preparación Individual (IRAT) seguida de una Prueba de Garantía de Preparación de Equipo (T-RAT). El grupo SBL participó en presentaciones y discusiones de seminarios. Se recopilaron datos utilizando un cuestionario con dos secciones: información demográfica y una sección de 24 preguntas sobre el cuidado de la EPOC, con 20 preguntas aprobadas después de revisión por profesores. Se confirmó la fiabilidad con un alfa de Cronbach de 0.86. El análisis estadístico se realizó utilizando el software SPSS 16. Resultados: Antes de la intervención, ambos grupos tenían niveles de conocimiento comparables. Después de la intervención, el grupo TBL mostró niveles de conocimiento significativamente más altos que el grupo SBL. Sin embargo, un mes después, ambos métodos tuvieron efectos similares en el aprendizaje a largo plazo. Conclusión: El método TBL fue más efectivo que el SBL para mejorar el conocimiento a corto plazo sobre el cuidado de la EPOC entre los estudiantes de enfermería, aunque ambos métodos fueron efectivos para el aprendizaje a largo plazo. Este estudio enfatiza la importancia de las metodologías de aprendizaje activo en la educación en enfermería.

Introduction: the prevalence of osteoporosis among candidates for lung transplantation is high and its pathophysiology is multifactorial. Objectives: to evaluate differences in bone mineral density, risk of fractures and bone remodeling markers in patients with terminal lung disease, at the time they are evaluated for lung transplantation, comparing two types of pathologies. Material and methods: fifty-nine subjects, proposed to receive a lung transplant due to advanced lung disease, were included in this study. They were divided into two groups according to their respiratory pathology: chronic obstructive pulmonary disease (COPD) and diffuse interstitial pulmonary disease (ILD). Demographic data were collected and bone densitometry, blood analysis with markers of bone remodeling, spirometry, six-minute walk test (6MWT), echocardiography and cardiac catheterization were performed Results: no differences were found between the groups, regarding their age, sex, BMI or exposure to tobacco. A higher prevalence of osteoporosis and a higher FRAX were observed in the group with COPD. Regarding bone remodeling markers, higher parathyroid hormone (PTH) and higher osteocalcin were found in the COPD group. Vitamin D was lower in COPD patients. Conclusions: two out of three of the patients evaluated for lung transplantation had osteopenia or osteoporosis. The prevalence of osteoporosis and FRAX is higher in COPD patients. Vitamin D supplementation should be considered in certain patients. Differences in bone remodeling markers may be useful for suspected osteoporosis and therapeutic management. [ABSTRACT FROM AUTHOR]

Introducción. Las enfermedades crónicas no transmisibles representan la principal causa de muerte en el mundo y su prevalencia va en aumento debido a la transición epidemiológica. A pesar de los avances en su manejo, las cifras de control son deficientes y esto se atribuye a múltiples factores, como el cumplimiento del tratamiento farmacológico, que es uno de los más representativos y menos estudiados en la población colombiana. Objetivo. Establecer la frecuencia de casos que cumplieron con el tratamiento farmacológico en pacientes colombianos con hipertensión arterial, enfermedad cerebrovascular, diabetes mellitus, asma, enfermedad pulmonar obstructiva crónica y dislipidemia, entre el 2005 y el 2022. Materiales y métodos. Se llevó a cabo una revisión sistemática de la literatura y un metaanálisis de los estudios identificados mediante las bases de datos Medline y LILACS para sintetizar cuantitativamente el porcentaje de cumplimiento del tratamiento. Resultados. Catorce estudios cumplieron los criterios de inclusión y se analizaron 5.658 pacientes. La enfermedad pulmonar obstructiva crónica (EPOC) es una enfermedad frecuente y progresiva, pero prevenible y tratable. Sin embargo, se han detectado importantes áreas de mejora en su diagnóstico, tratamiento y seguimiento, así como en la adherencia terapéutica. Con el propósito de aumentar la detección precoz de la EPOC y su manejo adecuado en España, se ha diseñado un Plan de intervención comunitaria (PIC) para la colaboración entre farmacias comunitarias y centros de salud de atención primaria. El PIC incluye un procedimiento normalizado de trabajo (PNT), desarrollado por un grupo de profesionales de farmacia comunitaria, medicina de familia, neumología y enfermería de atención primaria. En él se describe el algoritmo que guiará la actuación de los farmacéuticos comunitarios, incluyendo las pruebas y dispositivos para el cribado poblacional de la EPOC, la evaluación de la enfermedad, la adhesión a inhaladores y la detección de errores críticos en el uso de inhaladores entre otros aspectos. El PNT incluye también un formulario para la derivación del paciente al centro de salud correspondiente, donde se llevarán a cabo una serie de acciones preestablecidas en el plan de actuación según el escenario clínico. Además, se propone evaluar la efectividad de la intervención con unos indicadores recogidos en el centro de salud y en farmacia comunitaria. Por último, se recomienda establecer un plan formativo en EPOC para la óptima implementación del plan y una encuesta a pacientes sobre su grado de satisfacción con la intervención. En conclusión, la implantación del PIC podría reducir el infradiagnóstico en EPOC y optimizar el manejo de los pacientes diagnosticados en España, donde los farmacéuticos comunitarios tendrían una función esencial en el abordaje de estos pacientes.

