AbstractThe Great East Japan Earthquake and subsequent tsunami that struck the Fukushima Daiichi Nuclear Power Station gave ground to a set of events that resulted in an unprecedented severe accident occurring simultaneously in multiple reactors of one nuclear power station and significant release of radioactive materials into the environment. Following these events, Tokyo Electric Power Company in cooperation with domestic and international partners has made continuous efforts to clarify in detail the progression of the accident utilizing both analytical tools and robotic inspections of the damaged units. Recently, multiple inspections utilizing submersible remotely operated vehicles were conducted between December 2022 and March 2023, including the first entry into the pedestal area below the damaged reactor pressure vessel of Unit 1. This paper discusses the findings of these inspections in light of the evolving understanding of the accident scenario, leveraging insights gained through experimental and analytical studies on ex-vessel severe accident phenomena. The outcomes of these investigations have yielded new knowledge and highlighted existing gaps in our understanding. A study conducted in Italy evaluated the feasibility of expanding the services offered by local pharmacies to include screening for dry eye disease (DED). The study collected data from over 8,000 individuals who participated in a nationwide screening initiative. The results showed that a significant number of patients with dry eye symptoms were dispensed with tear substitutes for symptomatic treatment. Female sex, Asian ethnicity, and the degree of meibomian gland loss were found to be associated with dry eye symptoms. The study suggests that pharmacy-based screening programs could improve access to rapid diagnostic testing and optimize therapy for DED. However, further research is needed to evaluate the efficacy and cost-effectiveness of these programs. [Extracted from the article]

In this interventional case series including 57 eyes with Fuchs endothelial corneal dystrophy undergoing triple DMEK, the presence of topographic hot spots was associated with higher hyperopic shift after surgery. Purpose: To compare the refractive results of combined Descemet membrane endothelial keratoplasty (DMEK) and cataract surgery in eyes with Fuchs endothelial corneal dystrophy (FECD) with and without topographic hot spots. Setting: "Villa Igea" Hospital, Forlì, Italy. Design: Interventional case series. Methods: 57 eyes of 52 patients with FECD undergoing combined DMEK, cataract surgery, and monofocal intraocular lens (IOL) implantation were included in this single-center study. Patients were classified based on the presence or absence of topographic hot spots on the preoperative axial power map. Prediction error (PE) was calculated as the postoperative manifest spherical equivalent (SE) refraction minus the SE predicted refraction. Results: 6 months postoperatively, mean PE was +0.79 ± 1.12 diopters (D). In eyes with hot spots, mean keratometry (K), K flat, and K steep significantly decreased after surgery (all P <.05), whereas no significant changes were observed in eyes without hot spots (all P >.05). Eyes with hot spots showed a significantly more hyperopic PE than eyes without hot spots (+1.13 ± 1.23 vs +0.40 ± 0.86 D; P =.013). Conclusions: Combined DMEK and cataract surgery can result in a hyperopic refractive surprise. The presence of topographic hot spots before surgery is associated with a higher hyperopic shift. A magic partially filled array MPFΩ(m,n;s,k) ${\text{MPF}}_{{\rm{\Omega }}}(m,n;s,k)$ on a subset Ω ${\rm{\Omega }}$ of an abelian group (Γ,+) $({\rm{\Gamma }},+)$ is a partially filled array of size m×n $m\times n$ with entries in Ω ${\rm{\Omega }}$ such that (i) every ω∈Ω $\omega \in {\rm{\Omega }}$ appears once in the array; (ii) each row contains s $s$ filled cells and each column contains k $k$ filled cells; (iii) there exist (not necessarily distinct) elements x,y∈Γ $x,y\in {\rm{\Gamma }}$ such that the sum of the elements in each row is x $x$ and the sum of the elements in each column is y $y$. Localized prostate cancer is a very heterogeneous disease, from both a clinical and a biological/biochemical point of view, which makes the task of producing stratifications of patients into risk classes remarkably challenging. In particular, it is important an early detection and discrimination of the indolent forms of the disease, from the aggressive ones, requiring post-surgery closer surveillance and timely treatment decisions. This work extends a recently developed supervised machine learning (ML) technique, called coherent voting networks (CVN) by incorporating a novel model-selection technique to counter the danger of model overfitting. For the challenging problem of discriminating between indolent and aggressive types of localized prostate cancer, accurate prognostic prediction of post-surgery progression-free survival with a granularity within a year is attained, improving accuracy with respect to the current state of the art. The development of novel ML techniques tailored to the problem of combining multi-omics and clinical prognostic biomarkers is a promising new line of attack for sharpening the capability to diversify and personalize cancer patient treatments. The proposed approach allows a finer post-surgery stratification of patients within the clinical high-risk category, with a potential impact on the surveillance regime and the timing of treatment decisions, complementing existing prognostic methods. An aspect correlated with climate change is certainly represented by the alternation of severe floods and relevant drought periods. Moreover, there is evidence that changes in climate and land cover are inducing changes in stream channel cross-sections, altering local channel capacity. A direct consequence of a significant change in the local channel capacity is that the relationship between the amount of water flowing at a given point in a river or stream (usually at gauging stations) and the corresponding stage in that section, known as a stage–discharge relationship or rating curve, is changed. The key messages deriving from the present work are: (a) the more frequent and extreme the floods become, the more rapid the changes in the stream channel cross-section become, (b) from an operational point of view, the collection and processing of field measurements of the stage and corresponding discharge at a given section in order to quickly and frequently update the rating curve becomes a priority. It is, therefore, necessary to define a control system for acquiring hydrological data capable of keeping river levels and discharges under control to support flood early warnings and water management. The proposed