Early pregnancy loss, defined as pregnancy loss prior to 20 weeks’ gestation, occurs in about one in five pregnancies. Although there has been research on women’s experiences of healthcare and social support following miscarriage, research into workplace support is relatively limited. In this study, we explored women’s experiences of returning to work following early pregnancy loss, drawing on data from semi-structured interviews with 13 Australian women. Thematic analysis generated three main themes: returning to work too soon, decisions about disclosure and the importance of a flexible workplace. Findings revealed that return to work can be a difficult process to navigate, with many reporting feeling pressured to return to work. While disclosure of their miscarriage enabled women to feel supported by their colleagues and connect to those with a similar experience, they often found disclosure challenging. Flexibility with leave, return-to-work arrangements and role adjustments were highly valued and helped women return to work. Findings revealed that workplace context and culture were important in the extent to which women felt supported by their workplace. The proliferation of assisted dying legislative reforms globally is a significant change in the social and medico‐legal landscape of end‐of‐life care. Understanding the impacts of these legislative reforms on family members who care for a dying person is vital, yet under‐theorised in research. In this article, drawing on semi‐structured interviews with 42 carers for a person who has sought assisted dying in Australia, and extending ideas of ontological choreography we explore the new and complex choreographies enacted by carers in their endeavour to arrange a ‘good death’ for the dying person. We find that desires to fulfil the dying person’s wishes are often accompanied by normative pressures, affective tensions and complexities in bereavement. Enacting assisted dying requires carers to perform a repertoire of highly‐staged practices. Yet, institutional obstacles and normative cultural scripts of dying can constrain carer assisted dying practices. Understanding the nuances of carers’ experiences and how they navigate this new end‐of‐life landscape, we argue, provides critical insights about how assisted dying legislation is producing new cultural touchpoints for caring at the end of life. As the culture of silence that once surrounded cancer has gradually given way to greater public awareness, normative visions of what cancer survivorship should entail have proliferated. These visions emphasise positivity and perseverance in pursuit of cure. While these visions provide comfort to many, for people with metastatic cancer, the emphasis on cure can undermine their sense of belonging to the broader collective of people living with cancer. Drawing on semi‐structured interviews with 38 Australian women living with metastatic breast cancer, we explore how incurable cancer inflects understandings of self and transforms interpersonal relationships. Extending ideas around biosociality and belonging, we explore the tenuousness of social bonds, revealing how (in)visibility, (in)authenticity and (in)validation circulate within the daily lives of women with metastatic breast cancer. We conceptualise accounts according to four social bonds: (1) threatened bonds where a relationship is strained by misunderstanding, (2) severed bonds where a relationship is ruptured due to misunderstanding, (3) attuned bonds whereby a relationship is based on shared identification and (4) flexible social bonds when a relationship is based on mutual understanding. This discussion takes up the politics of gestational labour and uterine productivity in connection with genetic self-reproduction and the family in the oeuvre of Paul B. Preciado. In his autotheoretical treatise Testo Junkie, Preciado dispensed with the standard Marxist-feminist term 'sexual division of labour', positing instead a 'technogestational division of labour' to describe the mechanism by which capitalism segments people's bodies and constructs the capacity to make babies. Taking up that coinage, with enthusiasm for the political horizon it illuminates, I nevertheless read Preciado against Preciado. I identify moments (including in An Apartment on Uranus) of oscillation away from the earlier work's posit of always already non-sovereign 'politically assisted procreation'; moments which break faith with the 'copyleft' spirit of collective gestational anti-authorship insofar as they call for trans and queer individuals' equal access to 'genetic speech' and for our reappropriation of parental property rights qua 'confiscated patrimony'. Chronic health conditions represent a key challenge for contemporary public healthcare. Current policy promotes self-management support to reduce demands on health services and improve patients' health and wellbeing. Though there is emerging recognition that self-management is achieved in collaboration between health professionals and patients, how chronicity is managed in interaction remains relatively underexplored in research. In this paper we report on research examining how people are supported to self-manage their conditions through their healthcare encounters. We draw on observational data from