Introduction: The Bristol COPD Knowledge Questionnaire (BCKQ) presents detailed elements of symptoms, treatment, exacerbations, which make it a complete tool to assess knowledge of chronic obstructive pulmonary disease (COPD). Currently there is not a Colombian Spanish version of this questionnaire. Objective: Describe the process to translate and culturally adapt the BCKQ questionnaire to Colombian Spanish. Method: The process followed seven steps: preparation, translation from English to Spanish, synthesis meeting, back translation, back translation review, consultation with experts and informants, and final review of the questionnaire. Results: This study obtained a Spanish version of the Bristol COPD knowledge questionnaire. Consultation with thematic and language experts was required to resolve doubts in the translation process. The cultural adaptation included reaching the conceptual, semantic, and item equivalence of all the aspects evaluated in the questionnaire. Conclusions: The translation and cultural adaptation process should follow steps proposed in guidelines consulting language professionals, thematic experts, and official documents that facilitate achieving a version that is easy to understand according to the context of the questionnaire application. Acute respiratory distress syndrome (ARDS), chronic obstructive pulmonary disease (COPD) and COVID-19 have as a common characteristic the inflammatory lesion of the lung epithelium. The therapeutic options are associated with opportunistic infections, a hyperglycemic state, and adrenal involvement. Therefore, the search for new treatment strategies that reduce inflammation, and promote re-epithelialization of damaged tissue is very important. This work describes the relevant pathophysiological characteristics of these diseases and evaluates recent findings on the immunomodulatory, anti-inflammatory and regenerative effect of mesenchymal stem cells (MSC) and their therapeutic use. In Pubmed we selected the most relevant studies on the subject, published between 2003 and 2022 following the PRISMA guide. We conclude that MSCs are an important therapeutic option for regenerative treatment in COPD, ARDS, and COVID-19, because of their ability to differentiate into type II pneumocytes and maintain the size and function of lung tissue by replacing dead or damaged cells. El síndrome de dificultad respiratoria aguda (SDRA), la enfermedad pulmonar obstructiva crónica (EPOC) y la COVID-19 tienen en común provocar lesión inflamatoria del epitelio pulmonar. El tratamiento actual suele asociarse con infecciones oportunistas, hiperglicemia y afectación suprarrenal, por lo que es importante proponer opciones relacionadas con la disminución de la inflamación y estimulación de la reepitelización del tejido dañado. En esta revisión se detallan las características fisiopatológicas relevantes de dichas enfermedades y se evalúan los hallazgos recientes del efecto inmunomodulador, antiinflamatorio y regenerativo de las células troncales mesenquimales (MSC) y sus aplicaciones terapéuticas. Se seleccionaron los estudios sobresalientes del tema, publicados entre 2003 y 2022 en PubMed, siguiendo los criterios de la guía PRISMA. Las células troncales mesenquimales representan una opción importante de tratamiento regenerativo en pacientes con EPOC, SDRA y COVID-19, pues se diferencian a neumocitos tipo II, y mantienen el tamaño y la función del tejido pulmonar, supliendo a las células muertas o dañadas.

Introducción: el cuestionario Bristol COPD Knowledge Questionnaire (BCKQ) presenta elementos detallados de los síntomas, tratamiento, exacerbaciones, entre otros, que lo hacen una herramienta completa para evaluar el conocimiento de la enfermedad pulmonar obstructiva crónica (EPOC). En la actualidad no se cuenta con una versión en español colombiano del instrumento. Objetivo: describir el proceso realizado para traducir y adaptar culturalmente el cuestionario BCKQ al español colombiano. Método: el proceso se llevó a cabo en siete pasos: preparación, traducción inglés a español, reunión de síntesis, retrotraducción, revisión de retrotraducción, consulta con expertos e informantes y revisión final del cuestionario. Resultados: se obtuvo una versión en español del cuestionario Bristol de conocimientos de EPOC. Se requirió consulta con expertos temáticos y del lenguaje para resolver dudas en el proceso de traducción. La adaptación cultural tuvo en cuenta alcanzar la equivalencia conceptual, semántica y de ítem de todos los aspectos evaluados en el cuestionario. Conclusiones: en los procesos de traducción y adaptación cultural se deben seguir pasos propuestos en guías para traducción y adaptación, incluyendo la consulta a profesionales del lenguaje, expertos temáticos y documentos oficiales que faciliten lograr una versión de fácil comprensión en función del contexto de aplicación del instrumento.