stage–discharge management system is used by the Civil Protection Service of the Marche Region (east-central Italy) for the monitoring of river runoff in the regional watersheds. The Civil Protection Service staff performs stage–discharge field measurements using water level sensors and recorders (e.g., staff gauges, submersible pressure transducers, ultrasound and radar sensors) and a current meter, acoustic doppler velocimeter, acoustic doppler current profilers, portable mobile radar profiler and salt dilution method equipment, respectively. Power functions are fitted to the stage–discharge field data. Furthermore, extrapolation is performed to cover the full range of flow measurements; in general, extrapolation is not an easy task because of sharp changes in the stream cross-section geometry for very high or very low stages. Introduction: To analyze outcomes on objective ocular surface parameters and subjective symptoms of serial weekly sessions using the Activa mask in patients with meibomian gland dysfunction (MGD). Methods: This is a prospective study including patients with symptomatic MGD who were treated with four weekly sessions using the Activa mask (SBM Sistemi, Turin, Italy). Noninvasive ocular surface examination was carried out before (T0) and 2 weeks after the last mask session (T1) using Idra (SBM Sistemi, Turin, Italy) for the measurement of: (1) noninvasive break-up time (NIBUT); (2) lipid layer thickness (LLT); (3) tear meniscus height (TMH); (4) meibomian gland loss (MGL) of upper and lower eyelids. The I-Pen tear osmolarity system (I-Med Pharma Inc, Dollard-des Ormeaux, Quebec, Canada) was used to measure tear osmolarity values. Ocular discomfort symptoms were ascertained by means of the ocular surface disease index (OSDI) questionnaire. Results: All 25 patients (11 males, 14 females; mean age 57.1 ± 11.9 years) regularly completed the cycle of four mask sessions. No patients used prohibited medications, and no device-related adverse events were noted. At T1, mean values of NIBUT and LLT increased significantly compared to T0 (respectively from 6.0 ± 1.4 to 6.6 ± 1.2 s, P = 0.043, and from 53.2 ± 17.4 to 65.3 ± 16.3 nm, P < 0.001), while mean values of MGL and tear osmolarity decreased significantly (respectively from 17.1 ± 9.3 to 15.1 ± 8.0%, P = 0.014, and from 307.3 ± 12.2 to 301.5 ± 6.8 mOsm/l, P = 0.005). In parallel, OSDI score reduced significantly from 62.4 ± 11.7 at T0 to 34.5 ± 11.2 at T1 (P < 0.001). Conclusion: Weekly serial sessions using the Activa mask significantly improved objective parameters of the ocular surface as well as subjective ocular discomfort symptoms in patients with recalcitrant MGD. To compare multimodal imaging findings in patients with choroidal lymphoma (CL). Multicenter retrospective observational case series. Multimodal imaging features of patients with CL were reviewed with particular attention to the patterns of choroidal infiltration on indocyanine green angiography (ICGA) and optical coherence tomography (OCT). Eighteen eyes of 15 patients were included in this study. Average tumor thickness on ultrasonography was 2.6 mm (range, 1.2-5.7 mm). Choroidal infiltration on ICGA was characterized by multifocal, round areas (300-500 microns diameter) of hypocyanescence in all cases, whereas OCT at the same region disclosed diffuse choroidal infiltration. By OCT, the tumor surface contour was primarily placid (22%), dome-shaped (11%), or undulating (67%). In this analysis of eyes with CL, ICGA demonstrated multifocal sub-millimeter regions of choroidal hypocyanescence whereas OCT documented diffuse choroidal infiltration. This incongruence could be a distinctive diagnostic feature of choroidal lymphoma, assisting with differentiation from other pathological entities. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate the outcomes of various lamellar keratoplasty techniques performed at our Institution in children aged 14 years or younger over the last decade. Methods: This single‐centre study reviewed 72 eyes that underwent lamellar keratoplasty for various indications. Deep anterior lamellar keratoplasty (DALK) was performed in 19 eyes, mushroom penetrating keratoplasty (PK) in 27 eyes and Descemet stripping automated endothelial keratoplasty (DSAEK) in 25 eyes. The main outcome measures included best spectacle‐corrected visual acuity (BSCVA), complications and rate of graft failure which was defined as any graft requiring repeat transplantation. Results: Best spectacle‐corrected visual acuity (BSCVA) significantly improved after DALK, mushroom PK and DSAEK (all p < 0.05), with 50%, 60% and 56% of eyes reaching ≥20/40, respectively. Stromal rejection was observed in 1 eye (5.3%) after DALK, whilst endothelial rejection occurred in 1 eye after mushroom PK (3.7%) and 1 eye after DSAEK (4.0%). Overall survival was 100% after DALK (mean follow‐up: 23.0 months), 92.8% after mushroom PK (mean follow‐up: 42.3 months) and 96.0% after DSAEK (mean follow‐up: 33.6 months). Conclusion: Deep anterior lamellar keratoplasty (DALK), mushroom PK and DSAEK offer good visual outcomes for children with corneal pathology, with low rates of immunological rejection and graft failure. To investigate whether diurnal changes in noninvasive ocular surface parameters and subjective symptoms occur in healthy subjects wearing face mask who were analyzed before and after 8 h of continuous use. In this prospective cross-sectional study, healthy volunteers attending the same workplace environment underwent a noninvasive ocular surface workup by means of Keratograph 5 M (Oculus, Wetzlar, Germany) in the same day at 2 different time points: (i) in the early morning before wearing face mask (T0); (ii) after 8 h of continuous face mask use (T1). Noninvasive break-up time (NIBUT), tear meniscus height (TMH), ocular redness and meibomian gland dropout were measured. All subjects were asked to complete the Ocular Surface Disease Index (OSDI) questionnaire before and after 8 h of face mask wearing. Data from 20 healthy subjects (10 males and 10 females, mean age 25.1 ± 3.9 years) were included. Mean value of TMH decreased significantly from 0.29 ± 0.07 at T0 to 0.23 ± 0.07 mm at T1 (P < 0.001); conversely, mean values of NIBUT, redness score and meibomian gland dropout did not change significantly after continuous face mask wearing (always P > 0.532). Concerning ocular discomfort symptoms, mean value of OSDI score worsened significantly at T1 compared to T0 (from 12.9 ± 12.6 to 19.4 ± 12.0; P = 0.017). One of the most critical greenhouse gases in the atmosphere is carbon dioxide (CO2) due to its long-lasting and negative impact on climate change. The global atmospheric monthly mean CO2 concentration is currently greater than 410 ppm which has changed dramatically since the industrial