The composition of food baskets can significantly influence the water consumption rate and society's environmental impact. This study evaluates the sustainability of Iran's food and nutrition security by evaluating five food baskets using water and carbon-footprint indices. These food baskets are the common Iranian food basket (CIFB), the vegetarian food basket (VFB), the Ministry of Health desired food basket (MHDFB), the Swiss Society for Nutrition food basket (SSNFB), and the Mediterranean food basket (MFB). This study also examines the role of Iran's agricultural sector under the Paris Climate Change Agreement (COP21). The TOPSIS multi-criteria decision-making method was used to identify the most desirable food basket. The results of this study show that the VFB could be the most desirable alternative to the CIFB. Replacing the CIFB with the VFB causes a 10% and 21% lower water and carbon footprint, respectively. From a 30-year perspective (around 2050), choosing the VFB would reduce the blue water footprint by 19.7 BCM and the gray water footprint by 3.6 BCM compared to the CIFB. According to the provisions of COP21 regarding Iran's voluntary program to reduce greenhouse gas emissions, the agricultural sector will be one of the sources of greenhouse gas emissions that will face an upward trend. Hence, to adhere to this voluntary commitment, these emissions should be reduced by changing the CIFB to the VFB or reducing emissions in other sectors, such as industry. Choosing the VFB can reduce 10.7 million tons of CO2 emissions, which can be a significant step for the agricultural sector in Iran in reducing greenhouse gas emissions. Discourse markers (DMs) assist and persuade EFL classroom interlocutors to monitor their discourse production and comprehension process. This exploratory research investigated the index of pragmatic use (IPU), rate of use, frequency, and the pragmatic functions of "well" in Iranian university EFL teachers' and learners' classroom interactions. The researchers applied Jucker's (1997) inventory to explore the pragmatic functions of "well." The data was collected from four university EFL classes. The results revealed that the IPU was 72%, rate of use was 9.7%, and frequency of distribution was 0.98%. Compared with the London Lund corpus, the IPU and the rate of use were very low, but the frequency of occurrence was very high. The findings indicate the improper and unnatural pragmatic input offered in this EFL context. Also, the instances of "well" applied by the interactants in the monitoring of discourse comply with Jucker's index of functions in 90% of the cases by the teachers and in 65% by the students. They overused "well" as a face-threat mitigator, underused it as a frame, and the teachers' gender played no significant part. The insufficiency in IPU, rate of use, extra and excessive frequency, and imbalanced approach in pragmatic functions of "well" provide the evidence for reformulation in EFL education. Classroom discourse variables, i.e. teacher training, material production procedures, and classroom strategies and teaching methodology need to be directed in a way to provide chances for the teachers and the students to observe how native speakers apply "well" in authentic situations, underline its instances of use, and discuss its pragmatic functions in texts.

This study aims at evaluating perceived service quality in Zanjan University of Medical Sciences (ZUMS). This study was cross-sectional and authors surveyed educational services at ZUMS. Through stratified random sampling, 384 students were selected and an adapted SERVQUAL instrument was used for data collection. Data analysis was performed by using SPSS 11.5. The results indicated that there were the negative mean score of quality gap in all dimensions of the service quality. Overall services quality mean was (-1.6 ± 0.78). The highest and lowest differences of mean scores between students' expectations and perceptions were in the responsiveness and (-1.83 ± 1.01) and reliability (-1.33 ± 0.79) dimensions respectively. Paired t-test showed the significant statistical differences between students' expectations and perceptions (? < 0.001) in all dimensions of the service quality. Thus, improvement actions are felt in all domains of the educational service quality; Students rated the responsiveness as the most important dimension of service quality in ZUMS.