α1-antitrypsin (AATD) deficiency is a genetic condition that causes low levels of this protein whose main function is to inhibit serine proteases, leading mainly to lung and liver damage. We describe the case of a 61-year-old man with symptoms of dyspnea, fixed airflow obstruction, decreased CO2 diffusion capacity and lung volumes that demonstrated hyperinflation, with low exposure risk, in whom severely decreased α1- antitrypsin levels were evident. Subsequently, a genotypic study was carried out that confirmed the presence of PI*Z alleles (PI*ZZ), considering it a serious decrease. Introduction: the process of medical care for patients with COPD exacerbation in the province of Sancti Spíritus shows deficiencies in the uniformity of the use of the clinical method in a structured way based on the diagnosis and treatment of these patients. Objective: to design a Clinical Practice Guideline for hospital emergency medical care for patients with COPD exacerbation.Methods: an investigation was carried out that corresponds to the researchdevelopment typology, multistage, with a mixed approach, in the province of Sancti Spíritus from 2017 to 2019. Results: methodological deficiencies were detected in the medical care process of patients with exacerbation of COPD. A clinical practice guideline was developed and executed that allowed the structuring of theoretical, clinical and investigative elements to evaluate this type of patients. Conclusions: the clinical practice guideline proved to be relevant, feasible and effective; it made possible an approach to patients with exacerbation of COPD and allowed raising the level of knowledge of doctors about this condition. Introduction: The non invasive ventilation is an effective treatment in patients with acute lung disease. Objective: To describe the demographic, clinical, ventilatory and hemogasometric characteristics in patients treated with non invasive ventilation. Methods: An observational, descriptive, longitudinal and prospective study of 234 patients with chronic obstructive lung disease, non-invasive ventilated in the Intensive Cares Unit of Saturnino Lora Teaching Clinical-Surgical Provincial Hospital in Santiago de Cuba, was carried out, from January, 2011 to September, 2021. Results: In the series there was a prevalence of male sex, non hospital acquired pneumonia and chronic heart failure; while the mean age was 71 years and non invasive ventilation failed in 53.8 % of those affected. Also, the respiratory rate decreased from 34.3 to 23.5 breaths per minute in the second hour and an increase in pH was also observed, as well as in the relationship arterial pressure of O2/inspired fraction of O2 and oxygen saturation to the pulsioximetry/inspiratory fraction of O2. The arterial pressure of CO2 had average values of 61.8 mmHg to the beginning and of 60.7 mmHg in the second hour. Conclusions: The basal values of clinical, hemogasometric and ventilatory variables improved after treatment with non invasive ventilation. Among the parameters associated with the treatment failure there were heart frequency, respiratory frequency, arterial pressure of CO2, coma Glasgow scale, pH and leaks; equally, the long stay, the ventilation for more than 48 hours and mortality were related to this failure. Introducción. La enfermedad pulmonar obstructiva crónica es un problema de salud pública a nivel mundial y el cese del consumo de tabaco es la medida más efectiva para disminuir la incidencia y su progresión. Esta medida es especialmente eficaz en personas envejecidas, en quienes las consecuencias son más agudas a causa de los cambios biopsicosociales, cambios que aumentan la probabilidad de infecciones, dependencia funcional y fragilidad. El objetivo de este reporte es describir los cambios en el hábito tabáquico de una persona mayor con enfermedad pulmonar obstructiva crónica, tras ser incluida en un plan de cuidado organizado y según las etapas del proceso de Enfermería dirigido al cese del hábito tabáquico. Metodología. Reporte de caso de Enfermería bajo el Modelo de Promoción de la Salud de Nola Pender y el proceso de Enfermería, articulado con la taxonomía diagnóstica North American Nursing Diagnosis Association International, la Clasificación de Resultados de Enfermería (Nursing Outcomes Classification) y la Clasificación de Intervenciones de Enfermería (Nursing Interventions Classification). Resultados. Tras el proceso de Enfermería se evidencia la cesación del hábito tabáquico en la persona mayor y se destaca en el proceso la participación del núcleo familiar y el reconocimiento de los riesgos en salud asociados. Conclusiones. Se reconoce el rol de Enfermería como preponderante en la prevención y el abandono del hábito tabáquico que, orientado por un proceso de cuidado organizado, con respaldo disciplinar y científico, ayuda a mejorar el estado de salud y manejo de los factores de riesgo. Introduction. Quitting tobacco consumption is the most effective measure for reducing the incidence and progression of chronic obstructive pulmonary disease, which is a global public health issue. This measure is especially effective in seniors, in whom the consequences are more acute due to biopsychosocial changes, changes that increase the probability of infection, functional dependence and fragility. The objective of this report is to describe the changes in the tobacco habits of a senior with chronic obstructive pulmonary disease after being included in an organized health care plan according to the Nursing process’ stages for quitting tobacco habits. Methodology. A nursing case report under Nola Pender’s Health Promotion Model and the Nursing process, articulated with the diagnostic taxonomy North American Nursing Diagnosis Association International, Nursing Outcomes Classification and Nursing Interventions Classification. Results. It was evident the senior quit their tobacco habits after the Nursing process was carried out, and participation from family members and recognition of the associated health risks stood out in the process. Conclusions. The role of Nursing is pivotal in preventing and quitting tobacco habits, which, guided by an organized health care process with disciplinary and scientific support, helps improve health conditions and manage risk factors. Introdução. A doença pulmonar obstrutiva crônica é um problema de saúde pública global e a cessação do consumo de tabaco é a medida mais eficaz para reduzir sua incidência e progressão. Essa medida é especialmente eficaz em idosos, nos quais as consequências são mais agudas devido a alterações biopsicossociais, alterações que aumentam a probabilidade de infecções, dependência funcional e fragilidade. O objetivo deste relato é descrever as mudanças no hábito de fumar de um idoso com doença pulmonar obstrutiva crônica, após a inclusão em um plano de cuidados organizado e de acordo com as etapas do processo de Enfermagem visando a cessação do tabagismo. Metodologia. Relato de caso de Enfermagem sob o Modelo de Promoção da Saúde de Nola Pender e o processo de Enfermagem, articulado com a taxonomia diagnóstica North American Nursing Diagnosis Association International, a Classificação de Resultados de Enfermagem Nursing Outcomes Classification e a Classificação de Intervenções de Enfermagem Nursing Interventions Classification. Resultados. Após o processo de Enfermagem, evidencia-se a cessação do tabagismo no idoso e destaca-se a participação do núcleo familiar e o reconhecimento dos riscos à saúde associados no processo. Conclusões. O papel da Enfermagem é reconhecido como preponderante na prevenção e cessação do tabagismo, o que, pautado por um processo assistencial organizado, com respaldo disciplinar e científico, auxilia na melhoria do estado de saúde e no manejo dos fatores de risco.