era. To choose suitable climate change mitigation and adaptation strategies it is necessary to define carbon dioxide mass distribution and global atmospheric carbon dioxide mass. The available method to estimate the global atmospheric CO2 mass was proposed in 1980. In this study, to increase the accuracy of the available method, various observation platforms such as ground-based stations, ground-based tall towers, aircrafts, balloons, ships, and satellites are compared to define the best available observations, considering the temporal and spatial resolution. In the method proposed in this study, satellite observations (OCO2 data), from January 2019 to December 2021, are used to estimate atmospheric CO2 mass. The global atmospheric CO2 mass is estimated around 3.24 × 1015 kg in 2021. For the sake of comparison, global atmospheric CO2 mass was estimated by Fraser's method using NOAA data for the mentioned study period. The proposed methodology in this study estimated slightly greater amounts of CO2 in comparison to Fraser's method. This comparison resulted in 1.23% and 0.15% maximum and average difference, respectively, between the proposed method and Fraser's method. The proposed method can be used to estimate the required capacity of systems for carbon capturing and can be applied to smaller districts to find the most critical locations in the world to plan for climate change mitigation and adaptation. Purpose: To evaluate the efficacy and predictability of cataract extraction with toric intraocular lens (IOL) implantation after deep anterior lamellar keratoplasty (DALK) or mushroom penetrating keratoplasty (PK). Setting: Villa Igea Hospital, Forli, Italy. Design: Prospective case series. Methods: Toric IOL implantation was offered to patients with cataract, corneal astigmatism >1.5 diopters (D) and regular central corneal topography after complete suture removal. Phacoemulsification was performed through a 2.4 mm scleral tunnel and an enVista monofocal toric MX60T or Eyecryl monofocal toric IOL was inserted in the capsular bag. Main outcome measures were uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refraction, total prediction error, and IOL misalignment. Results: 37 consecutive patients who had previously undergone either DALK (n = 27, 73%) or 2-piece mushroom PK (n = 10, 27%) were included. All patients completed the 6-month follow-up. The mean toric IOL power was 5.3 +/- 1.1 D. Both UDVA and CDVA significantly improved (from 1.02 +/- 0.27 to 0.46 +/- 0.31 logMAR and from 0.65 +/- 0.27 to 0.11 +/- 0.12 logMAR, respectively; P <.001). 20 eyes (54%) reached UDVA >=20/40, whereas 35 eyes (95%) reached a CDVA >=20/40. Final refractive astigmatism was 0.93 +/- 0.87 D, with 35 eyes (95%) within 2 D. Prediction error was <=1 D in 18 eyes (49%). Absolute IOL misalignment was 3.3 +/- 3.5 degrees. /p> Conclusions: Toric IOL implantation in postkeratoplasty eyes allowed reduction of refractive astigmatism to predictably low levels with concomitant improved visual outcomes. Macular OCT diagnoses were cystoid macular oedema in five patients, epiretinal membrane in three patients, and exudative age-related macular degeneration in one patient; while the OCT was normal in 21 patients. Although posterior segment optical coherence tomography (OCT) would be the preferred diagnostic tool for this purpose, significant corneal oedema can reduce the quality of OCT images possibly leading to a missed or erroneous diagnosis. Background: The purpose of this study was to assess the accuracy of intraocular lens power (IOL) formulas for cataract surgery after deep anterior lamellar keratoplasty (DALK). Methods: This retrospective study included eyes which had previously undergone DALK and underwent standard phacoemulsification with monofocal IOL implantation between January 2012 and January 2021 at Ospedali Privati Forlì "Villa Igea" (Forlì, Italy). The predicted spherical equivalent (SE) was calculated using the Barrett Universal II, Emmetropia Verifying Optical (EVO), Haigis, Hoffer Q, Hoffer QST, Holladay 1, Holladay II, Kane and SRK/T formulas. Prediction error (PE) was calculated as the actual postoperative SE refraction minus the SE predicted refraction. Results: Eighty-two eyes of 82 patients were included. The mean PE was negative using all formulas. Friedman test revealed a statistically significant difference of the median absolute PE (MedAE) among the different IOL formulas (P = 0.005). On the basis of the MedAE, the formulas were ranked as follows: SRK/T (0.805 D), Kane (0.810 D), EVO (0.845 D), Hoffer QST (0.847 D), Barrett (0.895 D), Holladay 1 (0.915 D), Haigis (1.010 D) and Hoffer Q (1.070 D) formulas. Conclusions: All formulas had a tendency towards a myopic refractive shift in post-DALK eyes. Pontederiaceae is a small family of Monocots, composed of two genera (i.e., Heteranthera and Pontederia) and ca. 40 species. Two genera and 27 species are accepted for Brazil. In the state of Espírito Santo, Heteranthera is represented by two species and Pontederia by four species. This work is a part of the Flora do Espírito Santo project. The aim was to morphologically characterize the species of Commelinaceae occurring in the state of Espírito Santo and provide data on their geographical distribution, phenology, and comments. Fieldwork and herbarium studies were carried out. A total of 38 species, arranged in 10 genera, were recorded. Dichorisandra is the richest genus (17 species), followed by Commelina (seven spp.), Tradescantia (four spp.), Siderasis (three spp.), Tripogandra (two spp.), and Aneilema, Callisia, Floscopa, Gibasis and Tinantia (one species each). Two species are herein described as new to science, and the synonymization of D. albomarginata is proposed under the older D. procera. Inflorescence (type and position), stamens (number, color, and anther dehiscence), fruit (shape and ornamentation), and seeds (ornamentation, the position of the embryotega, and hilum shape) are the key diagnostic morphological characters for genus and species delimitation. Most of the species richness and abundance are concentrated in the mountain region of Espírito Santo, one of the largest forest areas in the state, showing the importance of conserving the remaining Atlantic Forest's fragments. Background: To date, there is still no consensus regarding the effect of binocular treatment for amblyopia. The purpose of this systematic review and meta-analysis was to summarize the available evidence to determine whether binocular treatment is more effective than patching in children with amblyopia. Methods: Four electronic databases (PubMed, Scopus, Web of Science, and Cochrane Central Register of Controlled Trials) were searched for studies that compared binocular treatment and patching in children with