Background: Poisoning among children and adolescents is a public health problem worldwide. To take preventive measures, the pattern of this problem should be determined. This study aimed to describe the demographic characteristics of poisoning in children and to investigate the relationship between the types of poisoning and demographic factors in children in Kermanshah province. Methods: This cross-sectional, descriptive-analytical study was conducted on 250 children and adolescents under 18 years of age who were referred to Mohammad Kermanshahi Pediatric Hospital in Kermanshah province due to poisoning during 2019–2022. The demographic and epidemiological data of patients were extracted from their medical files and analyzed. Results: Out of 250 cases of poisoning, 173 (69.2%) cases were unintentional, 96 (55.5%) of whom were boys. Further, 77 (30.8%) cases of poisoning were intentional, of whom 49 (63.6%) were girls. There was a significant difference between gender and intentional and unintentional poisonings (p-value = 0.005). The median age of unintentional poisoning was 3 (IQR = 2.5) and that of intentional poisoning was 14 (IQR = 2). Most cases of poisoning were in cities, 145 (83.8%) of them were unintentional and 66 (85.7%) were intentional. Most cases of intentional and unintentional poisonings occurred in spring 2017 (35.1%) and autumn 2016 (34.6%), respectively. The most common causes of poisoning were narcotics (n = 36, 34.3%) and drugs (n = 35, 33.3%) in the age group 0–3 years and drugs (n = 46, 66.9) in the age group 11–18 years. Conclusions: The most common causes of poisoning were narcotics and drugs in children and drugs in adolescents. To prevent poisoning in children, parents are required to increase their knowledge of the safe storage of narcotics and drugs, such as not storing methadone in a water bottle. Background: The causative agent of plague, Yersinia pestis, is maintained in nature via a flea-rodent cycle. Western Iran is an old focus for plague, and recent data indicate that rodents and dogs in this region have serological evidence of Y. pestis infection. The purpose of this study was to conduct a large-scale investigation of Y. pestis infection in shepherd dogs, rodents, and their fleas in old foci for plague in Western Iran. Materials and methods: This study was conducted in Hamadan province from 2014 to 2020. Rodents and fleas were collected from various locations throughout this region. Y. pestis was investigated in rodent spleen samples and fleas using culture, serology, and real-time PCR methods. Additionally, sera samples were collected from carnivores and hares in this region, and the IgG antibody against the Y. pestis F1 antigen was assessed using an ELISA. Results: In this study, 927 rodents were captured, with Meriones spp. (91.8%) and Microtus qazvinensis (2.6%) being the most prevalent. A total of 6051 fleas were collected from rodents and carnivores, most of which were isolated from Meriones persicus. None of the rodents or fleas examined tested positive for Y. pestis using real-time PCR and culture methods. Meanwhile, IgG antibodies were detected in 0.32% of rodents. All serologically positive rodents belonged to M. persicus. Furthermore, none of the sera from the 138 carnivores (129 sheepdogs, five Vulpes vulpes, four Canis aureus), and nine hares tested positive in the ELISA test. Conclusion: This primary survey of rodent reservoirs shows serological evidence of Y. pestis infection. Western Iran is an endemic plague focus, and as such, it requires ongoing surveillance. Author summary: This article investigates the ongoing presence of the plague in Iran and stresses the importance of continuous monitoring in the western region, a major area for endemic plague. Rodents are the primary carriers of diseases in their natural habitats and contribute significantly to the ongoing presence of infectious cycles. The study focuses on the presence of Yersinia pestis, the bacteria responsible for the plague, in guard dogs, rodents, and fleas in the historical center of western Iran. Background and Aims: Spinal canal stenosis is one of the most common vertebral column diseases, which can lead to disability. Developing a registry system can help in research on the prevention and effective treatment of it. This study designs a minimum data set (MDS) as the first step in creating a registry system for spinal canal stenosis. Method: The present research is of applied‐descriptive type, performed in 2022. First, the applicable data elements about the disease were selected from a vast range of English and Farsi references, including peer reviewed articles, academic books, credible websites, and medical records of hospitalized patients. Through the extracted data, the primary MDS plan was designed as a questionnaire. The validity of the questionnaire was conducted via asking the opinion of experts (neurosurgeons, physiotherapists, epidemiologists, and health information management specialists). Also, its reliability was calculated via Cronbach ⍺ coefficient, which was 86%. Finally, the MDS of the spinal canal stenosis national registry system (for Iran) was confirmed through a two stage Delphi technique. Data analysis was applied through descriptive statistics via SPSS21 software. Results: The proposed MDS is offered in two general sets of data: administrative and clinical. For the administrative data set, 40 data elements had been proposed, as five classes. Twenty‐six of them were confirmed. In the clinical section, 95 data elements had been proposed in 14 classes; 94 of which were finally confirmed. Conclusion: Since there is no spinal canal stenosis MDS available, this study can be a turning point in the standardization of the data on this disease. Moreover, these precise, coherent, and standard data elements can be contributed to improving disease management and enhancing the public healthcare quality. Tularemia and Q fever are endemic diseases in Iran; however, little information is available on the prevalence of the causative agents, Coxiella burnetii and Francisella tularensis, in Iranian ticks. This study investigated C. burnetii and F. tularensis among hard ticks in this country. We collected ticks from livestock and other mammals in Guilan, Mazandaran, Golestan (northern Iran), Kurdistan (western Iran), and West Azerbaijan (northwestern Iran) provinces. Genomic DNA from collected ticks was extracted and screened for C. burnetii and F. tularensis using Real-time PCR. A total of 4,197 ticks (belonging to 12 different species) were collected, and Ixodes ricinus (46.4%), Rhipicephalus turanicus (25%), and Rhipicephalus sanguineus sensu lato (19.1%) were the most collected species. Of 708 pooled tick samples, 11.3% and 7.20% were positive for C. burnetii and F. tularensis, respectively. The genus of Rhipicephalus had the highest (18.3%) C. burnetii infection among the collected tick pools (P<0.001). Furthermore, the most positive pools for F. tularensis belonged to Haemaphysalis spp. (44.4%). Kurdistan had the most significant percentage of C. burnetii-infected ticks (92.5%), and there was a meaningful relationship between the provinces and the infection (P< 0.001). The ticks from Golestan exhibited the highest F. tularensis infection rate (10. 9%), and the infection showed no significant relationship with the provinces (P = 0.19). Ticks collected from grasslands had a higher Coxiella burnetii infection rate than those collected from animals (39.4% vs. 7.9%; p<0.01). However, ticks collected from animal surfaces had a slightly higher rate of Francisella tularensis infection than those collected from grasslands (7.6% vs. 3.9%; p = 0.24). Here, we demonstrated the presence of both pathogens in the north (Guilan, Mazandaran, and Golestan provinces), the west (Kurdistan province), and the northwest (West Azerbaijan province) of Iran. Objectives: This study aimed to examine the effect of the coronavirus disease 2019 (COVID-19) outbreak on the hospitalization rate, emergency department (ED) visits, and outpatient clinic visits in western Iran. Methods: We collected data on the monthly hospitalization rate, rate of patients referred to the ED, and rate of patients referred to outpatient clinics for a period of 40 months (23 months before and 17 months after the COVID-19 outbreak in Iran) from all 7 public hospitals in the city of Kermanshah. An interrupted time series analysis was conducted to examine the impact of COVID-19 on the outcome variables in this study. Results: A statistically significant decrease of 38.11 hospitalizations per 10,000 population (95% confidence interval [CI], 24.93-51.29) was observed in the first month of the COVID-19 outbreak. The corresponding reductions in ED visits and outpatient visits per 10,000 population were 191.65 (95% CI, 166.63-216.66) and 168.57 (95% CI, 126.41-210.73), respectively. After the initial reduction, significant monthly increases in the hospitalization rate (an increase of 1.81 per 10,000 population), ED visits (an increase of 2.16 per 10,000 population), and outpatient clinic visits (an increase of 5.77 per 10,000 population) were observed during the COVID-19 pandemic. Conclusion: Our study showed that the utilization of outpatient and inpatient services in hospitals and clinics significantly declined after the COVID-19 