consultations between people with multiple chronic health conditions and their healthcare professionals, and semi-structured interviews with both patients and professionals about these consultations. We illuminate points of disconnect between patients and health professionals and demonstrate how these disconnects unfold in self-management support interactions. We argue that self-management is temporally and socially situated, incorporating past, present and (anticipated) future experiences. However, there is a disjuncture between the temporal logics of self-management enacted by health professionals and the subjective temporalities of people's lived experience of chronicity. Health professionals focus on patients progressing toward optimistic futures but give less attention to the complexities of patient life histories that render self-management more difficult. For self-management support to be effective, we argue that health professionals need to consider the complexities of people's life histories and how these shape imagined futures. Policy guidelines, we argue, should attend to how relations between patients and professionals shape self-management support, and the historical and social factors that shape experiences of living with chronic conditions. Loneliness is one of the most pressing and rapidly growing contemporary social challenges around the world. Yet we still lack a good understanding of how loneliness is constituted and experienced by those most affected. We conducted semi-structured interviews with 40 people with chronic illness who were experiencing loneliness to explore what loneliness means to them and how it impacts in their daily lives. Drawing on ideas around liquidity and performativity, we identified the relational, temporal and social layers of loneliness. Our analysis revealed the interconnectedness of chronic illness and loneliness in participants' daily lives, as well as how chronic illness shifts temporal orientation, and transforms interpersonal relationships and relationship with self, contributing to the experiences of loneliness. Though participants described the many social conditions that restricted their opportunities for social participation, giving them a sense of being left behind and spectating the social life of others, a rhetoric of loneliness as a problem and responsibility of the individual was still prominent. A narrative of the need to perform social connection emerged in the absence of meaningful social bonds with others. We argue that normative ideals of wellness and positivity circulating in chronic illness communities and society more broadly are implicated in the experience of loneliness for people with chronic conditions. What does migrancy mean for personhood, and how does this flow through caring relations? Drawing on life history interviews and photo elicitation with 43 people who identify as migrants and live with cancer, here we argue for the significance of recognising complex personhood as it inflects illness and care. Drawing on social science theory around temporalities, moralities and belonging, we assemble a series of cross-cutting themes at the intersection of personhood and care; relations that transcend cultural origins yet are vividly illustrated in relation to migrant pasts. In seeking a multidimensional view of personhood, we attend to the intersecting layers of complexity that make up care in this context vis-a-vis an emphasis on forms of difference, vulnerability and otherness. In this way, we develop an approach to personhood and care that broadens the lens on migrancy and cancer, but also, one that speaks to the importance of recognition of complexity and how it shapes care more generally. Young people's experiences of living with a long-term health condition have been largely investigated from the perspective of developing autonomy and optimal self-management of treatment regimens. Little existing research explores how young people adjust to the experience of chronic illness within everyday social contexts. Drawing on sociocultural theories of healthism, in this article, we examine the everyday strategies students employed to manage their health condition at university. Data were drawn from a qualitative study with 16 undergraduate students in Australia. Findings from interviews highlight how participants took up discourses of the (hard-working, diligent) Self to discursively position themselves as 'health conscious' and 'in control'. This positioning was maintained through separating the controlled Self from the (uncontrollable) body. The unpredictability of the body posed a threat to young people's abilities to maintain control and denied them opportunities to exercise personal agency. Yet, participants also described a number of subversive strategies in order to take back control and resist the experience of ill health. These potential agentic practices often held unintended consequences, including loss of optimal medical control or (self) exclusion from university life – offering new insights into the differing ways young people concomitantly take-up, rework and resist the pursuit of healthism to 'successfully' manage their health conditions. The Intercollegiate Membership of the Royal College of Surgeons (MRCS) examination is a mandatory