Results: In the series there was a prevalence of male sex, non hospital acquired pneumonia and chronic heart failure; while the mean age was 71 years and non invasive ventilation failed in 53.8 % of those affected. Also, the respiratory rate decreased from 34.3 to 23.5 breaths per minute in the second hour and an increase in pH was also observed, as well as in the relationship arterial pressure of O2/inspired fraction of O2 and oxygen saturation to the pulsioximetry/inspiratory fraction of O2. The arterial pressure of CO2 had average values of 61.8 mmHg to the beginning and of 60.7 mmHg in the second hour. Conclusions: The basal values of clinical, hemogasometric and ventilatory variables improved after treatment with non invasive ventilation. Among the parameters associated with the treatment failure there were heart frequency, respiratory frequency, arterial pressure of CO2, coma Glasgow scale, pH and leaks; equally, the long stay, the ventilation for more than 48 hours and mortality were related to this failure.

The article presents a Community Intervention Plan (CIP) for the management of chronic obstructive pulmonary disease (COPD) in Spain. The CIP aims to improve the diagnosis, treatment, and monitoring of COPD through collaboration between community pharmacies and health centers. The plan includes a standardized work procedure that describes the actions to be taken by community pharmacists, such as population screening for COPD and disease evaluation. It also proposes to evaluate the effectiveness of the intervention and establish a training plan on COPD. The implementation of the CIP could reduce underdiagnosis of COPD and improve patient management in Spain. [Extracted from the article]

Introduction: The monitoring of the carbon dioxide exhaled is frequently used in the intensive cares units, but its use in non invasive ventilation is scarce. Objective: To identify the association between the blood pressure of carbon dioxide and the carbon dioxide exhaled, during non invasive ventilation, in patients with acute chronic obstructive lung disease. Methods: An observational, descriptive, longitudinal and prospective study of 126 patients admitted with acute chronic obstructive lung disease was carried out, they were treated with non invasive ventilation, in the Intensive Cares Unit of Saturnino Lora Torres Teaching Provincial Clinical-Surgical Hospital in Santiago de Cuba, from January, 2019 to the same month in 2022, selected by intentional non probabilistic sampling. Clinical, ventilatory and hemogasometric variables were analyzed, of which the minimum and maximum values were identified, as well as the mean, standard and medium deviation. The Pearson correlation coefficient was applied. Results: The average values of carbon dioxide exhaled were 57.83 ± 8.9 and those of arterial pressure of carbon dioxide, 59.85± 9.3. When analyzing the correlation among the variables, positive correlation was observed among both, for a Pearson correlation coefficient of 0.920. Conclusions: The monitoring of carbon dioxide exhaled acts as a variable to consider in the monitoring of patients with acute chronic obstructive lung disease, treated with non invasive ventilation, whenever the appropriate face mask is used and the leaks are controlled, with strong correlation with the arterial pressure of the carbon dioxide. Introduction: Dyspnea is one of the most characteristic symptoms of Chronic Obstructive Pulmonary Disease and is directly related to the reduction in a person's capacity for exercise, performance of daily life activities, and self-care. The purpose of this study was to understand the level of dyspnea, the ability to perform daily life activities, and the dependence level on self-care in individuals with Chronic Obstructive Pulmonary Disease. Methodology: Exploratory, descriptive and cross-sectional study, with a quantitative approach. Results: The sample was formed by 52 participants, predominantly male, with an average age of 72 years. Regarding the degree of dyspnea, assessed by the modified Medical Research Council dyspnea scale, 76.9% of the participants reported dyspnea ? 2. The London Chest Activity of Daily Living scale revealed that 55.8% of participants had limitations in performing daily life activities. According to the Barthel Index, 67.3% of the sample were independent in self-care. Conclusion: Participants in the study revealed a high level of dyspnea and limitations in performing daily life activities, but when assessing the degree of autonomy in self-care, the majority of participants are independent. Therefore, the Specialist Nurse in Rehabilitation interventions should aim to optimize the capabilities of individuals with Chronic Obstructive Pulmonary Disease, promote their autonomy and quality of life, as well as empower them for disease self-management., Introducción: La disnea es uno de los síntomas más característicos de la Enfermedad Pulmonar Obstructiva Crónica y está directamente relacionada con la disminución de la capacidad de ejercicio, la realización de actividades de la vida diaria y el autocuidado en la persona. El presente estudio tuvo como objetivo profundizar en el conocimiento sobre el nivel de disnea, la capacidad para llevar a cabo actividades de la vida diaria y la dependencia en el autocuidado en personas con Enfermedad Pulmonar Obstructiva Crónica. Metodología: Estudio exploratorio, de naturaleza descriptiva, transversal, con un enfoque cuantitativo. Resultados: La muestra estuvo compuesta por 52 participantes, predominantemente de sexo masculino, con una edad promedio de 72 años. En cuanto al grado de disnea, evaluado mediante la escala modificada del Medical Research Council, el 76,9% de los participantes informó de una disnea ? 2. La Escala de Actividades de la Vida Diaria London Chest mostró que el 55,8% de los participantes presentaban limitaciones en la realización de actividades de la vida diaria. Según la evaluación del Índice de Barthel, el 67,3% de la muestra era independiente en el autocuidado. Conclusión: Los participantes en el estudio presentan un alto nivel de disnea y limitaciones en la realización de actividades de la vida diaria, pero cuando se evalúa el grado de autonomía en el autocuidado, la mayoría de los participantes son independientes. Conclusão: Os participantes no estudo apresentam alto nível de dispneia e limitação na realização das atividades de vida diária, porém quando avaliado o grau de autonomia para o autocuidado, a maioria dos participantes é independente. Assim, as intervenções do Enfermeiro Especialista em Enfermagem de Reabilitação deverão ter como objetivos a otimização das capacidades da pessoa com Doença Pulmonar Obstrutiva Crónica, a promoção da sua autonomia e a qualidade de vida, bem como a capacitação para a autogestão da doença.