amblyopia. The outcome measures were visual acuity and stereopsis. Pooled effects sizes were calculated with a random-effect model. The standardized difference in means (SDM) with 95% confidence intervals (CI) was calculated. Sensitivity analysis and assessment of publication bias were performed. Results: Five randomized clinical trials were included. No significant difference in visual acuity between patients treated with binocular treatment and patching was observed (SDM = -0.12; 95% CI: -0.45–0.20; P = 0.464). No significant difference in stereopsis between patients treated with binocular treatment and patching was observed (SDM = -0.07; 95% CI: -0.61–0.48; P = 0.809). For both variables, the between-study heterogeneity was high (respectively, I2 = 61% and I2 = 57%). Conclusions: This meta-analysis found no convincing evidence supporting the efficacy of binocular treatment as an alternative to conventional patching. Therefore, the binocular treatment cannot fully replace traditional treatment but, to date, it can be considered a valid complementary therapy in peculiar cases. Computational drug repositioning aims at ranking and selecting existing drugs for novel diseases or novel use in old diseases. In silico drug screening has the potential for speeding up considerably the shortlisting of promising candidates in response to outbreaks of diseases such as COVID-19 for which no satisfactory cure has yet been found. We describe DrugMerge as a methodology for preclinical computational drug repositioning based on merging multiple drug rankings obtained with an ensemble of disease active subnetworks. DrugMerge uses differential transcriptomic data on drugs and diseases in the context of a large gene co-expression network. Experiments with four benchmark diseases demonstrate that our method detects in first position drugs in clinical use for the specified disease, in all four cases. Application of DrugMerge to COVID-19 found rankings with many drugs currently in clinical trials for COVID-19 in top positions, thus showing that DrugMerge can mimic human expert judgment. Purpose. To investigate the short-term effects on tear film parameters and ocular symptoms of a novel eye mask producing heat and vibration developed for the treatment of dry eye disease owing to meibomian gland dysfunction (MGD). Methods. This is a pilot study including the first 20 consecutive patients (6 males, 14 females; mean age 52.4 ± 16.8 years) who were treated with a novel eye mask producing heat (42°) and vibration (up to 20 Hz) (Activa, SBS Sistemi, Turin, Italy) for 15 minutes. The treatment incorporates 2 phases in the following chronological order: 5 minutes of heating (phase I); 10 minutes of combination of heating and vibration (phase II). Noninvasive ocular surface examination was carried out before (T0) and 30 minutes after the mask session (T1) by means of Idra (SBS Sistemi, Turin, Italy) for the measurement of noninvasive break-up time (NIBUT) and lipid layer thickness (LLT). Patients' satisfaction after treatment was ascertained by asking the patients whether they perceived improvement from their baseline symptoms according to a 5-grade scale: none = 0; trace = 1; mild = 2; moderate = 3; high = 4. Results. All patients completed regularly the mask session and no device-related adverse events were noted. NIBUT improved significantly from T0 to T1 (from 7.2 ± 1.8 s to 8.1 ± 2.1; P = 0.014). In parallel, also LLT improved from from T0 to T1 (72.5 ± 13.9 nm to 83.1 ± 16.1; P = 0.016). Seven patients (35% of the total) had a moderate satisfaction (grade 3) while 13 patients (65%) had a high satisfaction (grade 4) with treatment. Conclusions. This eye mask represents a novel well tolerated tool in the armamentarium of MGD treatments. Thirty minutes after the session, NIBUT and LLT increased significantly; furthermore, all patients reported an improvement of discomfort symptoms with a moderate to high satisfaction with treatment. For a patient affected by breast cancer, after tumor removal, it is necessary to decide which adjuvant therapy is able to prevent tumor relapse and formation of metastases. A prediction of the outcome of adjuvant therapy tailored for the patient is hard, due to the heterogeneous nature of the disease. We devised a methodology for predicting 5-years survival based on the new machine learning paradigm of coherent voting networks, with improved accuracy over state-of-the-art prediction methods. The 'coherent voting communities' metaphor provides a certificate justifying the survival prediction for an individual patient, thus facilitating its acceptability in practice, in the vein of explainable Artificial Intelligence. Ultrasound cyclo plasty (UCP) is a recently developed surgical technique for glaucoma allowing a selective and controlled coagulation of the ciliary body. We herein investigated the long-term efficacy and safety of UCP for the treatment of glaucoma. This prospective study included patients with primary and secondary glaucoma. All surgeries were performed using the EyeOP1 device (Eye Tech Care, Rillieux-la-Pape, France). Sixty-six patients were included, and 60 completed regularly the 2-year follow-up. Preoperative IOP was 28.5 ± 9.6 mmHg and significantly decreased to 17.0 ± 5.4 at 2 years (p < 0.001). The daily number of both hypotensive eye drops and acetazolamide tablets decreased significantly (respectively, from 2.6 ± 1.1 to 1.7 ± 1.2 and from 0.7 ± 0.8 to 0.2 ± 0.5; both p < 0.001). At 2 years, 68.1% of patients met the definition of qualified success (IOP < 21 mmHg regardless of glaucoma medications) and 10.3% of patients met the definition of complete success (IOP < 21 mmHg without glaucoma medications). No major intra- or postoperative complications occurred; however, 15 eyes required additional glaucoma surgery. To compare the outcomes of deep anterior lamellar keratoplasty (DALK) using dehydrated versus standard organ culture-stored donor corneas for eyes with keratoconus. Prospective, randomized, single-center trial conducted in Italy. Adult patients (age ≥ 18 years) with keratoconus scheduled for elective DALK. Patients undergoing successful type 1 bubble pneumatic dissection using a standard DALK technique were randomized during surgery to receive either dehydrated (n = 30) or standard organ culture-stored (n = 30) donor corneas. The primary study outcome was best spectacle-corrected visual acuity (BSCVA) 12 months after surgery. Secondary outcomes were refractive astigmatism (RA), endothelial cell density (ECD), and complication rates. Postoperative BSCVA did not significantly differ between groups at both time points: mean difference at 6 months was 0.030 logarithm of the minimum angle of resolution (logMAR; 95% confidence interval [CI], –0.53 to 0.10 logMAR; P = 0.471) and at 