requirement for higher specialty surgical training in the UK. However, there is a significant economic impact on trainees which raises the question of whether the costs of this exam hinder surgical career progression. This study explores the burden of these exams on trainees. A 37-point questionnaire was distributed to all trainees who were preparing for or have sat MRCS examinations. Univariate analyses included the cost of the preparatory resources, extra hours worked to pay for these and the examinations, and the number of annual leave (AL) days taken to prepare. Pearson correlation coefficients were used to identify possible correlation between monetary expenditure and success rate. On average, trainees (n ​= ​145) spent £332.54, worked 31.2 ​h in addition to their rostered hours, and used 5.8 AL days to prepare for MRCS Part A. For MRCS Part B/ENT, trainees spent on average £682.92, worked 41.7 extra hours, and used 5 AL days. Overall, the average trainee spent 5–9% of their salary and one-fifth of their AL allowance to prepare for the exams. There was a positive correlation between number of attempts and monetary expenditure on Part A preparation (r(109)=0.536, p ​< ​0.001). There is a considerable financial and social toll of the MRCS examination on trainees. Reducing this is crucial to tackle workforce challenges that include trainee retention and burnout. AbstractLegislation in the United States has increasingly led to limitations on what and how educators are able to teach in the classroom. These measures, often deemed divisive concept laws, can lead to a host of challenges for music educators, impacting student/teacher relationships, repertoire selection, pedagogical practices, and student and teacher safety and mental health, among other concerns. Furthermore, constriction of content can frame the music educator as a deliverer of curriculum wherein the educator may feel stripped of their professional agency. The purpose of this study was to explore how, if at all, music educators from four states considering or actively enacting divisive concept laws achieved agency in their unique contexts. Data included semi-structured interviews with eight music educators. Findings suggest that stakeholders responded to policies in varying ways, the language of policies played a significant role in how policies were understood and enacted, and participants found divisive concept laws as particularly challenging in the midst of broader deprofessionalization of teaching. Background: A multitude of information sources are available to publics when novel infectious diseases first emerge. In this paper, we adopt a qualitative approach to investigate how Australians learnt about the novel coronavirus and COVID-19 and what sources of information they had found most useful and valuable during the early months of the pandemic.Methods: In-depth semi-structured telephone interviews were conducted with a diverse group of 40 Australian adults in mid-2020 about their experiences of the COVID-19 crisis. Participants were recruited through Facebook advertising. Detailed case studies were created for each participant, providing the basis of a thematic analysis which focused on the participants' responses to the questions about COVID-19-related information sources.Results: Diverse sources of COVID-19-related information, including traditional media, online media and in-person interactions, were actively accessed, appraised and engaged with by participants. There was a high level of interest in COVID-19 information as people grappled with uncertainty, anxiety and feeling overwhelmed. Certain key events or experiences made people become aware that the outbreak was threatening Australia and potentially themselves. Most people demonstrated keen awareness that misinformation was rife in news outlets and social media sites and that they were taking steps to determine the accuracy of information. High trust was placed in health experts, scientists and government sources to provide reliable information. Also important to participants were informal discussions with friends and family members who were experts or working in relevant fields, as well as engaging in-person in interactions and hearing from friends and family who lived overseas about what COVID-19 conditions were like there.Conclusion: A constantly changing news environment raises challenges for effective communication of risk and containment advice. People can become confused, distressed and overwhelmed by the plethora of information sources and fast-changing news environment. On the other hand, seeking out information can provide reassurance and comfort in response to anxiety and uncertainty. Clarity and consistency in risk messaging is important, as is responding quickly to changes in information and misinformation. The article discusses a study that examined group facilitators' and participants' experiences of and engagement with goal setting in long-term condition self-management group programmes. Highlights include patient-provider interactions in group and one-to-one settings, development of a coding framework where text from three data sources were coded into categories and subcategories, and the link between meaningful goals, increased motivation, and taking on responsibility.