- Abstract
El consumo de tabaco es uno de los factores de riesgo que mayor mortalidad produce a nivel mundial y es el principal factor de riesgo del cáncer de pulmón. El radón es el segundo factor de riesgo en el desarrollo de cáncer de pulmón en fumadores y el primero en nunca fumadores. Los resultados de esta tesis permiten caracterizar la epidemia tabáquica en España al estudiar la evolución de la prevalencia y la mortalidad atribuida a su consumo. Además, se estima por primera vez la mortalidad atribuida al consumo de tabaco y a la exposición a radón residencial en las 17 comunidades autónomas de España y al consumo de tabaco en las siete regiones de Portugal. Por último, se presenta una guía metodológica que permite aumentar la calidad de las estimaciones de mortalidad atribuida a diferentes factores de riesgo.

- Abstract
Introduction: EPOCONSUL is the first national audit to analyze medical care for COPD in pulmonology departments in Spain. The main objective was to perform a retrospective analysis to determine the distribution of GesEPOC 2017 COPD risk levels and to evaluate clinical activity according to the new recommendations. Material and methods: This is a cross-sectional clinical audit in which consecutive COPD cases were recruited over one year. The study evaluated risk and clinical phenotype according to GesEPOC 2017, and their correlation with the clinical interventions employed. Results: The most common risk category was high risk (79.8% versus 20.2%; p < 0.001), characterized by a higher level of severity on BODE and BODEx indexes, and a higher comorbidity burden. The most common phenotype was non-exacerbator. The most commonly used treatment in low-risk patients was bronchodilator monotherapy (34.8%) and triple therapy in high-risk patients (53.7%). High risk was most frequently characterized by phenotype (57.6% versus 52%; p = 0.014) and pulmonary function test results: lung volume (47.7% versus 35.8%; p < 0.001), lung diffusion (51.4% versus 42.1%; p < 0.001) and walk test (37.8% versus 15.8%; p < 0.001). Conclusions: Most patients treated in pulmonology departments were high-risk and non-exacerbator phenotype. 40. Acceptance by Nurses of the use of a Massive Open Online Course in a context of continuous training