outbreak, and use of these services did not return to pre-outbreak levels as of June 2021. Background: Plague may recur after several decades in its endemic regions; therefore, the continuous monitoring of wildlife is essential, even when no human cases are reported in the old foci. The present study was conducted to monitor rodents and their ectoparasites as well as carnivores to learn about the epidemiology of plague infection in an old focus of Iran. Methodology: The present study was conducted from 2019 to 2020 in Takestan county of Qazvin Province in northwestern Iran. Rodents were caught using live traps, and their fleas were separated. Blood and spleen specimens were taken from the captured rodents. Serum samples were also collected from sheepdogs and wild carnivores. The collected samples were tested by culture, serology (ELISA), and molecular methods to detect Yersinia pestis infection. Findings: A total of 399 small mammals were caught, of which 68.6% were Meriones persicus. A total of 2438 fleas were collected from the rodents, 95.3% of which were Xenopsylla buxtoni. Overall, 23 out of 377 tested rodents (5.7%, CI 95%, 3.9–9.0) had IgG antibodies against the F1 antigen of Y. pestis, and all the positive samples belonged to M. persicus. Nine (4.8%) out of 186 collected sera from the sheepdogs' serum and one serum from the Canis aureus had specific IgG antibodies against the F1 antigen of Y. pestis. There were no positive cases of Y. pestis in the rodents and fleas based on the culture and real-time PCR. Conclusion: Serological evidence of Y. pestis circulation was observed in rodents and carnivores (sheepdogs and C. aureus). The presence of potential plague vectors and serological evidence of Y. pestis infection in the surveyed animals could probably raise the risk of infection and clinical cases of plague in the studied region. Training health personnel is therefore essential to encourage their detection of possible human cases of the disease. Background: Alcohol poisoning causes several clinical signs and symptoms. The present study was carry out to examine the epidemiology and characteristics of poisoned patients with alcohol, who were treated at the Sina hospital Tabriz in Iran. Purpose: The purpose of this study was to determine the c linical features and laboratory tests in alcohol poisoning. Methods: In the current expressive cross-sectional report, during 2018-2020 details were obtained over referring to all 91 alcohol poisoned patients' files. The data were obtained based on the time interval between poisoning and time of admission to hospital. The data were then analyzed through SPSS software and descriptive statistics. Results: All over this period, 91 cases of alcohol-poisoned patients were hospitalized, out of which 81 patients (90%) were male and 9 patients (10%) were female. Mostly ethanol was used in poisoning (68.7%) and methanol (28%). 56% of alcohol poisoned patients used alcohol for euphoria, 33.3% used it for suicide, and 10.7% used it recreationally. Most poisoning symptoms occurred between 6 to 12 hours after poisoning. Neurological symptoms occurred among 70 patients (76.9%), 52 cases (57%) had gastrointestinal symptoms, and 19 patients (20.8%) had visual symptoms. Conclusion: Most of poisoned patients were men below the age of 30 years and unemployed. Alcohol consumption for euphoria were more common among people with higher education occasionally for recreation and those with high school education level were for suicide to get rid of problems from living environment. Particular attention of health authorities should be directed towards alcohol drinking prevention measures as well as production and availability of surrogate alcohols in the community. Background and Aims: One of the main vital needs for self‐care in patients with advanced cancer is information need. Meeting this need has significant positive effects on improving their treatment and care. This study was conducted to identify the unmet information needs of patients with advanced cancer in Iran. Methods: This exploratory study was performed from July to February 2021 in the Kerman University of Medical Sciences cancer treatment centers. Oncologists selected eligible patients by purposeful sampling method. Semistructured and in‐depth interviews were conducted with selected patients to collect data. Interviews continued until data saturation. Each interview was audio‐recorded and transcribed verbatim. Results: In the interviews, 15 patients with advanced cancer ranging in age from 43 to 65 years participated. The most common type of cancer in women was breast (71.4%) and prostate (50%) in men. The two main categories of "types of unmet information needs" and "reasons for not meeting information needs" were extracted from the analysis of patient interviews, with six and four subcategories, respectively. Conclusion: Cancer patients had a large number of unmet information needs. At the time of identifying the unmet information needs of cancer patients, the basic reasons for not meeting these needs should also be considered because cultural differences and social gaps in societies are inevitable. Background: Associations between the COVID-19 pandemic and hospitalizations have not been studied Iran. This study aimed to examine the impact of the COVID-19 pandemic on hospital admissions for nine categories of disease in seven public hospitals in Kermsnahah city, the capital of Kermsnahah province, in the west of Iran. Methods: Data on monthly hospitalization rates (number of hospitalizations per 100,000 population) were collected for nine categories of disease for a period of 40 months (23 months before and 17 months after the COVID-19 outbreak in Iran) from the health information systems of all seven public hospitals in Kermanshah city. Categories of disease included those related to pregnancy, childbirth and the puerperium period, neoplasms, diseases of the digestive, respiratory, circulatory, genitourinary and nervous systems, mental and behavioural disorders, and infectious and parasitic diseases. Population data were extracted from the Statistics Centre of Iran. An interrupted time series analysis with segmented regression was used to examine the impact of COVID-19 on hospital admissions. Findings: Average monthly hospitalization rates fell for all nine categories of disease included in the study after the onset of the pandemic, with overall rates of 85.5 per 100,000 population in the period before the COVID-19 outbreak and 50.4 per 100,000 population after the outbreak began. The relative reduction in hospitalizations for the nine diseases was 56.4%. Regression analysis of monthly data indicated a sharp decrease in hospitalisations during the first month after the COVID-19 outbreak, which was statistically significant for all diseases (p < 0.001). After the initial reduction following onset of the pandemic, significant increases were observed for some diseases, including neoplasms (increase of 3.17 per 100,000 population; p < 0.001), diseases of the digestive system (increase of 1.17 per 100,000 population; p < 0.001) and diseases related to pregnancy, childbirth and the puerperium period (increase of 1.73 per 100,000 population). For other categories of disease, rates significantly declined, including infectious and parasitic diseases (decrease of 2.46 per 100,000 population; p < 0.001). Hospitalization rates did not increase to pre-pandemic levels for any disease, with the exception of those related to pregnancy, childbirth and the puerperium period. Conclusions: Our study indicated that the COVID-19 pandemic had a significantly negative effect on hospitalizations in Iran. Objectives: This study aimed to determine the efficacy of the therapeutic use of play on improving the attention span of children with Mild Intellectual Disability (MID) in elementary school. Methods: This was a quasi-experimental study with a pre-test-post-test with a control group so that 30 girls with MID were selected from an exceptional school in Tehran City, Iran, via convenience sampling method and assigned to the experimental and control groups. The attention span of all subjects was assessed by the Toulouse-Pieron test and the Cognitive Diagnostic Battery (CDB) before and after the therapeutic sessions. The experimental group participated in 16 therapeutic play sessions (35 minutes for each session) for 8 weeks, but the control group did not. The analysis of covariance (ANCOVA) was used to analyze the data. Results: The Mean±SD age were 9.80±1.32 and 9.73±1.22 for the experimental and control groups, respectively. In addition, the mean attention span in the experimental group varied from 7.60 to 18.78 after the therapeutic sessions. In addition, after the therapeutic sessions, a significant difference (P<0.001) was observed between the experimental and control groups relating to attention span. According to the Eta quotient, 94% of the variation in the attention span of the experimental group is due to participating in the intervention sessions. Discussion: Therapeutic use of play is an effective method to improve the attention span of children with MID. Therefore, the therapeutic use of play can be applied to improve the academic performance of students with MID and reduce negative outcomes due to inattention. [ABSTRACT FROM AUTHOR]