First-person accounts of voice hearing are scarce. This research aims to explore the role of spirituality in the sense-making process of hearing voices. Five semistructured interviews explored experiences of spirituality and hearing voices. Qualitative data was analyzed using interpretive phenomenological analysis (IPA). Three superordinate themes were identified: need for connection, values about self and identity, and making sense. The findings suggest a relationship between spirituality and voice hearing, and relate to the need for belongingness and self-identity. Acknowledging the sense making process and engaging in conversations about spirituality are implicated when providing clinical interventions for distressing voice hearing. Background: Before implementation of the Massachusetts Partnerships for Care (MA P4C) project, Massachusetts did not routinely provide HIV partner services to newly diagnosed individuals. Identification of individuals disengaged from HIV care and assistance for re-engagement relied on community- and clinic-based HIV medical case management services. Processes to identify individuals out of HIV care used either surveillance or clinic data, but did not combine both. Methods: The Massachusetts Department of Public Health collaborated with 6 community health centers to implement HIV partner services and out-of-care re-engagement services. Implementation of these services required development of both health department and community health center capacity. Capacity development strategies included the following: development and implementation of operational protocols, establishment of communication strategies and processes, training and education for health department and health center staff, and enhancement of disease surveillance systems. Results: Development of operational protocols supported implementation of public health services and collaboration with health centers. Health department-facilitated meetings, training, and technical assistance enhanced communication with health centers and promoted buy-in for collaboration. A strategy for combining clinic and surveillance data to identify individuals out of HIV care was implemented. Surveillance data system enhancements improved efficiency of out-of-care identification, assignment of cases for public health follow-up, and provided quality improvement tools. Conclusions: Collaboration between health departments and community health centers to identify and support patient engagement in HIV medical care is feasible and supports improved continuity of care. Premised on the idea that setting goals motivates action and allocation of resources toward a desired future state, goal-setting has become a key component of self-management support. The notions that underpin goal-setting situate self-management firmly within the control and responsibility of individuals. Yet, we argue that goals are not solely individual pursuits to be achieved if individuals have the right knowledge and enough motivation; rather, they are social products, influenced by individual and collective subjectivities, which are structured over time. Drawing primarily on Bourdieu's concept of habitus, along with capital and field, we examine how goals are constructed in self-management support encounters. Seventeen patient-professional dyads comprising 15 patients and 11 health professionals were interviewed on up to three occasions. In total 64 semi-structured interviews were conducted between 2015 and 2017 in Sydney, Australia. Goals were manifested through patient participants' expressions of goal agency (as self-owned wants and opportunities; responsibilities to self and others; or necessities); goal-oriented dispositions (determined; responsible; or powerless); temporal orientations (past, present or future); and access to resources to support self-management. These characteristics were grouped into three typologies: 'Goals as opportunities'; 'From goals to responsibilities'; and 'Necessities when living precariously'. The way goals were constructed was structured over time by present experiences superimposed on past experiences and access to resources. These findings help broaden understandings of the tensions between patient and professionals' goals for living well with chronic conditions and draw attention to the need for structural change to support people to live well with their chronic conditions. • Goals for self-management are complex social products. • Goal orientations are shaped by dispositions, temporalities and resources. • People are differentially positioned in the field of self-management support. • Structural factors must be addressed for all to have opportunities for meaningful goals. Extreme temperature and precipitation events occurring in Australia in recent decades have caused significant socio-economic and environmental impacts, and thus determining the factors contributing to these extremes is an active area of research. Many recently occurring record-breaking temperature and rainfall events have now been examined from an extreme event attribution (EEA) perspective. This paper describes a set of studies that have examined the causes of extreme climate events using various general circulation models (GCMs), presenting a comprehensive methodology for GCM-based attribution of extremes of temperature and precipitation observed on large spatial and temporal scales in Australia. First, we review how Coupled Model Intercomparison Project Phase 5 (CMIP5) models have been used to examine the changing odds of observed extremes. Second, we review how a large perturbed initial condition ensemble of a single climate model (CESM) has been used to quantitatively examine the changing characteristics of Australian heat extremes. For each approach, methodological details and applications are provided and limitations highlighted. The conclusions of this methodological review discuss the limitations and uncertainties associated with this approach and identify key unexplored applications of GCM-based attribution of extremes. The likelihood and severity of high‐impact future temperature extremes can be reduced through climate change mitigation efforts. However, meeting the Paris Agreement warming limits requires notably stronger greenhouse gas emissions reduction efforts by major emitters than existing pledges. We examine the impact of Paris‐era decision‐making by