Introduction: Scientific and technological developments require continuous nurses' updating. This update would be achieved through continuous training and lifelong learning, which face some obstacles such as the need for reconciliation with personal and family life. The use of massive, online and asynchronous courses allows nurses access to current and targeted content to their needs, enhancing autonomy and learning management. With this study, we aim to assess the ease of use, perceived utility and intention for using a Massive Open Online Course (MOOC) in continuous nursing training. Methodology: Observational, exploratory and descriptive study using a non-probabilistic convenience sample. The study took place in the Department of Internal Medicine of a Central Hospital in the North of Portugal. Results: 141 nurses participated in this study. The results showed high perceived user-friendliness and utility of the learning programme, and the nurses' intention for using a MOOC in the future. Discussion: The perceived usefulness, ease and intention are supported by the perceived quality in the contents, by the adequacy to perceived needs, using active pedagogical strategies such as the integration of virtual clinical simulation, and the perceived accessibility to training content. Conclusion: The use of a MOOC, made available from Information and Communication Technologies, has a high potential for acceptance and use in continuing nursing training, contributing not only to the safety and quality of care, but also for an adequate management of nurses' personal and professional lives., Introducción: Los avances científicos y tecnológicos exigen que los enfermeros se mantengan permanentemente actualizados. Esta actualización se concretaría a través de la formación continua y del aprendizaje permanente, que afrontan algunos obstáculos como la necesidad de conciliación con la vida personal y familiar. El recurso a cursos masivos, online y asíncronos posibilita a los enfermeros el acceso a contenidos actuales y dirigidos a sus necesidades, potenciando autonomía y gestión del aprendizaje. Con este estudio pretendíamos evaluar la facilidad de uso, utilidad percibida e intención para la utilización de un Massive Open Online Course (MOOC) en la formación continua de los enfermeros. Metodología: Estudio observacional, exploratorio y descriptivo, recurriendo a una muestra de conveniencia no probabilística. El estudio tuvo lugar en el Departamento de Medicina Interna de un Hospital Central del Norte de Portugal. Resultados: En este estudio participaron 141 enfermeros. Los resultados mostraron un gran nivel de percepción de la facilidad de uso y la utilidad del curso de formación, así como la intención de los enfermeros de usar un MOOC más tarde. Discusión: La utilidad, facilidad e intención percibidas, son soportadas por la calidad percibida en los contenidos, por la adecuación a las necesidades percibidas, por la utilización de estrategias pedagógicas activas como la integración de la simulación clínica virtual, y por la accesibilidad percibida a los contenidos formativos. Conclusión: El uso de un MOOC, que se ofrece a partir de las tecnologías de la información y la comunicación, tiene un alto potencial de aceptación y uso en la formación continuada de enfermería, contribuyendo no solo a la seguridad y calidad de la atención, pero también para una adecuada gestión de la vida personal y profesional de los enfermeros., Introdução: A evolução científica e tecnológica exige dos enfermeiros uma atualização permanente. O estudo decorreu no Departamento de Medicina Interna de um Hospital Central do Norte de Portugal. Resultados: Neste estudo participaram 141 enfermeiros. Os resultados revelaram uma elevada facilidade de utilização e utilidade percebidas do programa formativo, assim como a intenção dos enfermeiros para a utilização futura de um MOOC. Discussão: A utilidade, facilidade e intenção percebidas, são suportadas pela qualidade percebida nos conteúdos, pela adequação às necessidades percebidas, pela utilização de estratégias pedagógicas ativas como a integração da simulação clínica virtual, e pela acessibilidade percebida aos conteúdos formativos. Conclusão: A utilização de um MOOC, disponibilizado a partir das Tecnologias de Informação e Comunicação, tem um elevado potencial de aceitação e utilização na formação contínua em enfermagem, contribuindo não só para a segurança e qualidade dos cuidados, mas também para uma adequada gestão da vida pessoal e profissional dos enfermeiros.

41. Guía española de la enfermedad pulmonar obstructiva crónica (GesEPOC). Diagnóstico y tratamiento hospitalario de la agudización

Las agudizaciones de la enfermedad pulmonar obstructiva crónica (EPOC), especialmente cuando precisan asistencia hospitalaria, son episodios que generan una notable morbimortalidad, gran carga asistencial y elevados costes, por lo que resulta prioritario reducir sus consecuencias, de acuerdo a las mejores evidencias científicas disponibles. El documento actual presenta las recomendaciones que establece la Guía Española de la EPOC (GesEPOC), para guiar el proceso diagnóstico y terapéutico del paciente con agudización que precisa asistencia hospitalaria. Como principales novedades, se proponen nuevas definiciones que tratan de diferenciar la aparición de una nueva agudización de otros conceptos vinculados como son la recaída o el fracaso terapéutico y se establece un proceso diagnóstico en 3 pasos dirigidos a: 1) establecer el diagnóstico de la agudización; 2) valorar su gravedad; y 3) identificar la etiología. Desde el punto de vista terapéutico, la guía propone una actuación en urgencias hospitalarias estructurada en 4 fases: 1) admisión y clasificación; 2) asistencia; 3) evolución, espera y observación; y 4) resolución, orientación y transferencia. También se incluyen recomendaciones para el paciente hospitalizado, criterios de ingreso, de alta y pautas de seguimiento y control orientadas hacia los fenotipos clínicos. 42. Development of a web application to evaluate spirometry metrics and clinical variables to support COPD diagnosis in primary care