12 months was –0.013 logMAR (95% CI, –0.10 to 0.08 logMAR; P = 0.764). No significant differences between groups were observed in terms of postoperative RA and ECD at all time points. In the first 3 days after DALK, an epithelial defect was present in 10 patients (33%) in the organ culture cornea group and in 29 patients (97%) in the dehydrated cornea group. Complete re-epithelialization was achieved by day 7 in all patients (100%) in both groups. The study provides evidence that the use of dehydrated corneas is noninferior to the use of standard organ culture donor corneas for DALK. Evaluating severe accidents in Generation IV sodium-cooled fast reactors (SFRs) is challenging due to the eutectic reaction between boron carbide (B 4 C) and stainless steel (SS), which results in boron migration and enhanced neutron absorption. A novel quantitative visualization method using radiative heating to observe eutectic behavior and resulting melt structure during boron migration. Experiments, replicating real-scale B 4 C-SS control rods, were conducted in an integrated test facility under core disruptive accidents (CDAs) conditions, introducing a new approach. This study explores the long duration melting (candling phenomenon) of boron within stainless steel, shedding light on relocation behavior not previously studied. We identified two distinct failure mechanisms: the separation of SS from the B 4 C pellet, resulting in the formation of a later melting drop, and the fracture of the B 4 C pellet into multiple pieces, possibly due to thermal stress. The visualization technique and thermal interfacial resistance analysis precisely captured the eutectic temperature, aligning well with prior research. Our study yield insights into eutectic melt relocation behavior by systematically characterizing relocated solidified eutectic melt with a cooling rate of 50 °C/s in stagnant argon environment via scanning electron microscopy-energy dispersive X-ray spectroscopy (SEM-EDS), X-ray diffraction (XRD), and microhardness testing. Our study shows that due to high chromium content, (Cr,Fe) 2 B phases are observed in XRD analysis instead of just Fe 2 B phase. Other phases present in our solidified eutectic melt include γ-Fe (fcc), (Cr,Fe) 23 (B,C) 6 (cubic), and (Cr,Fe) 5 B 3. The atomic composition within these zones was ascertained through SEM-EDS analysis. Our study shows varying hardness values in distinct phases, reflecting differing boron concentrations within micrograph zones. Chromium promotes (Cr,Fe) 2 B and (Cr,Fe) 5 B 3 boride formation, transforming the structure from tetragonal to orthorhombic with increased chromium content, thereby increasing hardness. Introduction: The purpose of this study was to evaluate choroidal vascular changes in patients with neovascular age-related macular degeneration (nAMD) treated with aflibercept injection over a 3-month period. Methods: Enhanced depth imaging optical coherence tomography scans of 60 eyes with treatment-naïve nAMD and 60 unaffected fellow eyes were retrospectively analyzed. Data was collected at baseline and after 3 monthly intravitreal injections of aflibercept. The ImageJ software was used to binarize OCT scans and measure total choroid area (TCA), luminal area (LA), and stromal area (SA). Choroidal vascularity index (CVI) was defined as the ratio of LA to TCA. Results: After treatment, subfoveal choroidal thickness (CT) in nAMD eyes significantly decreased from 210. 6 ± 61.6 to 194.6 ± 58.7 μm (P < 0.001), TCA from 1.620 ± 0.502 to 1.500 ± 0.451 mm2 (P < 0.001), LA from 1.075 ± 0.335 to 0.985 ± 0.307 mm2 (P < 0.001), SA from 0.545 ± 0.176 to 0.516 ± 0.153 mm2 (P = 0.005), and CVI from 66.36 ± 2.89 to 65.46 ± 2.87% (P = 0.009). The decrease of CVI after treatment was significantly correlated with baseline CVI (Rs = 0.466, P < 0.001), but not with the change in BCVA and presence of dry macula after treatment (always P > 0.05). Conclusion: Choroidal thickness and vascularity significantly decreased after treatment with aflibercept in nAMD eyes. A new species of Heteropterys belonging to the Parabanisteria informal group is described for the savannas grasslands of the Serra do Tombador Natural Reserve, municipality of Cavalcante, State of Goiás, Brazil. Heteropterys rosmarinifolia is easily distinguished from the remaining Brazilian species of the Parabanisteria informal group due to its stems unbranched, leaves associated with the inflorescence verticillate, blades linear to very narrowly oblong, strongly conduplicate, ascending to upright, and sparsely sericeous to glabrous at age. We present a complete morphological description for the new species, photographic plates, comments on its distribution, ecology, and taxonomy, besides an identification key to the species of Heteropterys from the Parabanisteria group from the State of Goiás, Brazil. Purpose: To evaluate choroidal structural changes in patients with multiple evanescent white dot syndrome (MEWDS) during the acute and recovery stages.Methods: Enhanced-depth imaging optical coherence tomography (EDI-OCT) scans of 16 patients with unilateral MEWDS were acquired during the acute and recovery stages in both eyes. Images were binarized with the ImageJ software to measure subfoveal choroidal thickness (CT), total choroid area, luminal area and choroidal vascularity index (CVI).Results: In the acute stage, subfoveal CT, total choroidal area and CVI were significantly higher in eyes with MEWDS compared to fellow eyes (371.2 ± 101.8 vs 317.1 ± 90.3 µm, p = .001; 2.826 ± 0.686 vs 2.524 ± 0.674 mm2, p = .014; 69.49 ± 3.51 vs 68.27 ± 3.41%, p = .044, respectively). Continuous variables were compared between patients with DED and controls by using the Mann–Whitney U test. The area under the curve (AUC) of receiver operating characteristic curves was calculated. The correlations between ocular surface parameters were evaluated with Pearson correlation analysis. Results. Patients with DED showed significantly lower values of NIBUT, LLT, and TMH compared to controls (6.9 ± 2.5 vs 10.4 ± 2.4 s, P < 0.001; 64.6 ± 20.3 vs 73.4 ± 21.9 nm, P = 0.003; 0.231 ± 0.115 vs 0.289 ± 0.164, P = 0.012, respectively). Based on extensive herbarium, field, botanical illustration, and molecular phylogenetic research, five genera and eight species are recognised for the Neotropical Haemodoraceae. New taxa include Cubanicula Hopper et al., Xiphidium pontederiiflorum M.Pell. et al. and Schiekia timida M.Pell. et al. Two new combinations are made, Cubanicula xanthorrhizos (C.Wright ex Griseb.) Hopper et al. and Schiekia silvestris (Maas & Stoel) Hopper et al. We also correct the author citation for Xiphidium, provide the necessary typifications for several names and present an updated identification key, comments, and photo plates for all