the world's three largest greenhouse gas emitters (EU, USA, and China) on projected future extreme temperature events. Country‐level contributions to the occurrence of future temperature extremes are calculated based on current emissions policies and sequential mitigation efforts, using a new metric called the Contribution to Excess Risk Ratio. We demonstrate the Contribution concept by applying it to extreme monthly temperature projections. In many regions, future extremes depend on the current and future carbon dioxide emissions reductions adopted by major emitters. By implementing stronger Paris‐era climate pledges, major emitters can reduce the frequency of future extremes and their own calculated contributions to these temperature extremes. Plain English Summary: Temperature extremes can damage aspects of human society, infrastructure, and our ecosystems. The frequency, severity, and duration of high temperatures are increasing in some regions and are projected to continue increasing with further global temperature increases as greenhouse gas emissions rise. A person-centered approach to goal-setting, involving collaboration between patients and health professionals, is advocated in policy to support self-management. However, this is difficult to achieve in practice, reducing the potential effectiveness of self-management support. Drawing on observations of consultations between patients and health professionals, we examined how goal-setting is shaped in patient–provider interactions. Analysis revealed three distinct interactional styles. In controlled interactions, health professionals determine patients' goals based on biomedical reference points and present these goals as something patients should do. In constrained interactions, patients are invited to present goals, yet health professionals' language and questions orientate goals toward biomedical issues. In flexible interactions, patients and professionals both contribute to goal-setting, as health professionals use less directive language, create openings, and allow patients to decide on their goals. Mean and extreme northwest Australian (NWA) summertime rainfall has increased significantly since 1950. While previous studies have explored a range of possible factors impacting NWA rainfall, the causes of this increase and possible future changes remain uncertain. This study explores the increasing NWA summertime rainfall trends in Coupled Model Intercomparison Project phase 5 (CMIP5) models. By using a suite of models that contributed realizations of the historical period with various forcings, we explore the impact to this region of greenhouse gases and aerosol emissions since 1950 on mean rainfall and three extreme rainfall indices. The observed NWA rainfall trend is better captured in models when all forcings are included compared to simulations with only greenhouse gas forcing or with only natural forcing, although the models have a large spread. We hypothesise that anthropogenic aerosols played a major role in the observed rainfall trends, and the associated increase in monsoonal flow, and hence historicalNat and historicalGHG simulations tend not to capture observed rainfall trend. Throughout the 21st century, CMIP5 models simulate a stronger increase in mean summer precipitation and extreme indices of NWA rainfall in representative concentration pathway (RCP) 8.5 simulations than in RCP2.6. The NWA region shows intensified extreme events with fewer heavy precipitation days, but the reliability of these projections in this region should be further tested with estimates of future anthropogenic aerosol changes. This article reports on a discourse analysis of the representation of healthcare in the print news media, and the way this representation shapes perspectives of healthcare. We analysed news items from six major Australian newspapers over a three-year time period. We show how various framing devices promote ideas about a crisis in the current public healthcare system, the existence of a precarious balance between the public and private health sectors, and the benefits of private healthcare. We employ Bourdieu's concepts of field and capital to demonstrate the processes through which these devices are employed to conceal the power relations operating in the healthcare sector, to obscure the identity of those who gain the most from the expansion of private sector medicine, and to indirectly increase health inequalities. In this paper, we examine the intersections between place and healthcare choice, drawing on Bourdieu's concepts of distinction and social space, and engaging with data from interviews with 78 Australians living in varied geographic locations. We find the status of an area is used to judge the quality of its healthcare services. Areas with high status are assumed to have better quality health services than areas of disadvantage. Where people live shapes the choices they make and their judgements about the status of a place. Moreover, having less choice is not necessarily problematic. Participants in regional and remote areas with less choice tend to report positive experiences with healthcare providers. Place can constrain people's ability to make good healthcare choices, yet participants have differing capacities to mobilise resources to overcome the constraints of place. [ABSTRACT FROM AUTHOR]

Reproductive justice and gestational surrogacy are often implicitly treated as antonyms. Yet the former represents a theoretic approach that enables the long and racialised history of surrogacy (far from a new or ‘exceptional’ practice) to be appreciated as part of a struggle for ‘radical kinship’ and gender-inclusive polymaternalism. Recasting surrogacy as a dynamic contradiction in itself, full of latent possibilities relevant to early Reproductive Justice militants’ family-abolitionist aims, this article invites scholars in human geography and cognate disciplines to re-think the boundaries of surrogacy politics. As ethnographies of formal gestational workplaces, accounts of gestational workers’ self-organised resistance, and readings of the attendant public media scandals show (taking examples from India, Thailand, and New Jersey), there is no good reason to place these new economies of ‘third-party reproductive assistance’ in a ‘realm apart’ from conversations about social reproduction more generally. Surrogacy, I argue, potentially names a practice of commoning at the same time as it names a new wave of accumulation in which clinicians are capitalising on the contemporary - biogenetic-propertarian, white-supremacist - logic of kinmaking in the Global North. Ongoing experiments in the redistribution of mothering labour (‘othermothering’ in the Black feminist tradition) suggest that ‘another surrogacy is possible’, animated by what Kathi Weeks and the 1970s intervention ‘Wages Against Housework’ conceive as anti-work politics. In making this argument, the article revives the concept ‘gestational labour’ as a means of keeping the process of ‘literal’ reproduction open to transformation. [ABSTRACT FROM AUTHOR]