Introduction. COPD is the third mortality cause in the world and the development of useful diagnostic tools is necessary to improve timely diagnostic rates in primary care settings.Objective. To develop a web application that displays spirometric and clinical information, including respiratory symptoms and risk factors, to facilitate the diagnosis of COPD.Materials and methods. In this cross-sectional study, an Expert Consensus was carried out with three specialists through Delphi method to choose the relevant variables for COPD diagnosis. We developed a Python-based web application for the diagnosis of COPD, which displays the clinical variables deemed relevant by the experts, along the spirometric curve and metrics. Results. Twenty-six clinical variables were included in the web application for the diagnosis of COPD. A fourth expert used the Web Application to classify a cohort of 695 patients who had undergone spirometry in a third-level centre and had answered at least one of 5 questionnaires for COPD screening. Out of the 695 subjects, 34% had COPD according to the expert that diagnosed them using the web application. Only 42% of the patients in the COPD group had received a previous COPD diagnosis and 19% of the patients in the No COPD group had been misdiagnosed with the disease. Conclusion. We developed a web application that displays demographic and clinical information, as well as spirometric data, to facilitate the process of diagnosing COPD in primary care settings., Introducción. La EPOC es la tercera causa de mortalidad en el mundo y es necesario el desarrollo de herramientas diagnósticas útiles para mejorar las tasas de diagnóstico oportuno en los entornos de atención primaria. Objetivo. Desarrollar una aplicación web que muestre información clínica y espirométrica, incluyendo síntomas respiratorios y factores de riesgo, para facilitar el diagnóstico de la EPOC.Materiales y métodos. En este estudio transversal, se realizó un consenso de expertos con tres especialistas a través del método Delphi para elegir las variables relevantes para el diagnóstico de EPOC. Desarrollamos una aplicación web basada en Python que muestra la información clínica relevante según los expertos, junto con la curva y las métricas de la espirometría para el diagnóstico de la EPOC. Resultados. Se incluyeron 26 variables clínicas en la aplicación web para el diagnóstico de la EPOC. Un cuarto experto utilizó la aplicación Web para clasificar una cohorte de 695 pacientes, a los que se les había realizado una espirometría en un centro de tercer nivel y que habían contestado al menos uno de los 5 cuestionarios para la detección de la EPOC. De los 695 sujetos, el 34% tenían EPOC según el experto que les diagnosticó a través de la aplicación web. Sólo el 42% de los pacientes del grupo con EPOC habían recibido un diagnóstico previo de EPOC y el 19% de los pacientes del grupo sin EPOC habían sido diagnosticados erróneamente con la enfermedad. Conclusión. Desarrollamos una aplicación web que muestra información demográfica y clínica, así como datos espirométricos, para facilitar el proceso de diagnóstico de la EPOC en entornos de atención primaria.

43. [Clinical and prognostic differences in chronic obstructive pulmonary disease in women referred to respiratory rehabilitation: A gendered view].

Introduction: Respiratory rehabilitation programmes improve clinical and functional outcomes in patients with chronic obstructive pulmonary disease (COPD), but it is unknown whether their impact differs between men and women., Objectives: To assess possible sex differences in semiology, comorbidities and prognosis in COPD patients referred to respiratory rehabilitation., Material and Methods: Post-hoc analysis of a prospective cohort of COPD patients referred to rehabilitation from December 2012 to January 2020. Mortality and hospitalisations adjusted for sex, age, severity and body mass index were assessed. Statistical tests used: Chi-square, Student's t, linear regression model and Kaplan-Meier survival curve., Results: Of 173 patients (age 66.6 years; 70% severe COPD), there were 36 (20.8%) women. Compared to men, women were younger (-4.8 years [95% CI 1.5-8.0; P=.004]) and reported greater dyspnoea on the modified Medical Research Council scale (+0.4 points [95% CI 0.1-0.9; P=.026]). During follow-up, 21 (12.2%) patients died and 79 (45.9%) required ≥2 hospital admissions, with no significant differences between sexes. Despite having lower comorbidity (-0.8 Charlson index points [95% CI -1.3 to -0.3, P=.004]), women had a higher prevalence of depression (53.6% vs. 22.7%, P=.001) and anxiety (37% vs 16.7%, P=.020) than men., Conclusions: Prognosis in terms of mortality and hospital admissions is not different between men and women. 44. Clinical features, management in the emergency department and mortality of acute heart failure episodes in patients with chronic obstructive pulmonary disease.