species. Genes are organized in functional modules (or pathways), thus their action and their dysregulation in diseases may be better understood by the identification of the modules most affected by the disease (aka disease modules, or active subnetworks). We describe how an algorithm based on the Core&Peel method is used to detect disease modules in co-expression networks of genes. We first validate Core&Peel for the general task of functional module detection by comparison with 42 methods participating in the Disease Module Identification DREAM challenge. Next, we use four specific disease test cases (colorectal cancer, prostate cancer, asthma, and rheumatoid arthritis), four state-of-the-art algorithms (ModuleDiscoverer, Degas, KeyPathwayMiner, and ClustEx), and several pathway databases to validate the proposed algorithm. Core&Peel is the only method able to find significant associations of the predicted disease module with known validated relevant pathways for all four diseases. Moreover, for the two cancer datasets, Core&Peel detects further eight relevant pathways not discovered by the other methods used in the comparative analysis. Finally, we apply Core&Peel and other methods to explore the transcriptional response of human cells to SARS-CoV-2 infection, finding supporting evidence for drug repositioning efforts at a pre-clinical level. The Dichorisandra acaulis group has its circumscription broadened to include two additional species, viz. D. oxypetala and the herein newly described D. forzzae. Dichorisandra oxypetala has previously not been included in this morphological group due to its deviant vegetative morphology. However, its reproductive morphology is congruent with the D. acaulis group and, thus, it is here included as a member of the later. Dichorisandra forzzae is here described to accommodate the specimens with anthers dehiscent through one apical pore, erroneously identified as D. acaulis in the state of Espírito Santo, Brazil. We provide illustrations, taxonomic comments and an updated identification key for the species of the D. acaulis morphological group. Importance In elderly people, visual impairment is associated with depressive symptoms and cognitive decline. However, the impact of cataract surgery on depression and cognitive impairment is still controversial. Background: To evaluate the effect of cataract surgery on depression and cognitive status in the elderly. Design This study was a meta‐analysis. Participants: Patients with age‐related cataract who underwent cataract surgery were recruited for this study. Methods: A literature search was performed on PubMed, Scopus and Web of Science. Data were extracted from selected studies by two independent reviewers. The pooled standardized mean difference (SDM) was estimated using a random‐effects model. Heterogeneity was evaluated using the Q and I2 tests. Multiple sensitivity analyses and assessment of publication bias were performed. Main Outcome Measures: The main outcome was a report of a measure of depression or cognitive impairment before and after surgery. Results: Sixteen studies were included: 14 of them reported data on depression and 9 of them on cognitive function. Depression significantly decreased after surgery (SDM = 0.460; 95% CI: 0.223‐0.697; P <.001). In six controlled studies, the reduction of depression was higher in the surgery group than in the control group (SDM = 0.161; 95% CI: 0.027‐0.295; P =.019). Cognitive function significantly improved after surgery (SDM = 0.254; 95% CI: 0.120‐0.388; P <.001). In four controlled studies, the improvement of cognitive function was higher in the surgery group than in the control group (SDM = 0.188; 95% CI: 0.002‐0.374; P =.048). Results were stable after sensitivity analyses. A new species of Dichorisandra from the Brazilian Atlantic Forest is herein described, illustrated, and compared to morphologically similar species. Dichorisandra striatula is characterized by leaf blades with small and discontinuous white stripes on the adaxial surface, hirsutulous indumentum on both surfaces, obovate to broadly obovate petals, and ellipsoid fruits. Information on phenology, habitat, conservation status, and a geographical distribution map are also provided. Objectives: To investigate longitudinally corneal sub-basal nerve plexus (SNP) by means of in vivo confocal microscopy (IVCM) in the contralateral eye (CE) of patients with unilateral neurotrophic keratitis (NK) secondary to central nervous system (CNS) diseases who underwent different treatments. Methods: Ten patients with NK and 10 matched controls were included. In 7 NK patients, conservative treatment maintained unchanged the clinical picture over the 1-year follow-up (Group 1), while NK progressed in 3 patients who underwent direct corneal neurotization (Group 2). IVCM scans of SNP of NK patients were acquired in CE at baseline (V0) ad after 1-year follow-up (V1). All images were analyzed with the automated software "ACCMetrics" and compared with controls. The following IVCM corneal nerve parameters were calculated at V0 and V1 with ACCMetrics: fiber density (CNFD), branch density (CNBD), fiber length (CNFL), total branch density (CTBD), fiber area (CNFA), fiber width (CNFW), and fractal dimension (CNFrD). Results: At V0, significantly lower mean values of CNFD and CNBD, and higher values of CNFW were detected in CE of NK patients compared to controls (respectively, 16.9 ± 8.7 vs 25.0 ± 8.3 n/mm2, P=.029; 19.3 ± 13.8 vs 33.8 ± 18.9 n/mm2, P=.023; 0.022 ± 0.002 vs 0.020 ± 0.001 mm/mm2, P<.001). From V0 to V1, all IVCM metrics of CE remained unchanged in Group 1, while they improved in Group 2. Conclusions: Contralateral eye of patients with unilateral NK secondary to CNS disease showed lower CNFD and CNBD and higher CNFW compared to controls. Purpose: The purpose of this study was to assess the effect of long-lasting botulinum A toxin injections on ocular surface parameters and to further investigate the relationship between these parameters and the duration of the treatment.Methods: In this retrospective study, patients with unilateral hemifacial spasm who were receiving botulinum A toxin injections for at least 1 year were analyzed. Healthy contralateral eyes acted as controls. The ocular surface examination included Ocular Surface Disease Index questionnaire, Schirmer test type I, tear film break-up time (TFBUT), tear osmolarity, corneal sensitivity, and corneal fluorescein staining.Results: Twenty-six patients (6 males and 20 females; mean age 76.4 ± 8.9 years) were included in the study. The mean duration of the treatment was 7.2 ± 5.4 years, and the mean frequency of injections was of one every 3.3 ± 0.4 months. TFBUT, Schirmer test, and corneal sensitivity were significantly lower in the eye homolateral to hemifacial