Knowledge of the range of precipitation variability and extremes is restricted in regions such as Australia, where instrumental records are short and paleoclimatic records are limited in spatial and temporal extent and resolution. In such comparatively data-poor regions, there is limited context for understanding the statistical unusualness of recently observed extreme events, such as heavy rain and drought, and the influence of stochastic and anthropogenic forcings on their magnitude. This study attempts to further understandings of the range of forced and unforced variability using CMIP5 climate models. Focusing on extremes in the magnitude of monthly, seasonal, and annual precipitation, the distribution of instrumental-period observed precipitation in various Australian regions is compared to simulated precipitation in historical experiments as well as various long experiment (preindustrial control and Last Millennium) and anthropogenically forced simulations of the twenty-first century (RCP2.6 and RCP8.5). There is no systematic increase in the magnitude of simulated extremes corresponding to the length of model simulations, although many realizations reveal higher magnitude extremes compared to those observed, suggesting that the duration of the instrumental record may not capture the potential severity of stochastically driven extremes. A coherent increase in both wet and dry extremes is simulated throughout Australian regions in high greenhouse gas emissions scenarios, demonstrating a forced hydrological response. Objective Our aim was to systematically review the qualitative literature about the experiences of both facilitators and participants in a range of group-based programmes to support the self-management of long-term conditions. Methods We searched 7 databases using the terms ‘self-management’, ‘group’ and ‘qualitative’. Full text articles meeting the inclusion criteria were retrieved for review. A thematic synthesis approach was used to analyse the studies. Results 2126 articles were identified and 24 were included for review. Group participants valued being with similar others and perceived peer support benefits. Facilitators (HCP and lay) had limited group specific training, were uncertain of purpose and prioritised education and medical conformity over supportive group processes and the promotion of self-management agency and engagement. Overall, studies prioritised positive descriptions. Conclusion Group programmes’ medical self-management focus may reduce their ability to contribute to patient-valued outcomes. Further research is needed to explore this disconnect. Practice implications This review supports broadening the scope of group-based programmes to foreground shared learning, social support and development of agency. Record-breaking temperatures can detrimentally impact ecosystems, infrastructure, and human health. Previous studies show that climate change has influenced some observed extremes, which are expected to become more frequent under enhanced future warming. Understanding the magnitude, as a well as frequency, of such future extremes is critical for limiting detrimental impacts. We focus on temperature changes in Australian regions, including over a major coral reef-building area, and assess the potential magnitude of future extreme temperatures under Paris Agreement global warming targets (1.5°C and 2°C). Under these limits to global mean warming, we determine a set of projected high-magnitude unprecedented Australian temperature extremes. These include extremes unexpected based on observational temperatures, including current record-breaking events. For example, while the difference in global-average warming during the hottest Australian summer and the 2°C Paris target is 1.1°C, extremes of 2.4°C above the observed summer record are simulated. This example represents a more than doubling of the magnitude of extremes, compared with global mean change, and such temperatures are unexpected based on the observed record alone. Projected extremes do not necessarily scale linearly with mean global warming, and this effect demonstrates the significant potential benefits of limiting warming to 1.5°C, compared to 2°C or warmer. As surrogacy services expand globally, more and more nations are moving to ban the practice. Calls for its abolition couched in feminist terms returned to prominence in international public life in 2012. The resurgence follows a lapse since the heyday of the Feminist International Network of Resistance to Reproductive and Genetic Engineering (FINRRAGE) in the 1980s. In their moral opposition to prostitution (conflated with slavery) and cyborg human embodiment (which critics term "medical abuse"), the agendas of such feminisms and Christian conservatism overlap. This raises the question, what is the enemy against which childbearing is to be defended? This article analyzes the Euro-American and Australian anglophone feminist-abolitionist stance toward surrogacy, contextualizing it in relation to sex-worker-exclusionary antitrafficking, on the one hand, and transphobic imaginaries, on the other. These truncated abolitionisms, I argue, perform opposition to commodification rather than capitalism and encrypt profoundly antifeminist ideas under the guise of fighting patriarchy. The article focuses on defining new normal for extreme weather in climate change. A new normal has been applied to describe a diversity of weather and climate phenomena. Directing analysis of a new normal to extreme temperatures is motivated by several factors. Extreme weather and climate events occurring on subdaily to multiyear time scales and have significant socioeconomic costs and impacts on natural systems.

