A 29-year-old man experienced myocarditis and perimyocarditis after receiving the tozinameran (Pfizer BioNTech COVID-19) vaccine. The man had a history of previous perimyocarditis episodes and had been taking colchicine and metoprolol succinate for the past 21 months. He presented to the emergency room with abdominal pain, cramps, and diarrhea, and developed severe pericardial pain similar to his previous episodes. He was treated with colchicine and ibuprofen, and imaging showed multiple pericardial areas of late gadolinium enhancement.