Backgrounds: This study aims to estimate and compare the parameters of some univariate and bivariate count models to identify the factors affecting the number of mortality and the number of injured in road accidents.Methods: The accident data used in this study are related to Kermanshah province in march2020 to march2021. Accidents areas were divided into 125 areas based on density characteristics. In a one-year period, 3090 accidents happened on the suburban roads of Kermanshah province, which resulted in 398 deaths and 4805 injuries. Accident information, including longitude and latitude of accident location, type of accident (fatal and injury), number of deaths, number of injuries, accident type, the reason of the accident, and the kind of accident were all included as population-level variables in the regression models. We investigated four frequently used bivariate count regression models for accident data in the literature.Results: In bivariate analysis, except for the DNM model, there is a reasonable decrease in the AIC measures of the saturated model compared to the reduced model for the other three models. For the injury models, MSE is lowest, respectively for DIBP (137.87), BNB (289.46), BP (412.36) and DNM (3640.89) models. These results are also established for death models. But, in univariate analysis, only injury models almost present reasonable results.Conclusions: Our findings show that the IDBP model is better suitable for evaluating accident datasets than other models. Motorcycle accidents, pedestrian accidents, left turn deviance, and dangerous speeding were all significant variables in the IDBP death model, and these parameters were linked to accident mortality. Though increasing food supply in order to meet the rising demand for nutrition is a global social imperative, reducing the dependence on imports of essential food commodities is both an economic and a geo-political imperative for national governments. However, in light of the Sustainable Development Goals, although Zero Hunger (SDG2) and Good Health and Well-Being (SDG3) can be ensured within a country when the inhabitants are well-nourished and staple food items remain affordable to one and all, oftentimes, there are trade-offs in the process, with the environmental dimensions—SDGs 13 (Climate action), 14 (Life below water) and 15 (Life on Land). In this paper, using a combination of Environmental-Life Cycle Assessment (E-LCA) and Data Envelopment Analysis (DEA), the authors have evaluated the eco-efficiency of 169 wheat cultivation systems in the Golestan province in the north of Iran. Benchmarking performance based on the best-performing wheat farms and optimizing (decreasing essentially) the consumption of resources, will enable an average reduction of between 10% and 16% in global warming, acidification, eutrophication, and non-renewable energy usage of the wheat cultivation systems in the case study region. Background: We aimed to evaluate the rate of infection by endoparasites amongst rodents in the western regions of Iran to enhance the level of knowledge amongst health authorities in this entity. Methods: This study was conducted in the west and southwest of Kurdistan Province, including the cities of Sanandaj, Marivan, and Sarvabad. The field mission of this work was performed in three seasons' spring, summer, and autumn. The rodents were captured alive and their gastrointestinal tracts were evaluated for the worm endoparasites. Results: Herein, 208 rodents from 15 types of seven species, including Apodemus, Meriones, Mus, Sciurus, Cricetulus, Microtus, and Dryomys, were captured. In addition, 67 (32.21%) rodents were infected with endoparasites and 10 types of worms endoparasites, including Syphacia muris, Streptophagus spp., Mastophorus muris, Skrjabinema spp., Trichostrongylus spp., Trichuris muris, Hymenolepis nana, Hymenolepis dimimuta, Heligmosomoide spp., and other oxiuros were isolated from their gastrointestinal tracts. Most of these parasites (60%) were isolated from their small intestine whereas they were least (10%) found in their stomach and cecum. Conclusion: Having compared the results of this study with other studies in different regions of Iran, there is a higher variety of rodents and worm parasites in these regions of Iran. Background: Colorectal cancer has increased in Middle Eastern countries and exposure to environmental pollutants such as heavy metals has been implicated. However, data linking them to this disease are generally lacking. This study aimed to explore the spatial pattern of age-standardized incidence rate (ASR) of colon cancer and its potential association with the exposure level of the amount of heavy metals existing in rice produced in north-eastern Iran.Methods: Cancer data were drawn from the Iranian population-based cancer registry of Golestan Province, north-eastern Iran. Samples of 69 rice milling factories were analysed for the concentration levels of cadmium, nickel, cobalt, copper, selenium, lead and zinc. The inverse distance weighting (IDW) algorithm was used to interpolate the concentration of this kind of heavy metals on the surface of the study area. Exploratory regression analysis was conducted to build ordinary least squares (OLS) models including every possible combination of the candidate explanatory variables and chose the most useful ones to show the association between heavy metals and the ASR of colon cancer.Results: The highest concentrations of heavy metals were found in the central part of the province and particularly counties with higher amount of cobalt were shown to be associated with higher ASR of men with colon cancer. In contrast, selenium concentrations were higher in areas with lower ASR of colon cancer in men. A significant regression equation for men with colon cancer was found (F(4,137) = 38.304, P < .000) with an adjusted R2 of 0.77. The predicted ASR of men colon cancer was - 58.36 with the coefficients for cobalt = 120.33; cadmium = 80.60; selenium = - 6.07; nickel = - 3.09; and zinc = - 0.41. The association of copper and lead with colon cancer in men was not significant. We did not find a significant outcome for colon cancer in women.Conclusion: Increased amounts of heavy metals in consumed rice may impact colon cancer incidence, both positively and negatively. While there were indications of an association between high cobalt concentrations and an increased risk for colon cancer, we found that high selenium concentrations might instead decrease the risk. Further investigations are needed to clarify if there are ecological or other reasons for these discrepancies. Regular monitoring of the amount of heavy metals in consumed rice is recommended. Background and objectives: Gouda Cheese is regarded as a high quality and one of the most popular cheeses in the world. The defining characteristics of Gouda cheese are its yellow color, great aroma and taste of caramel sweetness. The cheese should be well chilled before waxing, to get better seal. The seal is very important to prevent contamination with molds and putrefactive bacteria. Cheese wax is made from paraffin with additional microcrystalline to make pliable for better seal. The aim of this study was to investigate antibacterial effects of Gouda cheese wax. Methods: Gouda cheese wax samples were collected from four different manufacturers in Iran. The total count of coliforms, Escherichia coli, Salmonella, coagulasepositive Staphylococcus and mold and yeast on the samples was determined. The antimicrobial activities of Gouda cheese wax against E. coli, S. aureus, Saccharomyces cerevisiae, Aspergillus brazilissis and Salmonella enterica were investigated by determining minimum bactericidal concentration and minimum inhibitory concentration. Results: The results indicated that all Gouda cheese wax samples were prepared in accordance with the national standards. In addition, the examined wax samples had no antimicrobial properties against the tested microorganisms. Background and objectives: Dried Black Curd (DBC), also known as Gharahghorut (Persian), is a dairy product produced from curd of yogurt or doogh. The aim of this study was to evaluate microbial contamination of industrially and traditionally produced DBC in Iran. Methods: Four DBC brands holding certification of the Institute of Standards and Industrial Research of Iran (ISIRI), Iran Ministry of Health and Medical Education (IMHME) and ISO were purchased from a market in Tehran, Iran. Microbial monitoring for presence of coliform, Escherichia coli, coagulase-positive Staphylococcus aureus, mold and yeast was performed using different basic, enriched, selective and differential media including peptone water agar, MacCankey agar, blood agar and brain heart infusion agar, etc. according to the ISIRI standards (No.13299). Results: None of the tested samples had microbial contamination. However, in one traditional DBC sample, the population of mold and yeast was higher than the acceptable level defined by the ISIRI standard. Conclusion: We found no contamination with coagulase-positive S. aureus, E. coli and coliforms in four brands of traditionally and industrially produced DBC in Iran. This indicates that the hygienic practices designated by the IMHME are well-implemented in the industrial sector. However, the mold and yeast contamination in traditionally produced DBC should be prevented by applying hygienic practices during the process of manufacturing and distribution. Background and Objectives: The prevalence of multiple sclerosis (MS) differs between different regions of Iran. This study aimed to investigate changes in prevalence and incidence rates of MS in ten ethnic and cultural zones of Iran set by the Ministry of Health and Medical Education. Materials and Methods: Information on MS incidence and prevalence was obtained from the national registry of MS during the study period 2006–2013. Demographic data were acquired from the Statistical Centre of Iran. Results: A rising trend of MS prevalence was observed in Iran, and overall, the prevalence of MS increased by 3.67% per year. The highest prevalence rates of MS, and the most dramatic increase in MS prevalence, occurred in the central areas of Iran. However, during the 7-year period, the prevalence gradually increased also in southern and western areas. Conclusion: The prevalence and incidence of MS have been rapidly increasing in all regions of Iran, especially from central to western and southern regions. [ABSTRACT FROM AUTHOR]