Metastatic breast cancer is a disease of changing status--once an imminent death sentence, now a chronic (albeit incurable) disease. Medical intervention advances mean women with metastatic breast cancer now have symptoms alleviated and, potentially, life extended. Living with this disease, however, requires more than a medical approach to symptoms. We were interested to know whether women manage, and if so, how, to "live well" with metastatic cancer. We conducted interviews with 18 women. Women differed in the approaches they used. Most common was the attempt to reestablish a sense of normality in their lives. However, a second group reevaluated and reprioritized their lives; and a third group was restricted in their capacity to live well because of symptoms. The findings provide the foundation for future research exploring normalization of experiences of metastatic cancer, and other chronic illnesses, where people are living with knowledge that they have contracted time. Australia experienced much above average rainfall in association with strong, extended La Niña conditions during 2010-2012. Was the heavy Australian rainfall influenced by La Niña conditions and/or anthropogenic greenhouse gases? We investigate the relative contributions of anthropogenic climate change and natural climatic variability to the likelihood of heavy Australian rainfall using three distinct model datasets. Area-average rainfall anomalies for model simulations with natural forcings only were compared to simulations with both anthropogenic and natural forcings using 16 models participating in the Coupled Model Intercomparison Project Phase 5. Using fraction of attributable risk to compare the likelihood of unusual rainfall between the parallel experiments, we find attribution statements are uncertain, with FAR values sensitive to the attribution parameters considered, including thresholds, regions and seasons. When heavy rainfall probabilities were next investigated in ensembles of two atmospheric general circulation models, run with and without anthropogenically-induced sea surface temperature changes, results were model-dependent. Overall, the attribution of seasonal-scale heavy Australia rainfall to a particular cause is likely more complicated than for temperature extremes. As estimates of the greenhouse gas attributable change in rainfall risk may depend on the model datasets considered, it is also useful to consider model outputs from several datasets and using various estimates of counterfactual surface conditions to establish robust attribution statements for extreme rainfall events. In contrast, comparing the likelihoods of heavy rainfall during simulated La Niña years with El Niño/neutral years reveals a substantial La Niña influence on Australian rainfall during 2010-2012 that is robust to changes in the attribution framework. In this article, we examine the choice to use a complementary and alternative medicine product (St John’s wort) for the management of mental health risk. We draw on data from a study in which we conducted in-depth, semi-structured interviews with 41 adults who self-reported depression, stress or anxiety, in Melbourne, Australia, in 2011. We identified three groups of users – regular St John’s wort users, whose use was continuous; irregular users, whose use was occasional; and non-users, who had stopped or were contemplating use. In each group, St John’s wort use centred around managing risk, taking control and self-management. Participants described a process of weighing up risks and benefits of different treatment options. They viewed St John’s wort as a less risky and/or safer option than antidepressants because they perceived it to be more natural, with fewer side effects. They saw their use of St John’s wort as a means of exercising personal control over mental health risks, for example, to alleviate or self-manage symptoms of depression. Their use of St John’s wort was also linked to perceptions of broader social risks including the stigma and shame of needing to use antidepressants. The findings deepen our understanding of notions of mental health risk by pointing to the importance of localised knowledge of risk in decision-making, and the ways in which perceptions of, and hence responses to, risk differ between groups. This article contributes to scholarship on the cultural politics of obesity by providing insights into how people considered ‘obese’ think news media reporting should be improved and their views on ideas such as reporting guidelines and promoting body diversity. A thematic analysis of interview data identified the following themes: ‘Challenging stereotypes’, ‘The limits of news’, ‘Individual responsibility’ and ‘Legitimating