Objectives: This study aims to analyse differences in clinical and therapeutic management for patients with chronic obstructive pulmonary disease (COPD) who present to the emergency department with acute heart failure (AHF). Additionally, it examines mortality rates during such episodes., Methods: We included patients diagnosed with AHF at 50 Spanish emergency departments from 2012 to 2022 who also had COPD. We compared their baseline characteristics, decompensation episodes, and emergency department management with those of AHF patients without COPD during the same period. We collected data on in-hospital and 30-day all-cause mortality, investigating differences between the two groups using crude and adjusted logistic regression models., Results: A total of 21,694 AHF patients were analysed (median age = 83 years, 56% female), including 4,942 (23%) with COPD. COPD patients were generally younger and more frequently male, with a higher prevalence of comorbidities (excluding valve disease and dementia, which were more common in non-COPD patients). They exhibited a worse respiratory functional class (NYHA) but a better overall functional capacity (Barthel Index). Decompensation in COPD patients was more often triggered by infection and less frequently by tachyarrhythmia, hypertensive crisis, or acute coronary syndrome. While there were differences in clinical findings in the emergency department, the severity assessed by the MEESSI-AHF Scale was similar across both groups. In terms of emergency department management, a higher proportion of COPD patients received oxygen therapy, non-invasive ventilation, bronchodilators, corticosteroids, and antibiotics, while fewer received intravenous nitroglycerin, and they were hospitalized more frequently. In-hospital mortality rates were 8.1% for patients with COPD and 7.5% for those without (OR = 1.088, 95% CI = 0.968-1.224), with 30-day mortality rates of 11.0% and 10.0%, respectively (OR = 1.111, 95% CI = 1.002-1.231). After adjusting for clinical characteristics, decompensation episodes, and emergency department management, these odds ratios decreased to 1.040 (95% CI = 0.905-1.195) and 1.080 (95% CI = 0.957-1.219), respectively., Conclusion: Patients with AHF and COPD exhibit distinct clinical and therapeutic management characteristics in the emergency department and require more frequent hospitalization. Background: Hypogonadism is a common finding of chronic obstructive pulmonary disease (COPD). However, the prevalence of hypogonadism in COPD varies among studies. The aim of this study was to determine and compare the prevalence of hypogonadism in men with and without COPD., Methods: We conducted a cross-sectional study with 134 patients with stable COPD and 70 age-matched men with non-COPD. Hypogonadism was defined by the presence of symptoms according to the Androgen Deficiency in Aging Males questionnaire, along with total testosterone deficiency (<300ng/dL)., Results: Patients had a mean age of 68 years (SD, 6), a body mass index of 28kg/m
2 (SD, 6), and 17% were current smokers. The prevalence of hypogonadism was 41.8% in COPD men (N=56, 95%CI, 33-51) and 10.0% in non-COPD men (N=7, 95%CI, 4-20), with a prevalence ratio of 4.2 (95%CI, 2.0-8.7, p<0.001). The prevalence of low total testosterone concentrations (<300ng/dL) were significantly higher in COPD patients vs the control group (47.0% vs 15.7%, p=<0.001). In the COPD group, 89.3% of patients had hypogonadotropic hypogonadism and 10.7%, hypergonadotropic hypogonadism. The prevalence of hypogonadism was higher in severe vs non-severe COPD patients (55.8% vs 35.2%; p=0.024)., Conclusions: The prevalence of hypogonadism was high and greater in COPD vs non-COPD men. - Abstract
The basis of COPD maintenance treatment is the long-acting bronchodilators and the inhaled corticosteroids. Faced with the recent modifications in the clinical practice guidelines, we have carried out a review of studies that contrast the various therapeutic alternatives and pharmacological agents within each category, with the fundamental purpose of shedding light on which of these options prove to be more effective. Triple therapy stands out as essential in poorly controlled patients or with an eosinophilic phenotype, surpassing dual therapy. However, among the combinations of LAMA/LABA or LAMA/LABA/IC, no drug is observed to be superior in the reviewed evidence. Although triple therapies include corticosteroids, there does not appear to be a significant increase in side effects or pneumonia. Regarding monotherapy with LAMA, no significant differences are seen between the drugs, but in dual therapy with LABA/IC, the budesonide/formoterol combination seems to offer better control than fluticasone/salmeterol., (Copyright © 2024 Sociedad Española de Médicos de Atención Primaria (SEMERGEN). La enfermedad pulmonar obstructiva crónica (EPOC) es una enfermedad con amplias implicaciones sociales y económicas, su incidencia mundial seguirá aumentando, siendo para la próxima década la cuarta causa global de muerte. El deterioro físico subsecuente a la disfunción pulmonar, trae consigo cambios multisistémicos que desencadenan la reducción de la capacidad funcional del individuo, dichos fenómenos están relacionados con la severidad de los síntomas y también el nivel detrimento del sistema cardiovascular y musculoesquelético. Por ende, los pacientes con EPOC tienden a reducir su calidad de vida relacionada con la salud (CVRS) y consigo, una reducción de la expectativa de vida. El tratamiento del paciente con esta enfermedad no se limita a la farmacoterapia y tratamientos paliativos secundarios, sino que también trasciende en la identificación e intervención de factores que afectan la CVRS. La presente revisión tuvo los objetivos de explorar, analizar y exponer evidencia científica conspicua que asocia a la funcionalidad y CVRS en pacientes con EPOC. Los hallazgos muestran la importancia de evaluar e intervenir los factores que reducen la CVRS. Alternativas como la educación y coaching en salud parecen lograr reducir el impacto de la enfermedad en diversas esferas del individuo.

Introducción: Numerosos estudios demuestran que actualmente la enfermedad pulmonar obstructiva crónica (EPOC) está mal controlada e infradiagnosticada en numerosos pacientes debido a la utilización inadecuada de la farmacoterapia. El conocimiento de los pacientes sobre los complejos medicamentos que manejan puede no ser suficiente, y es aquí donde la dispensación activa del farmacéutico comunitario juega un papel esencial. Los objetivos de este artículo son medir la adherencia al tratamiento y el grado de satisfacción con la intervención del farmacéutico de los pacientes que utilizan inhaladores. Método: Se ha realizado un estudio observacional transversal en 105 pacientes diagnosticados de enfermedad pulmonar obstructiva crónica (EPOC) tratados con inhaladores y que acuden a la farmacia comunitaria. A todos los pacientes se les realizó el test de Morisky-Green de adherencia al tratamiento y un test para valorar el grado de satisfacción de los pacientes que utilizan inhaladores con la intervención del farmacéutico. Resultados: Presentan adherencia al tratamiento el 60% de los pacientes. La adherencia al tratamiento es superior en mujeres que en hombres, y mayor en pacientes de edad igual o superior a 65 años. Respecto al grado de satisfacción con el farmacéutico se observa que el 52,4 % de los pacientes considera adecuada la intervención llevada a cabo por el farmacéutico y el 43,8 % totalmente adecuada. Conclusiones: Los pacientes con EPOC tienen una adherencia al tratamiento bastante mejorable y están satisfechos con la intervención del farmacéutico en la mejora de la utilización de los inhaladores.