Objectives: The present study aimed to adapt the strategic plans of medical science universities to the components of the third millennium universities. Materials and Methods: This cross-sectional study included all the faculty members of 5 top universities of Tehran Province in Iran. To collect data, a reliable and valid researcher-made questionnaire was developed based on the characteristics of higher education institutions in the third millennium (content validity index >0.79, content validity ratio > 0.62, α = 0.88). Statistical analysis was conducted by SPSS. Results: The highest mean score of the population's attitude was related to the visions of the universities (mean =3.44). Moreover, the highest mean score (the components of university headquarters) of the attitude belonged to environmental indicators (3.53) while the lowest mean score was related to administrative and financial indicators (3.4). Based on the results, there was a significant difference between the attitudes of faculty members (P < 0.01). Conclusions: Concerning the accordance between mission and headquarter activities, medical sciences universities were somewhat far from the third millennium universities and should be accountable for the quality of their actions. Finally, they need to show their merits and should work to promote themselves by acquiring necessary certificates and quality assurance. [ABSTRACT FROM AUTHOR]

OBJECTIVES: The aim of this study is to investigate the therapeutic property of hydroalcoholic extract of Fenugreek seeds in nonalcoholic fatty liver disease (NAFLD) in adult patients. METHODS: This randomized, placebo-controlled, parallel trial was conducted from November 2014 to June 2017. Patients aged between 18 and 70 years old with confirmed NAFLD were recruited from the Motahhari clinic, affiliated to Shiraz University of Medical Sciences, Iran. Participants either received 1 g hydroalcoholic extract of Fenugreek seeds or placebo daily for 3 months. The primary outcomes were changes in serum alanine transaminase and FibroScan controlled attenuation parameter score. Secondary outcome measures were changes in other laboratory data, liver stiffness measure, liver steatosis percent, and anthropometric variables. Participants were randomly assigned to the groups using blocked randomization method. Participants, investigators, and statistician were blinded to treatments allocation. RESULTS: After screening eighty patients, thirty patients met the inclusion criteria and were divided into two groups (1:1). After 3 months, two and four patients did not complete the trial in Fenugreek and placebo group, respectively. The changes in the anthropometrics, laboratories and FibroScan measurements were not statistically significant between the two groups. CONCLUSION: The evidence to prove the efficacy of the Fenugreek seeds' hydroalcoholic extract in NAFLD was not strong enough; hence, further experiments are still needed to assess the possible efficacy of Fenugreek on the treatment of NAFLD. The antibiotic residues were investigated in raw and pasteurized milk produced in Neyshabur region, Iran. To this end, 82 milk samples, including raw and pasteurized milk of different brands and local raw milk samples of collection centers, were collected from Neyshabur region in two seasons during 2016. The antibiotic four-plate test (FPT) was performed to examine the presence of tetracycline, flomocaine, sulfonamide, enrofloxacin, aminoglycoside, and macrolide in the samples. The data were analyzed by the chi-squared test and logistic regression analysis to determine significant differences among variables at the significance level of p < .05. The results revealed that, among the 82 milk samples, 16 (47.2%) collected raw milk, 10 (41.6%) raw milk in the factories, and 6 (25%) pasteurized milk samples were positive. The findings showed that the most positive antibiotics belonged to the flomocaine, tetracycline, sulfonamide, and enrofloxacin groups, respectively. However, no aminoglycoside or macrolide was found in the samples. Analysis of milk samples performed in two seasons and the site of the sample collection showed a significant difference in the antibiotic residue content (p < .05). Logistic regression analysis showed that the odds of contamination in winter were 3.73 times that of fall (p = .006). Positive antibiotic residues were present in a large number of milk samples. This study highlights the need for special measures and effective monitoring programs to reduce the hazard of chemical contamination in the Neyshabur region. [ABSTRACT FROM AUTHOR]

The objective of this study was to determine the genetic parameters of autosomal and sexlinked effects for birth weight (BW), weaning weight (WW), average daily gain from birth to weaning (ADG) and kleiber ratio (KR) of Makuie sheep. The data set used in this study was collected at Makuie sheep breeding station in West Azerbaijan province, Iran, between 1994 and 2011. The fixed effects included herd-year, sex (male or female), birth type (single or twin), and age of dam (five classes, 2-6 years old). The three multivariate linear animal models including direct (autosomal and sex-linked) and maternal effects were used to analyze data. The 2nd model (including direct additive genetic effects of animal (autosomal and sex-linked) and permanent environmental effects), with the lowest AIC value was considered as the most appropriate model. Based on the most appropriate fitted model, direct autosomal and sex-linked heritabilities of BW, WW, ADG and KR were 0.12±0.04 and 0.03±0.03, 0.18±0.04 and 0.05±0.03, 0.37±0.04 and 0.04±0.03, and 0.52±0.05 and 0.07±0.04, respectively. The magnitude of sex-linked variances as a proportion of phenotypic variance obtained by the best model were 4%, 4.5%, 3% and 5.7% for BW, WW, ADG and KR, respectively. Also, the autosomal and sex-linked chromosomes, maternal permanent environmental and phenotypic correlations were ranged from -0.20 to 0.96, 0.11 to 0.84, -0.34 to 0.89 and -0.20 to 0.91, respectively. The results of this study indicated that the sex-linked chromosomes have an affect like maternal effects, and it could contribute to a more accurate estimation of the direct autosomal heritability. The purpose of this study was estimation of genetic parameter using random regression model (RRM) with various error variance in Iranian Kordi sheep. The data (consisting of 7875 weight records from birth to 360 days of age) were collected during the period 2000 to 2013 from the rearing and breeding station of Kordi sheep in Shirvan, Iran. The independent variables were Legendre polynomials (LP) of age at weighing and orders of fit from 2 to 5 were considered. Analyses were carried out fitting sets of random regression coefficients due to direct additive genetic, direct and maternal permanent environmental effects, with heterogeneous and homogeneous error variances. To compare the model were used different criteria such as LogL, AIC, BIC and LRT. The best fitting RRM among homogeneous error variance was the model with a LP of fourth order for fixed effect, fourth order for direct additive genetic and fifth order for direct and maternal permanent environmental effects (model 4455). Among the models with heterogeneous error variances different, model 7 (Heterogeneous error variances of 72 various classes), was selected as the best model. The variances increased along the trajectory from 3.75 to 12.81, 4.43 to 30.28 and 1.49 to 8.49; 0.25 to 27.94, 0.03 to 12.32 and 0.15 to 22.66 for direct additive genetic, direct and maternal permanent environmental effect by homogeneous and heterogeneous error variances, respectively. The direct heritability ranged from 0.15 to 0.41 and 0.11 to 0.56 by homogeneous and heterogeneous error variances, respectively. Genetic correlation between adjacent test days was more than between distant test days. The aim of this research was to compare different polynomial functions including Legendre polynomials (LP), Wilmink (WRR) and Ali-Schaeffer (ARR) functions, in random regression model (RRM) for estimation of genetic parameters for milk production traits of Iranian Holstein dairy cattle. For this purpose the performance records obtained from test-day (TD) regarding milk yield, fat and protein contents of the cows calving for the first time were used. The numbers of records for the above mentioned traits were 701212, 657004, and 560775, respectively. These records were collected from the years 2006 to 2010 by the National Breeding Center of Iran. The genetic parameters were estimated using Restricted Maximum Likelihood (REML) method by applying RRM. Residual variances were considered homogeneous over the lactation period. To compare the model, different criteria (-2Logl, AIC, BIC and RV) were used for considered traits. Based on the results obtained, for all traits, RRM with LP function (2,5) were chosen as the best model. Considering residual variance (RV), LP (2,2) was proved to be a model which has the lowest performance, while using -2Logl, AIC, BIC criteria, RRM with ARR function was the worst model. According to the results, it is recommended to use LP with low orders for the additive genetic effects and with more orders for the permanent environment effects in the RRM for Iranian Holstein cattle. Permanent environment variance was higher in early lactation than during lactation and additive genetic variance in the early lactation was lower than at the end of lactation. Heritability range of milk yield, fat and protein contents was estimated to be from 0.08 to 0.23, 0.05 to 0.20 and 0.08 to 0.14, respectively. Phenotypic variance of the considered traits during lactation was not constant and it was higher at the beginning and the end of lactation. This study was conducted to detect the serotypes Arkansas, Connecticut, Beaudette and Florida in broiler chicken farms in Fars province of Iran. The samples were obtained from 32 broiler chicken farms experiencing respiratory signs with high mortality and clinically diagnosed as avian infectious bronchitis. Total RNAs were extracted from the scrapped mucosa of tracheas and RT-PCR tests were performed using different types of primers including Del1 (F, R), (XCE-1, 2), (XCE-3 & BCE- 1), (XCE-3 & MCE-1), S1 OLIGO (F, R) and S1 OLIGO NEW (F, R). Infectious bronchitis virus was detected in 72% poultry farms. The serotypes 4/91 and Massachusetts were detected in 47.9 and 21.8% farms, respectively, but the serotypes in 7 farms remained unclear. The results showed that there is no evidence for the presence of serotypes Arkansas, Connecticut, Beaudette and Florida in broiler chicken farms in Fars province of Iran. [ABSTRACT FROM AUTHOR]