fat’. These themes capture the divergence in views and reflect differences in how people construct obesity and conceive the influences of media on audiences. Situated in the context of the contested science and news frames surrounding obesity, the analysis also engages with wider debates about the potentially unintended consequences of seeking to challenge stigma. Objectives To investigate how gamblers interact with, and respond to, downstream social marketing campaigns that focus on the risks and harms of problem gambling and/or encourage help seeking. Methods Qualitative study of 100 gamblers with a range of gambling behaviours (from non-problem to problem gambling). We used a Social Constructivist approach. Our constant comparative method of data interpretation focused on how participants' experiences and interactions with gambling influenced their opinions towards, and interactions with social marketing campaigns. Results Three key themes emerged from the narratives. (i) Participants felt that campaigns were heavily skewed towards encouraging individuals to take personal responsibility for their gambling behaviours or were targeted towards those with severe gambling problems. (ii) Participants described the difficulty for campaigns to achieve 'cut through' because of the overwhelming volume of positive messages about the benefits of gambling that were given by the gambling industry. (iii) Some participants described that dominant discourses about personal responsibility prevented them from seeking help and reinforced perceptions of stigma. Conclusions and implications Social marketing campaigns have an important role to play in the prevention of gambling risk behaviours and the promotion of help seeking. Social marketers should explore how to more effectively target campaigns to different audience segments, understand the role of environmental factors in undermining the uptake of social marketing strategies and anticipate the potential unforeseen consequences of social marketing strategies. Self-management is widely promoted in Western health care policies as a way to address the impact of increasing rates of chronic conditions on health care systems. Mostly informed by psychological theories, self-management frameworks and interventions tend to target individual behaviours as demarcated from the many aspects of social life shaping these behaviours. Using Bourdieu's theory of practice, we develop four propositions for a relational and socially situated (re)conceptualisation of self-management. First, self-management is a field with its own distinctive logics of practice; second, self-management goals are social practices, emerging through co-constituted patient–professional interactions; third, self-management is energised by legitimised capital; and fourth, what goals feel possible are shaped through embodied knowledge and lived experience (habitus), linked to capital. Collectively these propositions enable focus on both the meanings and resources patients and professionals bring to self-management, along with the dynamic and relational ways goals are produced through patient–professional interactions within the broader field of health care. Diurnal temperature range (DTR) is a useful index of climatic change in addition to mean temperature changes. Observational records indicate that DTR has decreased over the last 50 yr because of differential changes in minimum and maximum temperatures. However, modeled changes in DTR in previous climate model simulations of this period are smaller than those observed, primarily because of an overestimate of changes in maximum temperatures. This present study examines DTR trends using the latest generation of global climate models participating in phase 5 of the Coupled Model Intercomparison Project (CMIP5) and utilizes the novel CMIP5 detection and attribution experimental design of variously forced historical simulations (natural-only, greenhouse gas-only, and all anthropogenic and natural forcings). Comparison of observed and modeled changes in DTR over the period of 1951-2005 again reveals that global DTR trends are lower in model simulations than observed across the 27-member multimodel ensemble analyzed here. Modeled DTR trends are similar for both experiments incorporating all forcings and for the historical experiment with greenhouse gases only, while no DTR trend is discernible in the naturally forced historical experiment. The persistent underestimate of DTR changes in this latest multimodel evaluation appears to be related to ubiquitous model deficiencies in cloud cover and land surface processes that impact the accurate simulation of regional minimum or maximum temperatures changes observed during this period.