spasm compared with the contralateral one (5.9 ± 3.2 vs 7.5 ± 4.2 s, P = 0.001; 6.2 ± 3.4 vs 9.2 ± 6.6 mm, P = 0.031; 50.8 ± 3.7 mm vs 52.3 ± 2.9 mm, P = 0.048, respectively). One month after the last injection, TFBUT further decreased from 5.9 ± 3.2 to 2.3 ± 1.2 s (P = 0.028). A significant positive correlation was found between the duration of treatment and tear osmolarity (ρ = 0.542, P = 0.025).Conclusion: Patients with hemifacial spasm under long-lasting treatment with serial botulinum A toxin injections showed a reduction in tear film production and stability, as well as corneal sensitivity in the treated eye compared with the contralateral one. Purpose: Graft-versus-host disease (GVHD) is a major complication of allogeneic hematopoietic stem cells transplantation, occurring in about half of transplanted patients. This condition seems to be the result of a progressive immune-mediated damage that can involve various tissues, including the eyes. The ocular surface system is the ocular structure most frequently impaired, and dry eye disease is considered the hallmark of ocular GVHD. Given the increasing prevalence and the frequent severe involvement of the ocular surface with vision-threatening complications, ocular GVHD represents a current diagnostic and therapeutic challenge. The purpose of this literature review is to describe all the clinical manifestations occurring in the setting of ocular GVHD, and to further report the outcomes of conventional and novel therapies. Methods: A literature search about ocular GVHD was performed in PubMed, Scopus, Medline databases, and ClinicalTrials.gov as well as through the reference lists of identified publications until January 2019. We have included RCTs, prospective observational studies, prospective and retrospective cohort studies, pilot studies, and review articles. Results: Overall, 107 articles, 3 book chapters, and 6 ongoing registered clinical trials were collected and analyzed. Ocular GVHD can affect all the structures of the entire ocular surface system, including lacrimal and meibomian glands, cornea, conjunctiva, eyelids, nasolacrimal duct, and tears. Current medical treatment is mainly focused on lubrication and control of drainage, tear evaporation, and ocular surface inflammation. Surgical treatment may be necessary in severe, recalcitrant, or complicated cases. Amniotic membrane and tectonic keratoplasty can be valid options to restore the integrity of the cornea. Recently, conjunctival and limbal transplantation from the same living-related bone marrow donor has been proposed to manage both dry eye and limbal stem cell deficiency, without any risk of immunologic rejection. Conclusion: This review provides an up-to-date analysis on clinical findings and current and future management of ocular GVHD. Purpose: To compare corneal biomechanics between patients with ocular graft versus-host disease (oGVHD) and healthy subjects (controls), and to further correlate these values with ocular and hematological characteristics. Materials and methods: The following procedures were performed in oGVHD patients and controls: Schirmer test (ST), break-up time (BUT), corneal and conjunctival staining, tear matrix metalloproteinase-9 (MMP-9) assay (InflammaDry test, Rapid Pathogen Screening, Inc, Sarasota, FL). Corneal biomechanics were calculated by using ocular response analyzer (ORA, Reichert Instruments, Depew, New York, USA). The Mann-Whitney U test was used to compare continuous variables between oGVHD patients and controls. Correlations of corneal biomechanics with ocular and hematological parameters were examined using Spearman's correlation. Results: A total of 45 oGVHD patients (mean age ± SD, 51.5 ± 7.1 years) and 34 controls (47.8 ± 6.1 years) were included. Patients with oGVHD showed significantly lower values of corneal hysteresis (CH) and corneal resistance factor (CRF) compared to controls (respectively, 9.4 ± 1.8 mmHg vs 11.6 ± 1.6 and 9.7 ± 1.4 mmHg vs 12.3 ± 1.3; always p<0.001). Twenty-nine of the oGVHD eyes (64.4%) were strong-positive for MMP-9, while 16 (35.6%) were weak-positive. Conversely, only 4 of the control eyes (11.8%) were weak-positive for MMP-9. In patients with oGVHD, CH was significantly correlated with corneal staining (Rs = -0.316, p = 0.035), conjunctival staining (Rs = -0.437, p = 0.003), ST (Rs = 0.390, p = 0.008), BUT (Rs = 0.423, p = 0.004), oGVHD severity grade (Rs = -0.383, p = 0.009), and MMP-9 positivity grade (Rs = -0.429, p = 0.003), while CRF was correlated only with corneal staining (Rs = -0.317, p = 0.034). Conclusions: Corneal biomechanics are reduced in patients with oGVHD, and CH is negatively correlated with disease severity grade and MMP-9 tear levels. [ABSTRACT FROM AUTHOR]

Purpose: To compare swept source OCTA device, with and without the extended field imaging (EFI) technique, to standard fluorescein angiography (FA) in the clinical practice. Methods: Consecutive patients with vascular disorder patients underwent FA with 55-degree lens (Spectralis Heidelberg Engineering, Heidelberg, Germany) and OCTA with the prototype PlexElite (Carl Zeiss Meditec, Dublin, CA) using a 12 mm x 12 mm volume scan pattern centered on the fovea and a prototype of + 20.00-diopter designed specifically by Zeiss. The imaging methods were compared for visible field of view, extension of non-perfused areas, presence and number of neovessels, vessel density (VD) and fractal dimension (FD). Results: Forty-three eyes of 27 patients were included. The mean extension ratio of EFI SS-OCTA compared to SS-OCTA without EFI and FA were 1.97 ± 0.02 and 0.85 ± 0.01. The mean extension of non-perfused areas with EFI SS-OCTA (34.22 ± 33.4 mm2) was significantly higher than SS-OCTA without EFI (20.46 ± 18.70 mm2), and with FA (27.55 ± 4.4 mm2). The mean VD and FD of EFI SS-OCTA were significantly different compared to SS-OCT without EFI. Conclusions: EFI SS-OCTA captured larger areas than SS-OCTA without EFI and FA. OCTA in a single shot is able to obtain more information of the retina without the use of montage techniques. Despite the determination of retinal ischemia seems to be easier and more accurate using EFI SS-OCTA, FA offers more details of the perfusion status of the retina. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate in vivo confocal microscopy (IVCM) features of corneal subbasal nerve plexus (SNP) in the setting of dry eye disease (DED) using fully automated software "ACCMetrics," and to further investigate its diagnostic performance in discriminating DED patients.Methods: IVCM exams of SNP in DED patients and matched control subjects were performed using Heidelberg Retina Tomograph with the Rostock Cornea Module. The following parameters were obtained with ACCMetrics: corneal nerve fiber density (CNFD), corneal nerve branch density (CNBD), corneal nerve fiber length (CNFL), corneal nerve total branch density (CTBD), corneal nerve fiber area (CNFA), corneal nerve fiber width (CNFW), and corneal nerve fractal dimension (CNFrD). The Mann-Whitney U test was used to compare variables. Receiver operating characteristic curves with calculations of the area under the curve (AUC) were used to describe the accuracy of IVCM parameters for discriminating DED patients from controls.Results: Thirty-nine DED patients and 30 control subjects were included. Significantly, lower values of CNFD, CNBD, and CNFL and higher value of CNFW were found in DED patients compared to controls (respectively, 20.5 ± 8.7 vs 25.4 ± 6.7 n/mm2; 25.6 ± 20.1 vs 37.6 ± 21.5 n/mm2; 12.6 ± 4.4 vs 14.5 ± 2.9 mm/mm2; 0.021 ± 0.001 vs 0.019 ± 0.001 mm/mm2; always p < 0.024). CNFW value had the highest diagnostic power in discriminating DED patients (AUC = 0.828). When the diagnosis of DED was made based on either CNFW or CNBD, the sensitivity was 97.4% and the specificity 46.7%.Conclusions: The software ACCMetrics was able to rapidly detect SNP alterations occurring in the setting of DED and showed good diagnostic performance in discriminating DED patients. Background: We previously reported the Screening with tomosynthesis or standard mammography-2 (STORM-2) trial, showing that tomosynthesis (3D-mammography) screening detected more cancers than 2D-mammography in double-reading practice. In this study, we report reader-specific detection measures for radiologists who performed the screen-reading in this trial.Methods: This is a sub-study of the STORM-2 trial which prospectively integrated 3D-mammography with acquired or synthetized 2D-mammograms in parallel double-reading arms. Asymptomatic women ≥49 years who attended population-based screening (Trento, 2013-2015) were recruited. Screening participants were recalled at any positive sequential screen-read in either reading arm of the trial. Radiologist-specific detection measures were calculated for each of seven radiologists who performed screen-reads: number of detected cancers, proportion of true-positive (TP) detection, and number and rate of false-positive (FP) recalls (FPR). We estimated incremental cancer detection rate (CDR) from integrating 3D-mammography in screen-reading.Results: Across all radiologists, TP detection (relative sensitivity) ranged between: 46% and 100% (median 59.5%) for 2D-mammography; 75% and 100% (median 76%) for integrated 2D/3D-mammography screening; 56% and 76% (median 64%) for 2Dsynthetic; 67% and 88% (median 78%) for 2Dsynthetic/3D-mammography. Integrating 3D-mammography led to incremental CDRs between 0/1000 and 3.5/1000 screens. FPR ranged between: 1.2% and 2.7% (median 2.25%) for 2D-mammography; 1.5% and 3.4% (median 2.75%) for 2D/3D-mammography; 1.6% and 4.6% (median 2.4%) for 2Dsynthetic; and 1.8% and 6.7% (median 3.0%) for 2Dsynthetic/3D-mammography.Conclusions: There was variability in the magnitude of effect from integrating 3D-mammography (relative to screen-reading with acquired or synthesised 2D-mammography alone) on individual radiologist's TP and FP detection, although there was an overall pattern of increasing cancer detection and also increasing FP recall for most readers. Fine particulate matter (PM) emission from biomass boilers for non-industrial heating represents one of the most important causes (together with the transport sector) of air pollution, in particular during winter. Separation technologies for fine PM are already well-known and adopted on an industrial scale, as a consequence of strict limits set by national and international regulations. On domestic boilers, the same technologies utilized on an industrial scale are not feasible due to high investment costs. Moreover, the emission limits for small size biomass boilers are higher than for industrial boilers, so high efficiency separation technologies are not needed, and are sometimes not present at all. The main goal of the paper is the development and testing of a mathematical model that is able to foresee the PM removal efficiency of a wet scrubber device. After an experimental validation based on several tests, it was possible to approach the preliminary design of an innovative wet scrubber, which is described in the paper. The main characteristics are (i) removal efficiency over 99.9%, (ii) specific energy consumption under 36 kJ m −3 , which is an industrial reference, and (iii) relatively low investment, operation and maintenance costs. A total evidence phylogeny for Pontederiaceae is herein presented based on new morphological and previously published molecular data. Our results led us to re-circumscribe Pontederia to include Monochoria, Pontederia s.s. and the polyphyletic Eichhornia. We provide the needed ten new combinations and 16 typifications, arrange a total of 25 accepted species (six representing re-established names) in 5 new subgenera. Furthermore, we provide an identification key for the two genera accepted by us in Pontederiaceae, an identification key to the subgenera, identification keys to the species of each subgenus and commentaries on Pontederia s.l., as well as for each subgenus and each species. To report a case of impending central retinal vein occlusion (iCRVO) in a COVID-19 patient. A 54 years old woman with COVID-19 related pneumonia presented to our emergency department complaining of scotomas and decreased vision in her right eye. Funduscopic examination and multimodal imaging revealed rare retinal hemorrhages, retinal whitening, and fern-like hypo-autofluorescent appearance typical of iCRVO. She had no risk factors other than a transient hyper-coagulability status likely related to the ongoing infection. Systemic treatment with steroids normalized her inflammatory and coagulation status and the occlusion completely resolved. I present the first taxonomic revision for T. subg. Austrotradescantia, based on extensive field, cultivation and herbaria studies. I accept 13 species, three of them (i.e. T. atlantica, T. hertweckii and T. tucumanensis) being described as new in the present study. I provide an identification key to the species, distribution maps, descriptions, comments, conservation assessments and illustrations for all species. The troublesome weed T. fluminensis has its specific limits clarified and its native range is presented so it can serve as a basis to better understanding its ecological requirements and to help control it throughout its invasive range. Furthermore, I highlight that T. mundula, a commonly neglected species closely related to T. fluminensis, might also represent a troublesome weed. Tradescantia mundula has been widely introduced in cultivation under the name T. albiflora and seems to have also escaped from cultivation.