Abstract: This paper studies the energy balance between the input and the output per unit area for greenhouse cucumber production. For this purpose, the data on 43 cucumber production greenhouses in the Tehran province, Iran, were collected and analyzed. The results indicated that a total energy input of 148836.76MJha−1 was consumed for cucumber production. Diesel fuel (with 41.94%) and chemical fertilizers (with 19.69%) were amongst the highest energy inputs for cucumber production. The energy productivity was estimated as 0.80kgMJ−1. The ratio of energy output to energy input was approximately 0.64. Results indicate 10.93% and 89.07% of total energy input was in renewable and non-renewable forms, respectively. The regression results revealed that the contribution of energy inputs on crop yield (except for fertilizers and seeds energies) was significant. The human labour energy had the highest impact (0.35) among the other inputs in greenhouse cucumber production. Econometric analysis indicated that the total cost of production for one hectare of cucumber production was around 33425.70$. Accordingly, the benefit–cost ratio was estimated as 2.58. The article focuses on the post-revolutionary exiles people of political leaders in Iran, which culminated in the establishment of the Islamic Republic in1978-1979. Approximately two million people have been exiled wherein a sizeable number of them were so-called children of the revolution. The Council of National Resistance, Fedayeen-e-Khalq and Tudeh Party and monarchists were among major groups included at the exile. The revolutionary transition and post-revolutionary diaspora of Iran were discussed.

The Kopet Dagh is a thick-skinned, tectonically inverted mountain range situated along the Iran-Turkmenistan border. It comprises clastic and carbonate sedimentary rocks ranging in age from Jurassic to Neogene. We determine the provenance of Middle Jurassic deep marine turbiditic sandstones, Upper Jurassic-Lower Cretaceous deltaic-shelf sandstones and Paleocene continental conglomerate by describing sandstone modal framework, heavy minerals, detrital zircon U-Pb ages and in-situ Hf isotopic analyses. This work aims to determine the assemblages of source rocks of the investigated sediments and to reconstruct the tectonic setting of clastic sediment deposition. Modal sandstone framework compositions indicate that a magmatic arc and recycled deformed sediments were the main sources of Middle Jurassic-Lower Cretaceous sandstones and Paleogene conglomerate, respectively. Heavy mineral assemblages including ultra-stable and meta-stable minerals indicate magmatic rocks and recycled sediments as a main source of clastic rocks. Detrital zircon U-Pb ages (2478 measured ages) cluster in six main age groups: The oldest zircon grains in the Kopet Dagh belong to two populations peaking at 1833 Ma (Paleoproterozoic) and 643 Ma (Neoproterozoic). These grains suggest a continental crust provenance within the Central Iranian and Eurasian blocks. Detrital zircon grains of Silurian (437 Ma), Lower Carboniferous (340 Ma), Triassic (233 Ma) and Lower Cretaceous (132 Ma) are the major age population in the Kopet Dagh. Hafnium isotopic compositions in Zircons (175 measurements) indicate their origin: Silurian and Triassic zircons show a magmatic provenance, preferentially related to intracontinental rifting. Hafnium isotopic compositions of Lower Carboniferous and Lower Cretaceous zircons indicate a continental magmatic arc source. Our results indicate that Middle Jurassic-Lower Cretaceous detritus have been supplied from Silurian and Triassic rifting-related magmatic rocks and Lower Carboniferous / Lower Cretaceous continental magmatic arcs. The variety of source rocks is in concordance with rifting and an extensional regime in Kopet Dagh during Middle Jurassic time. Paleocene clastic sediments result from erosion of deformed older sedimentary rocks within the basin. Background: Burn wounds rank among the most serious healthcare issues. Many studies reported the effectiveness of natural products in the wound-healing process. The present study compared the effects of a standardized herbal formulation derived from Boswellia carteri (B. carteri) and silver sulfadiazine (SSD) cream 1% on the healing of burn wounds. Methods: This randomized double-blind clinical trial was conducted at Shiraz Burn Hospital (Shiraz, Iran) between July 2012 to August 2013. A sterilized formulation comprising B. carteri 40% was prepared. 54 second-degree burn patients of both sexes with age ranges of 20 to 60 were invited to participate in this double-blind, randomized clinical trial. They were randomly divided into two groups and given either the Boswellia formulation or SSD cream. The healing index was determined based on the wound area assessment using the planimetry technique. The Kaplan-Meier survival analysis was used to assess the primary outcome, which was the amount of time until complete healing. Results: The trial was completed by 17 patients from the SSD group and 15 patients from the Boswellia group. During the study period, both groups showed a progressive healing trend. The mean (95% CI) healing time in the SSD group was 10.94 (9.03-12.85) days and 10.73 (9.23-12.23) days in the Boswellia group (P=0.71), indicating no significant difference. On the 17th day, the healing index of all patients in the Boswellia group reached 1. Conclusion: Boswellia topical formulation had a burn wound healing effect comparable to that of the standard SSD 1% treatment. Abstract: The objective of this study was to determine energy use efficiency in greenhouse cucumber production using a non-parametric production function. The method used to quantify energy use efficiency or, more specifically, excess input energy used, is data envelopment analysis (DEA). DEA creates a ‘‘best practice’’ production frontier based on the growers that produce their level of cucumber yield with the least amount of input energy. These greenhouses then serve as benchmarks against which the energy use inefficiency of all other growers can be measured. The data used in this study was obtained through a face-to-face questionnaire method in the surveyed region – Yazd province of Iran. The total energy input and energy output were calculated to be 1168023 and 124663MJha−1, respectively. According to this information, the energy productivity was 0.14kgMJ−1, and net energy value was estimated to be –1043360MJha−1 indicating energy has been lost in greenhouse cucumber production. Basic DEA models (CCR and BCC) were used to measure the technical efficiency of the growers based on seven energy inputs human labor, diesel fuel, FYM, fertilizers, chemicals, water for irrigation and electricity, and a single output included yield of cucumber (kgha−1). The CCR and BCC models indicated 10 and 25growers were efficient, respectively. The average values of overall technical efficiency, pure technical efficiency and scale efficiency of growers were found to be 0.8235, 0.9273 and 0.8880, respectively. The results also revealed that about 21% of the total input resources could be saved if the growers follow the input package recommended by the DEA. The maximum contribution to the total energy savings is 53% from diesel fuel, followed by electricity (27%). [Copyright &y& Elsevier]

Groundwater aquifers are considered the second most abundant water supply for drinking water all over the world. In Iran, ground waters are commonly employed for drinking water, irrigation, and industrial purposes. Heavy metals (HMs) pose human concerns about the groundwater contamination; these pollutants are recognized to be capable of bio-accumulation, long persistence in the natural environment, and toxic effects. In the present research, the content of HMs: Chromium (Cr), Cadmium (Cd), and Lead (Pb) were detected in 89 water samples collected in 2018 by underground water supplies (active wells) of Saravan city. Hazard Quotient (HQ) and Monte Carlo Simulation approach with 10,000 repetitions were applied to discover the human non-carcinogenic impacts of HMs in four groups of ages (adults, teenagers, children, and infants) of consumers. The concentrations of Cr, Pb, and Cd were in the range of 0.49–20, 0.1 to 58.34, and 0.11–12.8 μg/L, respectively. The mean HQ calculated due to exposure to Pb (0.0018–0.0023), Cr (0.0112–0.0186), and Cd (0.0370–0.0615) were lower than one. The findings of sensitivity analysis revealed that HMs concentration had the most contribution effect on human non-carcinogenic risk analysis in four different exposed populations. This study could assist researchers to perform more comprehensive studies with more samples. In northeast Iran, the suture zone of the Paleotethys is situated between the East Alborz Mountains to its south and the Kopet Dagh Mountains to its north. Both mountain belts represent large-scale oroclines, potentially resulting from the indentation of the quasi-rigid South Caspian Basin. The oroclinal shape of the Alborz Mountains has been associated with vertical-axis rotation based on paleomagnetic data. However, no evidence for rotation has been found in the northeastern Kopet Dagh. A plausible scenario is that most of the rotation in the Kopet Dagh has been absorbed along its western part. Here, we present results of vertical-axis rotation and anisotropy of the magnetic susceptibility (AMS) for sites located in the West Kopet Dagh. AMS data range from sedimentary to tectonic fabrics depending on the position with respect to local thrusts. Magnetic vectors do not show significant vertical-axis rotation, implying that no significant rotation took place since Cretaceous times in the western Kopet Dagh. Background: Bovine immunodeficiency virus (BIV) is a member of the Retroviridae family causing a progressive lifelong infection in cattle and buffaloes., Objective: Despite the worldwide distribution of the virus, the studies concerning the prevalence of BIV in buffalo populations have not been conducted in Iran as yet., Methods: The BIV proviral DNA was surveyed in 120 whole blood samples of water buffaloes in southwestern Iran. Nested PCR was employed to amplify a 298-bp fragment of the pol gene. The BIV Pol sequence was detected in 9.1% of the samples. Among PCR-positive samples, two amplified fragments were confirmed by nucleotide sequencing., Results and Conclusions: The studied sequences were completely identical to each other and had more than 98%-99% nucleotide homology to R-29 and HXB3 sequences previously deposited in GenBank. Some point mutations that caused coding substitutions were observed in the studied isolates, compared to other strains. A phylogenetic tree was generated based on the BIV Pol nucleotide sequences reported from other countries. All the BIV strains originated from a unique main cluster and then separated from each other over time. This is the first report on the molecular detection of BIV infections in water buffalo populations in Iran. The wide distribution of BIV in different countries including Iran indicates the importance of the infection as it relates to animal health. Although buffaloes show greater resistance to diseases, they should be considered a health risk to cattle. Furthermore, BIV has negative effects on buffalo milk production and can predispose them to secondary infections. Hence, the findings of this study can advance our understanding of the occurrence of BIV infection in Iran, which can play an important role in the distribution of the disease worldwide., (© 2022 The Authors. Purpose: Lead Poisoning is a major health problem in Iran. We aimed to compare efficacy of a standard regimen (Succimer) with that of a low-priced combination of D-penicillamine and Garlic in outpatients with lead poisoning., Methods: In this retrospective cross-sectional study, year-long clinical files of outpatients with lead poisoning in two referral toxicology clinics in Mashhad, Iran were reviewed. A total of 79 patients (all men), received either Succimer or a combination of D-penicillamen plus garlic (DPN + Gar), for 19 and 30 days, respectively. Clinical and laboratory data, including blood lead level (BLL), were analyzed and treatment expanses were compared between the two regimens., Results: Of 79 male patients, 42 were treated by DPN + Gar and 37 received Succimer. Mean BLL of DPN + Gar group before treatment (965.73 ± 62.54 µg/L) was higher than that of the Succimer group (827.59 ± 24.41) (p < 0.001). After treatment, BLL in both groups significantly reduced to 365.52 ± 27.61 µg/L and 337.44 ± 26.34 µg/L, respectively (p < 0.001). The price of a 19-day treatment with Succimer was approximately 28.6 times higher than a one-month course of treatment with garlic plus DPN. None of the treatments caused serious side effects in the patients., Conclusion: Combination therapy with DPN + Gar is as effective as Succimer in Pb poisoning, while treatment with Succimer is significantly more expensive., (© 2021. Background: Self-immolation, as a method of suicide, is one of the most violent and extreme ways which is usually attempted by the ignition of inflammable materials, with more than 70% fatality rate. In the literature, Iran has been reported to have a high rate of self-immolation; therefore, this study aimed to evaluate the prevalence and epidemiological features of self-immolated patients., Methods: In this retrospective cross-sectional multicenter study, data from burn patients from 2007 till 2017 due to self-immolation and suicide were enrolled in our study., Results: Based on our data, 657 out of 3530 burn patients (18.6%) with a mean age of 31.15 (SD = 0.452) were documented as suicidal attempts; the majority were female (63.2%) and married (66.3%). Most of the patients were from rural areas (58.3%) with an education level of under diploma (63.2%). Of the patients in our study, 22 (8.7%) had comorbid systemic diseases and 115 (50.5%) had psychiatric disorders., Conclusion: Due to the high prevalence of suicide by self-immolation among the Iranian population, further studies to evaluate the risk factors and clarify the high-risk group for more targeted approaches are recommended.

Background: Drug use can lead to several psychological, medical and social complications. The current study aimed to measure and decomposes socioeconomic-related inequalities in drug use among adults in Iran., Methods: This was a cross-sectional study The PERSIAN Cohort is the largest and most important cohort among 18 distinct areas of Iran. This study was conducted on 130,570 adults 35 years and older. A structured questionnaire was applied to collect data. The concentration index (C) was used to quantify and decompose socioeconomic inequalities in drug use., Results: The prevalence experience of drug use was 11.9%. The estimated C for drug use was - 0.021. The corresponding value of the C for women and men were - 0.171 and - 0.134, respectively. The negative values of the C suggest that drug use is more concentrated among the population with low socioeconomic status in Iran (p < 0.001). For women, socioeconomic status (SES) (26.37%), province residence (- 22.38%) and age (9.76%) had the most significant contribution to socioeconomic inequality in drug use, respectively. For men, SES (80.04%), smoking (32.04%) and alcohol consumption (- 12.37%) were the main contributors to socioeconomic inequality in drug use., Conclusions: Our study indicated that drug use prevention programs in Iran should focus on socioeconomically disadvantaged population. Our finding could be useful for health policy maker to design and implement effective preventative programs to protect Iranian population against the drug use.

Background: Standardized data collection supports disease information management and leads to better quality of care. The Islamic Republic of Iran lacks a standard data set for data collection in hospitals., Aims: The aim of this study was to design a minimum data set for hospital information systems in the Islamic Republic of Iran., Methods: This study was conducted in 2015. Data sets of other countries, hospital records, hospital information systems and electronic health record systems in the Islamic Republic of Iran were reviewed for data elements for the minimum data set. Data elements were collected using a data extraction form and were categorized into similar classes, which were divided into administrative and clinical sections. The list of data elements was reviewed by experts in technical offices of the Iranian Ministry of Health and Medical Education, and a minimum data set was drawn up., Results: There were nine and 18 data classes in the administrative and clinical sections with a total of 166 and 684 data elements respectively. After review by the expert panel, 159 administrative and 621 clinical data elements were retained as the minimum data set for the Iranian hospital information system., Conclusion: Our dataset can be used by the Iranian health ministry, hospital information system companies and health surveillance centres for more efficient management of health data., (Copyright © World Health Organization (WHO) 2020. Excessive intake of fluoride can cause adverse health effects. Consumption of tea as a popular drink could be a potential source of fluoride exposure to humans. This research aimed to evaluate the fluoride concentration in tea among the Iranian people using the available data in the literature and to assess the health risk related to the consumption of tea in men, women, and children. The health risk assessment was conducted using the chronic daily intake and hazard quotient according to the approach suggested by the Environmental Protection Agency. The fluoride content in published studies varied noticeably, ranging from 0.13 to 3.27 mg/L. The results revealed that the hazard quotient (HQ) in age groups of women (21-72 years) and children (0-11 years) was within the safe zone (HQ < 1) which showed that there was no potential of non-carcinogenic risk associated with drinking tea in these groups. However, in one case of the men (21-72 years), the HQ > 1 which shows a probable risk of fluorosis. The order of non-carcinogenic health risks in the studied groups was in the order of men > women > children. The results of this can be useful for organizations with the responsibility of human health promotion.

Background: Peste des petits ruminants (PPR) is a severe infectious disease in both domestic and wild small ruminants. Due to its heavy economic burden and hence social and health impacts on human populations, Food and Agriculture Organization of the United Nations (FAO) and The World Organization for Animal Health (OIE) have targeted PPR for eradication by 2030. In order to plan and implement a successful eradication program, factual status assessments prior to devising disease control strategies is a vital criterion. The aim of this study was to isolate and characterize PPR virus from a rising wave of outbreaks in southern Iran., Results: Twenty-one clinical samples, including blood as well as oral, nasal and ocular swabs were collected from ten sick animals in 4 various herds and were examined with ELISA and RT-PCR for the presence of PPR virus antigen and genome, respectively. The virus was successfully isolated in primary lamb kidney cell culture and identified by RT-PCR. Phylogenetic analysis of the sequenced N genes revealed that, while the earliest reports of Iran's outbreaks were grouped into clusters with Saudi Arabia, Turkey and Africa, in this study reported sequences were grouped with samples from Pakistan, Tajikistan and China in particular. This observation suggests a shift in PPRV flow from the western borders of the country to the eastern neighboring countries., Conclusions: Lineage IV of PPR virus is presently circulating in Iran, with certain levels of genetic diversity. Present study along with previous reports demonstrates the dispersal patterns and movements of PPR virus, which highlights the reversal pattern of virus flow in recent years. Such information is necessary to understand PPRV molecular epidemiology and to develop more proper control strategies to eradicate the disease in the planned time.

Prevalence of fluorosis is a worldwide public health issue, especially in the West Azerbaijan province of Iran. The aim of this study was to investigate fluoride concentration in drinking water resources within Maku city, in both the warm and cold seasons, to perform a health risk assessment. Fluoride were measured using UV-visible spectrophotometry. The spatial distribution was calculated by the software ArcGIS and Hazard Quotients (HQs) were calculated according to the US EPA method. The fluoride concentrations ranged between 0.29 to 6.68 and 0.1 to 11.4 mg/L in the cold and warm seasons, respectively. Based on this report, 30.64 and 48.15% of the samples revealed a fluoride level higher than the permissible level in the cold and warm seasons, respectively. Moreover, results showed that the HQ value in the warm season for different age groups was higher than the HQ value in the cold season. In both seasons, the non-carcinogenic risks of fluoride for the four exposed populations varied according to the order: children > teenagers > adults > infants. The HQ values for three age groups (children, teenager and adults) for both seasons were higher than 1 with a high risk of fluorosis. The results of this study, support the requests that government authorities better manage water supplies to improve health quality.

Skeletal fluorosis resulting from high fluoride level in drinking water is a major public health problem. The present study evaluated the association between exposures to drinking water fluoride and skeletal fluorosis in 5 villages of Poldasht County, Iran. All the data and information on the prevalence of bone diseases were obtained from the Health Record Department, Poldasht Health Centre. To obtain the odds ratio of bone disease problem in different risk factors, when considering the cluster effect of rural area, logistic regression in a multilevel model was used. Results showed that skeletal fluorosis of people who live in areas with high fluoride concentration is 18.1% higher than that of individuals who live in areas with low fluoride concentration. Skeletal fluorosis (54.5%) was observed in the age group of 71 years and above, and was more commonly found in females than males. According to Unadjusted, individuals who consume ≤3 unit milk and dairy products per week have almost the same level of bone diseases as compared to those that consume more than 3 units. This study indicated that, skeletal fluorosis is a general health problem in these rural areas because the results revealed that high percentage of the studied population had symptoms of skeletal fluorosis.

To assess quality of health services in Zanjan health centres based on clients' expectations and perceptions. The study was conducted by using service quality (SERVQUAL) scale on a sample of 300 females, clients of health care centres in the district of Zanjan, selected by cluster sampling. The results indicated that there were negative quality gaps at five SERVQUAL dimensions. The most and least negative quality gap mean scores were in reliability dimension (-2.1) and tangible (-1.13) respectively. There was statistically significant difference between clients' perceptions and expectations mean scores at all of the five service quality dimensions (P<0.001). The negative quality gap level in health service dimensions can be used as a guideline for redistribution of resources and managerial attempts to reduce quality gaps and improvement of health care quality.
